SUBJECT: Extension of CUP 194(A) & (B)
Thank you for applying for Land Use Permits in the City of Carlsbad. The Planning Department
has reviewed your Conditional Use Permit, application no. 's CUP 194(A)x1 and CUP 194(B)xI, as
to their completeness for processing.
Please review and provide the following list of items required by the Engineering Department to
complete their review of the subject application:

Site plans showing current and proposed facilities.
An overview of the entire process. Include a time line diagram or a table showing
time required for each stage.
Describe the product, to whom sold (including percentages) and delivery.
An assessment of the health hazards vis a vis the Health Department of the product
as used by the purchaser. This assessment should be shown in relation to the
water quality and health characteristics of the culture medium.
Any mitigation measure that may need to be taken as a result of the water quality
or health characteristics of the culture medium.
Provide a financial statement of the operation.
Provide a breakdown of financial impacts of any mitigation measures taken under
item 5 above.
Any recent correspondence with the Health Department regarding the issue of
coliform testing and associated health risks.
Copy of the S.D.G.& E. lease agreement.
