ENVIRONMENTAL INFORMATION FORM
This Environmental Information Form will be used to assist staff in determining what type of
environmental documentation (i.e., Environmental Impact Report, Mitigated Negative
Declaration, Negative Declaration or Exemption) will be required to be prepared for your
application, per the California Environmental Quality Act (CEQA) and Title 19 of Carlsbad's
Municipal Code. The clarity and accuracy of the information you provide is critical for purposes
of quickly determining the specific environmental effects of your project.
Any environmental studies (i.e., biological, cultural resource, traffic, noise) that are
necessary to substantiate a "no impact" or "yes impact" determination should be submitted
as an attachment to this Environmental Information Form. This is especially important
when a Negative Declaration is being sought. The more information provided in this form, the
easier and quicker it will be for staff to complete the Environmental Impact Assessment Form -
Initial Study.

St. Elizabeth Seton CUP Amendment
32. The site has been developed as a Church for the past two decades. The
existing Church includes a 17,568 square foot Sanctuary and 5,682 square
foot Education Building for a total existing square footage of 23,250 as well
as two separate parking lots. The site gently slopes from east to west.
There is no sensitive vegetation on-site. Road improvements on
surrounding roadways are complete.
The proposed project will not result in any new significant environmental
impacts that were not analyzed in the prior environmental review. Please
see attached photos of the project site.
33. The St. Elizabeth Seton site is located at 6628 Santa Isabel Street at the
southwest corner of Alga Road and El Fuerte Street. The surrounding land
uses consist of various multi-family residences, which are consistent with
the RD-M zone. The site was originally landscaped with CUP 139. There
are no known animals, cultural and historical aspects on this site.
The proposed modifications would be consistent with the General Plan. The
proposed project will comply with all of the development standards
associated with the RD-M zone. Setbacks: Front - 35 feet 1 inch, Street
Side (Santa Isabel) - 24 feet 1 inch, Street Side (Alga) - 23 feet 6 inches,
Rear - 24 feet 1 inch. Please see attached photos of the project vicinity.

ENVIRONMENTAL QUALITY ACT (CEQA) APPLICABILITY/PROCESS
DETERMINATION
This is to advise you that after reviewing the application for the project referenced above, the
City has determined that the following environmental review process (pursuant to CEQA) will be
required for the project:
The project is exempt from the provisions of CEQA, pursuant to CEQA Guidelines Section
15332 (In-Fill Development Projects) in that the project is compatible with the General Plan
Land Use designation as well as the Zoning regulations, occurs within the City limits on a
project site of no more than five acres substantially surrounded by urban uses, has no value as
habitat for endangered, rare or threatened species, would not result in any significant effects
relating to traffic, noise, air quality, or water quality, and the site can be adequately served by all
required utilities and public services. No other environmental review is required for the project.
A Notice of Exemption will be filed upon a decision of the project with the San Diego County
Clerk's Office which involves a filing fee. Please contact the project planner regarding the
timing of the filing fee.
