Regional Vice President Subject: Confirmation Proposal for Geotechnical Services Distress to Roadway Median College Boulevard Extension Carlsbad, California Gentlemen: 1. INTRODUCTION a) As requested, we are submitting this confirmation proposal to provide geotechnical services related to the median distress areas along the subject College Boulevard Extention between Palomar Airport Road and El Camino Real in Carlsbad, California. b) The areas of distress were noted by the Resident Engineer, Kurt Gierlich with NBS/Lowery. Robert Russell and Kurt Gierlich, in order to observe the reported distress and determine the scope of this proposal. 2. PURPOSE AND SCOPE a) The purpose of our proposed distress evaluations and shallow investigation will be to review the probable cause(s), obtain subsurface soil data, and define corrective recommendations. b) The scope of our services will include: i) Preliminary planning and preparation; ii) Review of previous and pertinent reports and plans submitted to us; iii) Examining and evaluating the more severe and typical examples of distress; iv) Performing field exploration with 18 test borings and subsurface soil sampling along the full roadway alignment and conducting pertinent laboratory testing, which will include moisture-densities, expansion tests, sieve analyses, compaction tests and a lime- treated series of soil tests. v) Geotechnical engineering analyses of the previous and current field and laboratory data; vi) Preparation of a report presenting our findings, conclusions and recommendations. City of Carlsbad May 2, 1988 Project 3814-05 Page Three FEES a) Our fees are estimated on a time and materials basis in accordance with the accompanying Fee Schedule b) The cost breakdown is as follows: i) Pre-exploration field observations, fee proposal and possible meeting with City $2,500.00 ii) Field Exploration 2,500.00 iii) Laboratory Testing 3,000.00 iv) Engineering and Analysis 1,000.00 v) Drafting and Reproduction 300.00 $9,300.00 Add for Contingencies 700.00 $10,000.00 c) We will not incur expenditures above $10,000.00 without prior authorization. d) Fees for professional services beyond the scope of this confirmation proposal, as outlined herein, such as for meetings with the client, or his designated representative, or responses to governmental agencies, or additional subsurface investigations, would be accrued and charged on a time and materials basis in accordance with our current Fee Schedule. These amounts would be pre-approved and billed separate to the above noted fee. SCHEDULE a) We commenced our field work on April 29, 1988, in accordance with Mr. Russell's verbal authorization. b) Contingent on the laboratory work being completed on schedule, we are planning to have a report available on May 20, 1988. MISCELLANEOUS a) In the event that unforeseen conditions are encountered during the process of our work that materially would alter our scope, you would be advised and arrangements would be made for payment of any excess changes incurred. b) Any minor damage to concrete flatwork, plants or utilities in the respective median areas during the course of our investigation will be the client's responsibility. c) As requested, we are also submitting a copy of our company brochure which includes resumes of the key personnel who will be working on this project.
