To view the BMP Manual, refer to the Engineering Standards (Volume 5). This questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of storm water standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your project will either be subject to 'STANDARD PROJECT' requirements, "PRIORITY DEVELOPMENT PROJECT (PDP) requirements or not considered a development project. This questionnaire will also determine if the project is subject to TRASH CAPTURE REQUIREMENTS. Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City staff has responsibility for making the final assessment after submission of the development application. If staff determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than initially assessed by you, this will result in the return of the development application as incomplete. In this case, please make the changes to the questionnaire and resubmit to the city. If you are unsure about the meaning of a question or need help in determining how to respond to one or more of the questions, please seek assistance from Land Development Engineering staff. Selection of trash capture BMPs must be based on the selection process described in Chapter 4. Trash capture BMPs must be verified by the City at the completion of construction. This may include requiring the project owner or project owner's representative to certify construction of the trash capture BMPs (see Section 1.12 of the BMP Design Manual). Trash capture BMPs must be maintained into perpetuity, and the City must confirm the maintenance (see Section 7 of the BMP Design Manual). Use this form to provide narrative description of the general strategy for trash capture BMP implementation at the project site in the box below. Add certified trash capture device to existing storm drain structure that is connected to the storm drain. The BMP used will be the Bio Clean Bio-Grate 12x12x12 model. The trash capture bmp will be installed in the box inlet for the proposed detention basin in order to catch any trash before entering the storm drain. Trash Capture BMP Sizing: As seen on the Bio-Grate fact sheet attached to this SWQMP, the Bio-Grate-Full-12x12x12 model which was selected for this site has a treatment flow rate of 1.04 CFS, and a bypass flow rate of 1.24 CFS. Both of these allowable flow rates are greater than the flow rate which the proposed basin outlet will experience during a 100yr-6hr storm event (0.86 CFS).
