The following supplemental information must accompany the completed application, according to project type.
1. ALL PROJECTS, EXCLUDING SIGNS:
\E\ A completed Environmental Impact Assessment Form - Part I.
S The following Items must be provided once it has been determined that the application is complete:
Radius Map, Two separate sets of the Property Owners List and Labels. The property owner list must
be typed and contain the names and addresses of all property owners within a 300 feet radius of the
subject property (Including the applicant and/or owner). The list shall Include the San Dlego County
Assessor's parcel number from the latest assessment rolls. Except for single family residences, the
apartment or suite number must be included on the address mailing labels for property owners within
300 feet radius of subject property; the parcel number should not be typed on the address labels. The
radius map must be provided with a scale not less than 1" = 200' showing each lot within 300 feet of the
exterior boundaries of the subject property.
2. NEW CONSTRUCTION OF BUILDING(S) or ADDmON(S) TO THE BUILDING FOOTPRINT;

_ Existing and proposed topographic contour lines.
_ Existing landscaping and proposed landscaping; please indicate landscaping to be removed, if
any.
13 Four (4) copies of building elevations and floor plans for all buildings on the site, identify new building or
addition, prepared on 24" x 36" sheets folded to QVk" xll" size.

10. FINDINGS AND CONDITIONS OF APPROVAL (IF APPLICABLE) Page 1 of 3
The following findings have been made in order to approve the Administrative Redevelopment Permit
for property located at 2780 Carlsbad Boulevard:
1. The Housing and Redevelopment Director has determined that the project is consistent with the
policies, goals and action programs set forth within the Carlsbad General Plan.
2. The Housing and Redevelopment Director has determined that the project is consistent with all
applicable development standards and land use policies set forth within the Village Master Plan
and Design Manual and Village Redevelopment Plan.
3. The Housing and Redevelopment Director has determined that the project is exempt from the
requirements of the California Environmental Quality Act (CEQA) per Section 15301(3)(c) of the
State CEQA Guidelines as a conversion of an existing small structure from one use to another
with minor exterior and interior renovations.

The following conditions hav^ffeen approved for the subject projecV^
1. This approved permit serves as the required redevelopment and coastal development permit for
the subject project, approving a change in land use from commercial/retail to restaurant.
2. The project shall be completed according to, and consistent with, the plans approved by the
Housing and Redevelopment Director on the date noted above. The approved plans have been
stamped by the Housing and Redevelopment Director and indicate the approval date of 04/21/09.
3. Developer shall obtain appropriate and applicable building permits to allow for all building
improvements required for conversion of commercial/retail to restaurant land and building use.
4. The operational hours of the approved restaurant shall be limited to those proposed by the
applicant, 11:00 AM to 10:00 PM. The hours may not be extended without an amendment to this
permit.
5. Developer shall comply with the City's Stormwater Regulations, latest version, and shall
implement best management practices at all times. Best management practices include but are
not limited to pollution treatment practices or devices, erosion control to prevent silt runoff
during construction, general housekeeping practices, pollution prevention and educational
practices, maintenance procedures, and other management practices or devices to prevent or
reduce the discharge of pollutants to stormwater, receiving water or stormwater conveyance
system to the maximum extent practicable.
6. Developer shall complete and submit to the City Engineer a Project Threat Assessment Form
(PTAF) pursuant to City Engineering Standards. Concurrent with the PTAF, the developer shall
also submit the appropriate Tier Level Storm Water Compliance Form and Tier Level Storm Water
Pollution Prevention Plan (SWPPP) as determined by the completed PTAF all to the satisfaction
ofthe City Engineer.
7. An adjustment plat application shali be submitted to the Engineering Department within 90 days
of the approval date of this Administrative Redevelopment Permit, or the applicant may submit a
certificate of compliance to the Engineering Department if the property is one legal lot.
8. The semi-trailer currently stored at the rear of the property shall be removed prior to Building
Permit issuance and shall not be stored on the site at any time.
9. The parking lot shall be striped per the approved project plans.
10. Per the State Department of Alcohol Beverage Control, the sales, service and consumption of
alcoholic beverages shall be permitted only between the hours of 9:00 AM and 11:00 PM each
day of the week. These hours, however, are modified by condition #4, noted above, which limits
the operational hours of said business.
11. There shall be no live entertainment or dancing permitted on the premises.
12. There shall be no pool or billiard tables, amusement machines or video game devices in the
premises at any time.
13. No sounds, including but not limited to loud speakers, sound systems, or other types of music or
noise from the operation of the business shall be audible beyond the boundaries of the
premises.

15. A separate sign permit shall be required prior to the installation of any signs advertising the
proposed use.
16. Before engaging in the proposed use, the applicant shall obtain the appropriate business license
from the City of Carlsbad.
17. The applicant shall implement an appropriate parking management plan, which may include valet
parking and requirements for employees to park in public parking lots, for said business to
reduce parking impacts on neighboring properties.
18. This permit shall be subject to revocation if the applicant fails to conform to any of the above
conditions of approval.
