To view the BM P Manual, refer to the Engineering Standards (Volume 5). This questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of storm water standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your project will either be subject to 'STANDARD PROJECT' requirements, "PRIORITY DEVELOPMENT PROJECT (PDP) requirements or not considered a development project. This questionnaire will also determine if the project is subject to TRASH CAPTURE REQUIREMENTS. Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City staff has responsibility for making the final assessment after submission of the development application. If staff determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than initially assessed by you, this will result in the return of the development application as incomplete. In this case, please make the changes to the questionnaire and resubmit to the city. If you are unsure about the meaning of a question or need help in determining how to respond to one or more of the questions, please seek assistance from Land Development Engineering staff. A completed and signed questionnaire must be submitted with each development project application. Only one completed and signed questionnaire is required when multiple development applications for the same project are submitted concurrently. Selection of trash capture BM Ps must be based on the selection process described in Chapter 4. Trash capture BMPs must be verified by the City at the completion of construction. This may include requiring the project owner or project owner's representative to certify construction of the trash capture BM Ps (see Section 1.12 of the BM P Design Manual). Trash capture BM Ps must be maintained into perpetuity, and the City must confirm the maintenance (see Section 7 of the BMP Design Manual). Use this form to provide narrative description of the general strategy for trash capture BMP implementation at the project site in the box below. Then complete the trash capture BMP summary information sheet for each trash capture BMP within the project (copy the BMP summary information page as many times as needed to provide summary information for each trash capture BMP). Describe the general strategy for trash capture BMP implementation at the site. This information must describe how the steps for selecting and designing trash capture BMPs presented in Section 4.4 of the BMP Design Manual were followed, and the results (type of BMPs selected). The proposed project includes the construction of new expanded decks along the front of the two residential buildings. The decks will each have catch basins and storm drain that route flows to a junction box with a collector screen TC-BMP (2 total). The current list of Certified Trash Full Capture Systems, provided by the State Water Resources Control Board (SWRCB), was used to determine available options. Given the relatively small area requiring trash capture, the primary goal of the project was to consolidate flows to the minimum number of TC-BMPs and to propose a device that's easy to maintain. After consulting the list of approved TC-BMPs, the ADS Flexstorm Connector Pipe Screen was selected for the project and will be installed in two locations. If not designed properly, a System may impede the mosquito vector control district's ability to (1) visually inspect the System and/or storm vault for mosquito breeding, and (2) apply the appropriate chemical treatment. Moreover, some Systems may create a habitat for mosquitoes. All the Systems in the tables below have been reviewed and approved by the Mosquito Vector Control Association of California for vector control accessibility. However, prior to installation of any Systems, the local mosquito vector control district should be contacted to ensure the installation conforms to the local district's visual inspection, treatment, and vector breeding minimizing guidelines. Certified Trash Full Capture System Tables The Systems included in the tables below are: 1) new Systems certified by the State Water Board Executive Director after adoption of the Trash Provisions, and 2) legacy Systems that were certified pursuant to the Trash Provisions include those full capture systems that were listed in Appendix I of the Bay Area-wide Trash Capture Demonstration Project, Final Project Report (May 8, 2014). All Systems remain certified unless and until they are decertified by the State Water Board's Executive Director or designee. Legacy Systems' descriptions are included in "fact sheets" that have been updated to address the application requirements for new Systems. The tables do not include the Department of Transportation's Systems as its Systems are not available to the public. Multi-benefit trash treatment systems are listed separately on the State Water Board's Trash Implementation webpage at: https://www.waterboards.ca.gov/water_issues/programs/stormwater/trash_implementati on.html. Systems are either identified by their application number or, for legacy Systems, by a fact sheet alpha numeric designation. Please note: Only Systems originally manufactured or distributed by the listed applicants are certified full capture systems. The alphabetical tables of System applicants are divided into two categories: • Catch Basin Inserts and Other Insert Systems. "50% Clogged Treated Flows" utilize an orifice flow equation where the orifice coefficient (C) has a value of 0.6 and downstream head has an assumed maximum of 25% of screen height when calculating head on the centroid of screen. Screen clogging is assumed to be uniform across the height of the screen. 4. "Bypass Capacity" calculated with an installed lid/deflector height of 811 over the top of the screen and a maximum water elevation of 10" over the installed screen height. TABLE 7-6.
