RE:Letter of Authorization (LOA) for QualTek Wireless LLC
QualTek Wireless LLC (QualTek) is an authorized representative of DISH and has been contracted to
perform various cellular site development activities (i.e., real estate leasing, site design/ architectural
engineering. land use entitlements, and permitting). This letter authorizes QualTek Wireless LLC to take all
necessary actions to apply for and obtain permits on behalf of Dish in support of our telecom network build-
out. QualTek Wireless is also authorized to make payments for any applications and/or permits on behalf of
Dish. However, Dish is ultimately the financially responsible party.
QualTek Wireless is not authorized by Dish to sign-off and/or accept any conditions of approval or permit
conditions. Any such sign-off and/or acceptance will need to be done by Dish.
