B. Materials the sign(s) will be constructed of.
C. Proposed sign copy.
APPLICANT MUST SUBMIT THREE (3) SETS OF SIGNBITE PLANS, A COMPLETED
APPLICATION FORM, AND THE APPLICATION FEE.

Enclosed please tind ( 1 approved copy of Chandler Signs Design #C 1 2480R 1 of the
proposed signage for the above noted location.
Please forward a copy of the design along with a Certificate of Insurance should Chandler
proceed with the scheduling of installing the sign. The Certificate of Insurance must be
received by our office along with a copy of the design prior to scheduling of the install.
A sample insurance certificate is enclosed for your reference.
