Box 8735 Mammoth Lakes, CA 93546 SUBJECT: HDP 02-1O/CDP 02 42N 02-04 - DUNN RESIDENCE Dear Jim: I have enclosed a Disclosure Statement form that is required for the staff report I have prepared for the subject project. As you are aware, the project has been scheduled for the April 21, 2004 Planning Commission public hearing. The Disclosure Statement must be filled out, signed and returned as soon as possible. The Planning Commission is obligated to review this to ensure that they have no conflict of interest before making a decision regarding your project. I am also enclosing a Land Use Review Application for your signature. The application is for an administrative Minor Coastal Development Permit required for the second dwelling unit. There is no charge for the application. The second dwelling unit requires administrative approval by the Planning Director after the Planning Commission has made a decision on your project. Staff has already determined that the second dwelling unit is consistent with our second dwelling unit regulations. To expedite receipt of the disclosure statement, I am requesting that you fax it to me, if possible, so that I can attach to the staff report that is currently going through the approval cycle in preparation for the public hearing. The Planning Department FAX number is (760) 602- 8559. Finally, your application for an administrative variance is not required. Please submit a letter requesting withdrawal of that application (VO2-04) and a refund of the fees. I will then initiate the refund request.
