On behalf of Verizon Wireless, we are submitting a Consistency Determination application to
allow for antenna modification at their existing site. The project site is located at 5220 Sunny
Creek Road.
Verizon is proposing to relocate six (6) previously approved antennas onto new mounting
brackets, remove and replace three (3) Raycaps and add six (6) new RRU's on the existing
tower. No changes are proposed to the existing equipment shelter. The underlying property is
owned by the Carlsbad Municipal Water District and is currently zoned Open Space/Local
Facilities Management.
Included with this submittal are the following documents:
• 2-full size sets of ZD's
• 1-11"x17"setsofZD's
• 1-Signed application form (Consistency Determination)
• 1 Photo Simulation
• 1 -Authorized Agent letter from VZW
