INSTRUCTIONS:
To address post-development pollutants that may be generated from development projects, the City requires that new development and
significant redevelopment priority projects incorporate Penmanent Storm Water Best Management Practices (BMP's) into the project
design per the City's Standard Urban Stormwater Management Plan (SUSMP). To view the SUSMP, refer to the Engineering Standards
(Volume 4, Chapter 2).
Initially this questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision,
discretionary pemnits and/or construction permits). The results of the questionnaire determine the level of storm water standards that
must be applied to a proposed development or redevelopment project. Depending on the outcome, your project wiil either be subject to
'Standard Stomiwater Requirements' or be subject to additional criteria called 'Priority Development Project Requirements'. Many
aspects of project site design are dependent upon the stonrn water standards applied to a project.
Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City staff has
responsibility for making the final assessment after submission of the development application. If staff detenmines that the questionnaire
was incorrectiy filled out and is subject to more stringent stonm water standards than initially assessed by you, this will result in the return
of the development application as incomplete. In this case, please make the changes to the questionnaire and resubmit to the City.
If you are unsure about the meaning of a question or need heip in detemiining how to respond to one or more of the questions, please
seek assistance from Land Development Engineering staff.
A separate completed and signed questionnaire must be submitted for each new development appiication submission. Only one
completed and signed questionnaire is required when multiple development applications forthe same project are submitted concurrently.
In addition to this questionnaire, you must also complete, sign and submit a Project Threat Assessment Form with construction penmits
for the project
Please start by completing Step 1 and follow the instructions. When completed, sign the form at the end and submit this with your
application to the city.

To detemiine if your project is a pnonty development project, please answer the following questions:
YES NO
1. Is your project LIMITED TO constructing new or retrofitting paved sidewalks, bicycle lanes or trails that meet
the following criteria: (1) Designed and constructed to direct stomri water runoff to adjacent vegetated areas, or
other non-erodible pemieable areas; OR (2) designed and constaicted to be hydrauiicaily disconnected from
paved streets or roads; OR (3) designed and constructed with pemieable pavements or surfaces in
accordance with USEPA Green Streets quidance?
2. Is your project LIMITED TO retrofitting or redeveloping existing paved alleys, streets, or roads that are
designed and constmcted in accordance with the USEPA Green Streets guidance? <
if you answered "yes" to one or more of the above questions, then your project is NOT a priority development project and therefore is
NOT subject to the storm water criteria required for priority development projects. Go to step 4, mark the last box stating "my
project does not meet PDP requirements" and complete applicant information.
If you answered "no" to both questions, then go to Step 2.

PROJECTNAME: Banfield Veterinarv Clinic #5113 - Cartsbad. CA
APPLICANTNAME: MEDICAL MANAGEMENT INTL. INC. DBA BANFIELD PET HOSPITAL
Please describe fully the proposed project by application type. Include any details necessary to
adequately explain the scope and/or operation of the proposed project. You may also include
any background information and supporting statements regarding the reasons for, or
appropriateness of, the application. Use an addendum sheet if necessary.
Description/Explanation:
Banfield Introduction:
Since 1955 Banfield has been providing high quality neighborhood veterinarian care throughout the United States.
With over 900 clinics nationwide, Banfield is the leader in innovations like Optimum Wellness Plans, and the first
and most extensive quality-assurance program in the industry. Banfield hospitals / clinics provide the finest, most
convenient human-quality medical services available, as well as industry-leading protocols and equipment. We
understand the special bond between pets and people. At Banfield, we provide a level of service second to none
in the industry.
Project Description:
Banfield Pet Hospital is proposing a 3,361 s.f +- veterinary clinic within the existing Plaza Paseo Real Shopping
Center in Carlsbad California. No outdoor facilities or changes are proposed other than tenant signage. Typical
Banfield Veterinary facilities such as what is being proposed at this location, include full service veterinarian care,
including diagnostic capabilities, a surgery suite, pharmacy and retail pet supply sales. All clinic activities occur
within the tenant space. We do not provide outdoor amenities such as dog runs or storage yards and no grooming,
long term kenneling, boarding or overnight facilities are provided at Banfield. The proposed veterinarian clinic will
primarily service the neighboring community providing a necessary service at a close and convenient location.
Located within the Plaza Real Shopping Center, Banfield reduces transportation time for neighbors / clients and
provides a location to receive needed veterinarian care services at a convenient one-stop shopping center
location.
Hours of operation:
Banfield clinics typically operate 7:00am - 7:00pm, 7 days per week.
No overnight care or boarding.
Employees:
Although staffing will vary depending on workload, typical Banfield clinics operate with a staff of 4 to 7, including
Licensed Veterinarians, Veterinary techs and administrative support staff.
Previous Use:
Banfield will be occupying the existing "Kidville" tenant space within an existing strip retail center. The space is an
"end-cap" space immediately adjacent to a Karate studio to the East. Von's Grocery store is across the parking lot
to the North, a US Post office is across the parking lot to the West and Aviara Parkway is immediately to the
South.
Project Description - Form P-1(B) continued
Noise & nuisance:
As mentioned, Banfield does not provide long term boarding, overnight stays or outdoor dog runs. All functions are
fully contained within the premises minimizing impact on adjacent tenants or properties. Any potential sound
transmission from within the premises, is mitigated thru double wall sound insulated demising walls providing
acoustic separation between our space and adjacent tenants. For animal safety reasons, Banfield does not "walk
animals" outdoors, but does provide a pet-waste station with bags and disposal adjacent to the clinic entry for
customer and employee use to clean up pet waste if were to occur while arriving or leaving the facility.
With over 900 faciiities nationwide, Banfield prides itself on being good neighbors and tenants.
As we have been thru dozens of conditional use applications across the country, we are very familiar with typical
concerns. Significant conditions are not typically applied to our proposed Banfield clinics as our impact on adjacent
tenants / neighboring properties is minimal with little or no impact on public peace, safety and welfare. With this
said Banfield is committed to being a "good neighbor" and working closely with the community and city jurisdiction
to alleviate any concerns.

INSTRUCTION SHEET FOR COMPLETING AN
ENVIRONMENTAL IMPACT ASSESSMENT
INFORMATION FORM
This Environmental Infonnation Form will be used to assist staff in determining what type of
environmental documentation (i.e.. Environmental Impact Report, Mitigated Negative
Declaration, Negative Declaration or Exemption) will be required to be prepared for your
application, per the California Environmental Quality Act (CEQA) and Title 19 of Carisbad's
Municipal Code. The clarity and accuracy of the information you provide is critical for purposes
of quickly determining the specific environmental effects of your project.
Any environmental studies (i.e., biological, cultural resource, traffic, noise) that are
necessary to substantiate a "no impact" or "yes impact" determination should be
submitted as an attachment to this Environmental Information Form. This is especially
important when a Negative Declaration is being sought. The more information provided in this
form, the easier and quicker it will be for staff to complete the Environmental Impact
Assessment Form - Initial Study.

DBA Banfield Pet Hospital

Portland, OR 97213-6655
SUBJECT: MCUP 15-18 - BANFIELD PET HOSPITAL - Request for approval of a Minor Conditional Use
Permit (MCUP 15-18) to allow for the operation of a veterinary hospital located at 6955 El
Camino Real, in the Local Shopping Center (C-L) Zone and Locai Facilities Management Zone 6.
Dear Phil Reis,
The City Planner has completed a review of your application for a Minor Conditional Use Permit (MCUP
15-18) to allow for the operation of a veterinary hospital at 6955 El Camino Real, Suites B101/B103. A
notice was sent to property owners within a 300' radius of the subject property requesting comments
regarding the above request. No comments were received within the ten day notice period (ending on
October 23, 2015). After careful consideration of the circumstances surrounding this request, the City
Planner has determined that the four findings required for granting a Minor Conditional Use Permit can
be made and therefore, approves this request based on the following findings and conditions.
Findings:
1. That the requested use is necessary or desirable for the development of the community, and is in
harmony with the various elements and objectives of the general plan, including, if applicable, the
certified local coastal program, specific plan or master plan in that the proposed veterinary hospital
will provide a necessary and desirable service to the residents of Carlsbad. The proposed veterinary
hospital use is consistent with the General Plan, in that the C-L Zone, which implements the Local
Shopping Center (L) General Plan Land Use designation, allows veterinary hospitals as a
conditionally permitted use.
2. That the requested use is not detrimental to existing uses or to uses specifically permitted in the zone
in which the proposed use is to be located in that the proposed veterinary hospital is a conditionally
permitted use in the Local Shopping Center (C-L) Zone and is compatible with the surrounding uses.
The existing shopping center provides adequate parking on-site and the site provides for traffic
circulation. Furthermore, the proposed veterinary hospital is proposed to be located within an
existing building approved under SDP 86-ll(A) and only tenant improvements to the interior are
required for the use.

3. That the site for the proposed conditional use is adequate in size and shape to accommodate the
yards, setbacks, walls, fences, parking, loading facilities, buffer areas, landscaping and other
development features prescribed in this code and required by the City Planner in order to integrate
the use with other uses in the neighborhood in that the proposed veterinary hospital will occupy
two suites totaling 3,361 square feet in an existing shopping center with existing onsite
landscaping and requires no site alterations or additional amenities. Furthermore, the existing
building complies with all of the required development standards of the C-L zone and 3,361
square foot lease space is adequate in size and shape to accommodate the proposed veterinary
hospital as shown on Exhibits "A" - "C".
4. That the street system serving the proposed use is adequate to properly handle all traffic generated
by the proposed use in that the impacts of future commercial tenants on the existing street
system were evaluated and addressed through the approval of a Site Development Plan, SDP 86-
11(A), which approved the commercial shopping center in which the veterinary hospital is
proposed to be located.
5. That the City Planner has determined that the project belongs to a class of projects that the State
Secretary for Resources has found do not have a significant impact on the environment, and it is
therefore categorically exempt from the requirement for the preparation of environmental
documents pursuant to Section 15301(a), Existing Facilities, of the state CEQA Guidelines. In
making this determination, the City Planner has found that the exceptions listed in Section 15300.2
ofthe state CEQA Guidelines do not apply to this project.
6. The City Planner has reviewed each of the exactions imposed on the Developer contained in this
letter, and hereby finds, in this case, that the exactions are imposed to mitigate impacts caused by
or reasonably related to the project, and the extent and the degree of the exaction is in rough
proportionality to the impact caused by the project.
Conditions:
1. Approval is granted for MCUP 15-18 as shown on Exhibits "A" - "C" dated October 27, 2015 on file
in the Planning Division and incorporated herein by reference. Development shall occur
substantially as shown unless otherwise noted in these conditions.

further condition issuance of all future building permits; deny, revoke, or further condition all

on the approved Exhibits. Any proposed development different from this approval shall require an
amendment to this approval.

hearing and after providing the permittee the opportunity to be heard, the City Planner may revoke
and terminate the minor conditional use permit in whole or in part, reaffirm the minor conditional
use permit, modify the conditions or impose new conditions.
8. This Conditional Use Permit is granted without an expiration date. This permit may be revoked at
