Conditional Use Permit Application Form. 3. 300-foot radius map. 4. Property owners list. 5. Photographs, drawings or other supplemental information. 1. Ordinance No. 9060 requires a filing fee for this type of application of $50.00. For list of uses requiring Application, see Article 14. 2. The Conditional Use Permit Application Form must be filled out completely with full answers to every statement. The Application must be signed before a Notary Public or the Filing Clerk by the owner or owners, lessee or lessees. Signatures of adjacent property"owners who approve the request may be signed in the space provided on Page 2. If space is not sufficient, a supplemental sheet may be added to the petition. Signatures are desirable but are not absolutely required. 3. The 300-foot radius map accompanying the application must be a dark line print or ink tracing to a scale of not less that 1"= 200' and must be correctly prepared. It must show each lot within 300 feet of the exterior boundaries of the property involved and the uses of each lot or present type of occupancy, such as single-family residence, duplex, apartment house, business building or type of industrial use or agricultural use, or if vacant. The various zones surrounding subject property must also be distinguished. Each lot must be consecutively numbered to correspond to owner's name on Property Owners List (as explained below). 4. The Property Owners List should be typewritten and must include the owner's name, mailing address and legal description of each lot shown on the aforesaid map. Each owner's name on this list must be numbered to correspond with the numbering placed on the aforesaid map. the subjects under discussion are always helpful to the Planning Commission and are suggested as exhibits with this Application. 5.
