The construction, addition, or alteration of buildings is governed by the California Contractors License Law
(commencing with Business Professions Code section 7000). The law has an exemption that allows a person
who is not a California licensed Contractor to construct a single-family residential structure and engage in
home improvements and repairs, provided they meet certain requirements. (Form 8-61 provides information
about the requirements that apply to Owner-Builders makes you aware of your responsibilities and possible
risk you may incur by having a permit issued to an Owner-Builder.) Permits for multi-family projects are only
to be issued to California licensed Contractors. Refer to Form 8-16 for other frequently asked questions.

proposed (B-29 form). I/We hereby certify the construction valuation provided is accurate and

plumbing, heating, air conditioning, elevator, labor costs, and all permanent equipment and systems.
If the valuation is underestimated, the permit may be denied, unless the Applicant can show detailed
estimates to meet the approval of the Building Official.
If the Applicant is not the Property Owner, both the Property Owner and Applicant (authorized
Contractor or authorized Agent of the Property Owner/Contractor) must sign below. By signing this
declaration, the Property Owner authorizes the Applicant listed in this application to act as the
Property Owner's Agent on all matters before the city pertaining to development on this property.

approved plan. I/We agree to comply with all applicable city and county ordinances and state laws
relating to building construction. I/We assume full responsibility for the satisfactory completion of all
work authorized under the permit(s), including work performed prior to assuming control.
After construction begins, I/we understand that a height certification, pad certification, or boundary
certification may need to be submitted to the Building Inspector, when required.
I understand that a building permit application will expire If a permit has not been issued/obtained
within one year from the application date (original filing date), unless an extension of time has been
granted (Form B-25). With some limited exceptions, if a building permit application expires and there
is interest in proceeding with the same project, a new application will be required and the project will
be subject to new fees and regulations that are in effect at the time the new application is submitted.
After submitting the application, the Billing Party designated as part of the application will be sent
future city invoices for above referenced building permit(s). I understand that if a permit fee or
development impact fee refund is issued, the check will be mailed to the financial responsible party
(the actual person or company that paid the invoice). If the information on file for the financially
responsible party is outdated or inaccurate, it must be corrected before the refund can be sent. Form
8-498 may be completed to update the financially responsible party's contact information.
I/We agree to waive, release, and discharge the City of Carlsbad ("City") and its officers, elected or
appointed officials, employees, volunteers and agents (the "Indemnified Parties") from all claims and
demands, rights and causes of action of any kind with respect to the building permit application, the
purpose of which is to challenge, attack set aside, void, modify, or annul the approval or denial of the
application, or the processing thereof, except liability caused by the Indemnified Parties' sole
negligence or willful misconduct, in which case the indemnity received by the city shall be reduced by
the amount: (a) that the city's active negligence contributed to the liability on a comparative basis; or
(bl such other amount as may be required by law. This obligation is effective without reference to the
existence or applicability of any insurance coverages which may have been required by city or any
additional insured endorsements which may extend to Indemnified Parties. Property Owner(s) and
Applica nt, on behalf of themselves and all parties claiming under or through them, waive all rights of
subrogation and contribution against the Indemnified Parties while acting within the scope of their

apply to their project. This completed checklist (B-50) and summary (B-55) must be included with the building permit
application. The Carlsbad Municipal Code (CMC) can be referenced during completion of this document.

The B-50 checklist was originally developed several years ago to support implementation of the CAP. Recent updates to
the California Building Standards Code have imposed newer performance standards on building permit applications.
Therefore, the applicant is advised to review all applicable code sections and apply the maximum performance standard,
which may exceed the CAP consistency checklist requirements
Consultation with a certified Energy Consultant is encouraged to assist in filling out this document. Appropriate
certification includes, but is not limited to: Licensed , practicing Architect, Engineer, or Contractor familiar with Energy
compliance, IECC/HERS Compliance Specialist, ICC GB Energy Code Specialist, RESNET HERS rater certified, certified
ICC Residential Energy Inspector/Plans Examiner, ICC Commercial Energy Inspector and/or Plans Examiner, ICC CALgreen
Inspector/Plans Examiner, or Green Building Residential Plan Examiner.

Site Review: The developed property includes a single-story, single-family residence with a
detached 512-sf garage, and two storage sheds. The site's overall topography as well as the
proposed ADU area are relatively flat.
Project Description: The project will include construction of a new 630-sf, single-story,
detached, Accessory Dwelling Unit (ADU) to be located near the northeast corner of the
subject property (Figure 1). An existing shed will be removed and replaced with the ADU and
associated parking.
Foundations will include a shallow, continuous, 12" wide by 12" deep, reinforced concrete
perimeter footing, and a 4" thick reinforced slab-on-grade, with a 10-mil vapor barrier
centered between 2"/2" of clean sand. Earthwork will include excavation for the new
foundation, as well as shallow removal and replacement of any disturbed soils.
Geology and Geotechnical Considerations: The site area is located near the western
margin of the coastal terrain and mapped as by Old paralic deposits, Units 2 through 4,

Based on our experience in the site vicinity, a review of in-house geotechnical data, and
geotechnical reports obtained through a City of Carlsbad public records search within a 0.25-
mile radius, site soils are anticipated to consist of minor surficial materials (typically less
than 12 to 24 inches thick) overlying relatively dense Paralic deposits. The Paralic soils are
expected to exhibit very low to low expansion potential and are considered non-corrosive.
During our site visit we did not note any obvious signs of geologic hazards, groundwater, or
other geomorphic or soil related features causing distress to the property. The groundwater
table is expected to be below a depth that will influence any planned improvements.
The site is not located within any designated geologic hazard zones, including fault rupture,
landslide, liquefaction, or tsunami hazard areas. The primary geotechnical consideration for
development is the potential for strong ground shaking resulting from regional seismic
activity. The nearest active fault is the Newport-Inglewood-Rose Canyon Fault Zone
(Oceanside section), located approximately 5.2 miles southwest of the site.
Plan Review: SGS! reviewed the referenced foundation plans for consistency with the site's
geotechnical conditions. As noted in Foundation Notes 2 and 3 on Sheet Sl.0, and Detail 3 on
Sheet SD-1, the following parameters were used in the foundation design:

Conclusions and Recommendations: Based on our assessment, the soil design parameters
and seismic design values incorporated into the foundation design appropriate for the site's
conditions and underlying soils. Construction of the proposed improvements is considered
feasible from a geotechnical standpoint, provided that the recommendations presented
below are properly implemented during construction.
• Although we do not anticipate significant issues with expansive or corrosive soils
based on current information, we recommend testing the subgrade soils at the start

of foundation excavation. Should the testing reveal expansive and/or corrosive
characteristics, appropriate mitigation measures will be provided.
• The project area is expected to be underlain by minor surficial soils, possible
disturbed soils once the shed et al. is removed, and granular Old Paralic deposits.
Surficial and disturbed soils are not considered suitable for support of foundations
or additional fill and should be removed and replaced with compacted fill within all
structural areas.
• All footing excavations should be observed by the geotechnical consultant prior to
placement of reinforcing steel, to verify proper embedment into suitable soils.
• Footings shall be moisture conditioned and compacted to at least 90-percent of the
maximum dry density obtained using ASTM D1557 as a guideline.
• General earthwork and grading recommendations which include guidelines for site
preparation fill compaction, slope work, temporary excavations, and trench backfill
are provided in the attached Appendix.

GENERAL EARTHWORK AND GRADING
General
These general earthwork and grading specifications are for the grading and ear thwork shown on
the approved grading or construction plan(s) and/or indicated in the geotechnical report(s).
These Specifications shall be used in conjunction with the Geotechnical Report for the project
prepared by SGS!. In the case of conflict, the recommendations contained in the text of the
Geotechnical Report are a part of the earthwork and grading specifications and shall supersede
the provisions contained hereinafter.
Earthwork and grading should be conducted in accordance with applicable grading ordinances,
the current California Building Code, and the recommendations of this report. The following
recommendations are provided regarding specific aspects of the proposed earthwork
construction. These recommendations should be considered subject to revision based on field
conditions observed by the geotechnical consultant during grading,
Geotechnical Consultant of Record
Prior to commencement of work, the owner shall employ the Geotechnical Consultant of Record.
The Geotechnical Consultant shall be responsible for reviewing the approved geotechnical
report(s) and accepting the adequacy of the preliminary geotechnical findings, conclusions, and
recommendations prior to the commencement of grading or construction,
During grading and earthwork operations, the Geotechnical Consultant shall observe, map, and
document the subsurface exposures to verify the geotechnical design assumptions. If the
observed conditions are found to be significantly different than the interpreted assumptions
during the design phase, the Geotechnical Consultant shall inform the owner, recommend
appropriate changes in design to accommodate the observed conditions, and notify the review
agency where required. Subsurface areas to be geotechnically observed, mapped, elevations
recorded, and/or tested include natural ground, after it has been cleared for receiving fill but
before it has been placed, bottoms of all "remedial removal areas, all key bottoms, and benches
made on sloping ground to receive fill.
The Geotechnical Consultant shall observe the moisture-conditioning and processing of the
subgrade and fill materials and perform relative compaction testing of fill to determine the
attained level of compaction. The Geotechnical Consultant shall provide the test results to the
owner and the contractor on a routine and frequent basis.
The Earthwork Contractor
The Earthwork Contractor shall be solely responsible for performing the grading in accordance
with the plans a nd specifications. The Earthwork Contractor shall review and accept the plans,
geotechnical report(s), and these Specifications prior to the commencement of grading.
The Earthwork Contractor shall have the sole responsibility to provide adequate equipment and
methods to accomplish the earthwork in accordance with applicable grading codes and agency
ordinances, these Specifications, and the recommendations in the approved geotechnical
report(s) and grading plan(s). If, in the opinion of the Geotechnical Consultant unsatisfactory
conditions, such as unstable soil, improper moisture condition, inadequate compaction, adverse
weather, etc. are resulting in a quality of work less than required in these Specifications, the
.INC.
Geotechnical Consultant shall reject the work and may recommend to the owner that
construction be stopped until the conditions are rectified.
Site Preparation
Clearing and Grubbing: Site preparation includes removal of deleterious materials, unsuitable
materials, and existing improvements from areas where new improvements or new fills are
planned.
Deleterious materials, which include vegetation, trash, and debris, should be removed from the
site, and legally disposed of off-site. Unsuitable materials include loose or disturbed soils,
undocumented fills, contaminated soils, or other unsuitable materials. The Geotechnical
Consultant shall evaluate the extent of these removals depending on specific site conditions.
Earth fill material shall not contain more than 1 percent of organic materials (by volume). No fill
lift shall contain more than 5 percent of organic matter. Nesting of the organic materials shall not
be allowed.
If potentially hazardous materials are encountered, the contractor shall stop work in the affected
area, and a hazardous material specialist shall be informed immediately for proper evaluation
and handling of these materials prior to continuing to work in that area.
As presently defined by the State of California, most refined petroleum products (gasoline, diesel
fuel, motor oil, grease, coolant etc. have chemical constituents that are hazardous waste. As such,
the indiscriminate dumping or spillage of these fluids onto the ground may constitute a
misdemeanor, punishable by fine and/or imprisonment and shall not be allowed.
Any existing subsurface utilities that are to be abandoned should be removed and the trenches
backfilled and compacted. If necessary, abandoned pipelines may be filled with grout or slurry
cement as recommended by, and under the observation of, the Geotechnical Consultant.
Excavation
After clearing and grubbing of organic matter and other unsuitable material, loose or porous soils
shall be removed to the depth recommended in the Geotechnical Report. The depth of removal
and compaction should be observed and approved by a representative of the Consultant. The
exposed surface shall then be plowed or scarified to a minimum depth of 6 inches and until the
surface is free from uneven features that would tend to prevent uniform compaction by the
equipment to be used.
Remedial removal depths are estimates only. The actual extent of removal shall be
determined by the Geotechnical Consultant based on the field evaluation of exposed
conditions during grading.
All areas to receive fill, including removal and processed areas, key bottoms, and benches, shall
be observed, mapped, elevations recorded, and/or tested prior to being accepted by the
Geotechnical Consultant as suitable to receive fill. The Contractor shall obtain a written
acceptance from the Geotechnical Consultant prior to fill placement. A licensed surveyor shall
provide the survey control for determining elevations of processed areas, keys, and benches.
Slopes
Where the slope ratio of the original ground is steeper than 5:1 (horizontal:vertical), or where
recommended by the Consultant, the original ground should be benched in accordance with the
following illustration:

1) Key width "B" should be a minimum of 4-feet. The bottom of the key should be graded horizontal or inclined slightly
into the natural slope.
2) The outside of the key should be below a minimum of at least 2-feet deep into dense formational material.
3) Bench heights shall not exceed 2-foot vertical.
Fill placed on ground sloping flatter than 5:1 shall also be benched or otherwise overexcavated to
provide a flat subgrade for the fill.
Compaction of slopes shall be accomplished by backrolling of slopes with sheepsfoot rollers at
increments of 3 to 4 feet in fill elevation, or by other methods producing satisfactory results
acceptable to the Geotechnical Consultant. Upon completion of grading, relative compaction of
the fill, out to the slope face, shall be at least 90 percent of maximum density per ASTM Test
Method D1557.
Cut and fill slopes shall be a maximum of 2:1 (horizontal to vertical) unless approved by the
Geotechnical Consultant.
Planting and irrigation of cut and fill slopes and/or installation of erosion control and drainage
devices should be completed due to the erosion potential of the soil.
Fill Material
Material to be used as fill shall be essentially free of organic matter and other deleterious
substances evaluated and accepted by the Geotechnical Consultant prior to placement. Soils of
poor quality, such as those with unacceptable gradation, high expansion potential, or low
strength shall be placed in a reas acceptable to the Geotechnical Consultant or mixed with other
soils to achieve satisfactory fill material. Any import soils shall be tested fo r suitability in advance
by the project Geotechnical Engineer.
Oversize rock material, or other irreducible material with a maximum dimension greater than 8
inches, shall not be buried or placed in fill unless location, materials, and placement methods are
specifically accepted by the Geotechnical Consultant. Placement operations shall be such that
nesting of oversized material does not occur and such that oversize material is completely

surrounded by compacted or densified fill. Oversize material shall not be placed within 10
vertical feet of finish grade or within 2 feet of future utilities or underground construction.
Compaction
All fill and backfill associated with the proposed construction should be placed at a minimum
relative compaction of 90% of the material's maximum dry density, and slightly above optimum
moisture content, using equipment capable of producing a uniformly compacted product
throughout the entire fill lift.
Fill materials below optimum moisture content should be moistened and mixed to achieve a
uniform condition above optimum moisture. Overly wet fill materials can be aerated by blading
or other suitable methods or mixed with drier materials to reach the required moisture content.
The fill and backfill should be placed in horizontal lifts with a loose thickness not exceeding 8
inches, appropriate for spreading, mixing, and compacting with the available equipment.
Retaining wall backfill should consist of granular material (maximum particle size :5 3 inches)
with an expansion index (EI) of no greater than 50 and a sand equivalent (SE) greater than 30.
No fill materials shall be placed during unfavorable weather conditions. If work is interrupted by
inclimate, fill operations should not resume until the Geotechnical engineer confirms through
field testing that the moisture content and density of the fill comply with the specified
requirements. Adherence to these procedures will ensure proper compaction and long-term
stability of the fill.
Compaction Testing
Field-tests for moisture content and relative compaction of the fill soils shall be performed
by the Geotechnical Consultant. Location and frequency of tests shall be at the Consultant's
discretion based on field conditions encountered. Compaction test locations will not
necessarily be selected on a random basis. Test locations shall be selected to verify
adequacy of compaction levels in areas that are judged to be prone to inadequate
compaction (such as close to slope faces and at the fill/bedrock benches).
Tests shall be taken at intervals not exceeding 2 feet in vertical rise and/or 1,000 cubic
yards of compacted fill soils embankment. In addition, as a guideline, at least one test shall
be taken on slope faces for each 5,000 square feet of slope face and/or each 10 feet of
vertical height of slope. The Contractor shall assure that fill construction is such that the
testing schedule can be accomplished by the Geotechnical Consultant. The Contractor shall
stop or slow down the earthwork construction if these minimum standards are not met.
Temporary Excavations
Temporary excavation shall be made no steeper than 1:1 (horizontal to vertical). The
recommended slope for temporary excavations does not preclude local raveling and sloughing.
Where wet soils are exposed, flatter excavation of slopes and dewatering may be necessary. In
areas of insufficient space for slope cuts, or where soils with little or no binder are encountered,
shoring shall be used.
All large rocks exposed above temporary cuts shall be removed prior to foundation excavation. In
addition, any rocks exposed during development from raveling and sloughing should be removed
immediately.

All excavations should comply with the requirements of the California Construction and General
Industry Safety Orders and the Occupational Safety and Health Act and other public agencies
having jurisdiction.
Trench Backfill
Exterior trenches, paralleling a footing and extending below a 1:1 plane projected from the
outside bottom edge of the footing, s hall be compacted to a minimum of 95-percent per ASTM
D1557. All trenches in structural areas and under concrete flatwork shall be compacted to a
minimum of 95-percent per ASTM D1557. All trenches in non-structural areas shall be
compacted to a minimum of 85-percent per ASTM D1557.
All material used for trench backfill shall be approved by the Geotechnical Engineer prior to
placement. All bedding and backfill of utility trenches shall be done in accordance with the
applicable provisions of Standard Specifications of Public Works Construction. Bedding material
shall have a Sand Equivalent greater than 30 (SE>30). The bedding shall be placed to 1-foot over
the top of the conduit and densified by jetting. Backfill shall be placed and densified to a
minimum of 95-percent of maximum from 1-foot above the top of the conduit to the surface.
Lift thickness of trench backfill shall not exceed those allowed in the Standard Specifications of
Public Works Construction unless the Contractor can demonstrate to the Geotechnical Consultant
that the fill lift can be compacted to the minimum relative compaction by his alternative
equipment and method. The Geotechnical Consultant shall test the trench backfill for relative
compaction. At least one test should be made for every 300 feet of trench and 2 feet of fill.
Regulations of the governing agency may supersede the above, and all trench excavations should
conform to all applicable safety codes. The Contractor shall follow all OSHA and Cal/OSHA
requirements for safety of trench excavations.
Subdrain Installation
Subdrain systems shall be installed in accordance with the approved geotechnical report(s), the
grading plan, and the Standard Details. The Geotechnical Consultant may recommend additional
subdrains and/or changes in subdrain extent, location, grade, or material depending on
conditions encountered during grading.
All subdrains shall be surveyed by a land surveyor/civil engineer for line and grade after
installation and prior to burial. Sufficient time should be allowed by the Contractor for these
surveys.

INSTRUCTIONS:
To address post-development pollutants that may be generated from development proje , th efty requires that new
development and significant redevelopment priority projects incorporate Permanent Stor Water Best Management
Practices (BMPs) into the project design per Carlsbad BMP Design Manual (BMP Manual). To view the BMP Manual, refer
to the Engineering Standards (Volume 5).
This questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision,
discretionary permits and/or construction permits). The results of the questionnaire determine the level of storm water
standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your
project will either be subject to 'STANDARD PROJECT' requirements, "PRIORITY DEVELOPMENT PROJECT (PDP)
requirements or not considered a development project. This questionnaire will also determine if the project is subject to
TRASH CAPTURE REQUIREMENTS.
Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City
staff has responsibility for making the final assessment after submission of the development application. If staff determines
that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than initially assessed
by you, this will result in the return of the development application as incomplete. In this case, please make the changes to
the questionnaire and resubmit to the city.
If you are unsure about the meaning of a question or need help in determining how to respond to one or more of the
questions, please seek assistance from Land Development Engineering staff.
A completed and signed questionnaire must be submitted with each development project application. Only one completed
and signed questionnaire is required when multiple development applications for the same project are submitted
concurrently
