I observed the installation of the four (4) anchor bolts, (7/8" x 24" threaded rod), for
the hold down installation at the above noted property at 7:30 a.m ., January 15,
1998. Two were placed at each side of the garage front.
The installation was done in accordance with structural detail R/SD2 as drawn on
plans dated September 18, 1997 and the instructions on the Simpson Epoxy Tie
tubes I.C.B.O. #4945. The plans onsite at the time of inspection were not stamped
as "Approved" by the City of Carlsbad.

NOTICE OF FOOTING EXCAVATION OBSERVATION
1) As per the request of Brooks Worthing, BA Worthing, Inc., we have observed the
two underpin column pad excavations in the garage area.
2) The underpin column pad excavations extend through the cap of apparent non-
expansive fill deposits and are embedded into apparent fractured meta-volcanic
rock. The fractures within the bedrock are filled with clayey soil deposits. It is our
understanding that a bearing value of 1000 pounds per square foot was used in
the design of the column pads.

3) The footing excavations are approved by this firm provided all loose deposits are
removed from the base of the excavations prior to pouring concrete. The footing
excavations were not reviewed for compliance with structural plans and location.
4) This footing notice is based on observations of the column pad excavations only.
No subsurface testing, evaluation or analysis of site development was conducted
due to the fact that the client did not chose to have a complete geotechnical
investigation. No warranty is expressed or implied.
