Dear Mr. McMenamin:
Pursuant to the request of Mr. Nasser Sionit, Ph.D. (Environmental Health Specialist,
Department of Environmental Health) and your verbal authorization, GeoSoils, Inc. (GSI)
is providing the following addendum to our agricultural chemical residue survey report on
the Hadley property in Carlsbad (establishment No. H38426). The purpose of this
addendum was to evaluate the recommended burial depths of agricultural chemical
residue-affected soils provided in the subject survey report, relative to depth to
groundwater on the subject property. The information provided herein is based upon
information provided by Mr. Sionit in a meeting on October 7,1999, as well as subsurface
data collected from a boring drilled on the site on October 11,1999.
Scope of Work
In fulfillment of the San Diego County Department of Environmental Health (DEH)
requirements regarding the installation of test borings/groundwater monitoring wells, GSI
submitted a permit application to the DEH on October 8,1999. Due to time constraints of
the subject property transaction, Mr. Michael Vernetti (Supervisor Environmental Health
Specialist) reviewed the permit application and provided the test boring permit over the
counter the same day (permit No. W98518). A copy of the permit application and boring
permit is provided in Appendix A.
On October 11,1999, a licenced well drilling contractor, West Hazmat Drilling Corporation
(C57 No. 554979), drilled a 511/2 foot deep boring (B-1) in the east-central portion of the
subject site. Due to the surface of the site being soft and loose from recent discing
operations, access to the boring location as presented in the permit application was
unattainable. The revised location is provided in Figure 1.
The eight-inch diameter boring, drilled with a hollowstem auger, encountered medium
dense to dense Terrace Deposits consisting of slightly moist, silty sands and silty clayey
sands. A log of the boring (B-1) is provided herein as Appendix B.
Since the cuttings displayed no physical signs of contamination (i.e., no noxious odors, no
discoloration), cuttings were placed on the surface of the property, down slope from the
boring. Prior to backfilling the boring with a bentonitic slurry, a waiting period occurred to
allow any seepage to accumulate. After 1/2 hour, there were no signs of subsurface water
in the boring.
Conclusions and Recommendations
Based upon the absence of groundwater encountered in the deep boring recently drilled
on the Hadley property, it is our opinion that the initial survey report recommendations
remain applicable.
Data submitted herein is consistent with conclusions and recommendations provided in
the initial report. Based upon the information collected by GSI during this survey, further
studies or action, other than the recommendations provided in the initial survey reports
(dated October 8,1999), are not proposed from an environmental viewpoint, at this time.
LIMITATIONS
GSI has performed the services for this project in accordance with the terms of a contract
between GSI and Client and in accordance with current professional standards for
investigations of this type. The conclusions presented in this report are based on the
information collected during the study, the present understanding of the site conditions,
and professional judgment.
Please note, subsurface conditions may vary throughout the property from those
encountered in subsurface excavations, and/or information provided in historical
documents reviewed by GSI. The interpretations and recommendations of GSI are based
solely on such information, and/or information supplied by Client. Findings of this
assessment based on data provided by others carries no warranty, expressed or implied,
as a result of the usage of such data.
It is possible that future assessments may reveal additional data or variations of the current
data which may require the current conclusions and recommendations to be reevaluated.
As a result, GSI makes no warranty, either express or implied, as to its findings, opinions,
recommendations, specifications, or professional advice except that they were
promulgated after being prepared in accordance with generally accepted standards of care
and diligence normally practiced by recognized consulting firms performing services of a
similar nature.

INSPECTIONS
Submit one (1) original and two (2) copies of this application package, including plan drawings with the required fee
to the Department of Environmental Health, Site Assessment and Mitigation Program (SAM). 1255 Imperial Avenue,
3rd Floor, San Diego, CA 92101. Or mail to P. 0. Box 129261, San Diego, CA 92112-9261. Checks should be
made payable to the County of San Diego.
A permit will be issued by SAM upon review and approval of the application and plans. The required fees must be
submitted with the application package. Information in addition to that presented in the application package may be
needed in order to obtain final approval. No work is to begin on the proposed project until a permit has been issued.
The required inspections cannot be scheduled until a permit is issued.
Once the permit has been issued, it is the responsibility of the permittee to notify SAM at least two (2) working days
in advance to schedule each required inspection.

A proposal has been made to install one or more monitoring wells on your property. Monitoring wells are
typically installed to gather data for environmental investigations. This letter is to inform you of your
responsibilities for monitoring wells installed on your property.
Whether or not you obtain the permit or are responsible for installing the monitoring wells you may be held
responsible for the maintenance and destruction of the wells on your property.
Section 67.43 1 of San Diego County Code states that "Whenever the Director of DEH detennines that an
abandoned or other -wefl is causing a nuisance by polluting or contaminating ground water, or constitutes a
safety hazard, he may issue a written order requiring that the conditions productive of the nuisance be abated "~
within a period often days thereafter and shall forthwith serve the order upon the person occupying the
premises, if any, and if no person occupies the premises, the order shall be posted upon said premises in a
conspicuous place. In addition, a copy shall be mailed to the owners of the premises as their names and
addresses appear upon the last equalized assessment role."
