Initially this questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of storm water standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your project will either be subject to 'Standard Stormwater Requirements' or be subject additional criteria called 'Priority Development Project Requirements'. Many aspects of project site design are dependent upon the storm water standards applied to a project. Applicant responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City staff has responsibility for making the final assessment after submission of the development application. If staff determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than initially assessed by the applicant, this will result in the return of the development application as incomplete. In this case, please make the changes to the questionnaire and resubmit to the City. if applicants are unsure about the meaning of a question or need help in determining how to respond to one or more of the questions, they are advised to seek assistance from Land Development Engineering staff. A separate completed and signed questionnaire must be submitted for each new development application submission. Only one completed and signed questionnaire is required when multiple development applications for the same project are submitted concun-ently. in addition to this questionnaire, applicants for construction permits must also complete, sign and submit a Project Threat Assessment Form to address construction aspects of the project. Please answer the questions below, follow the instructions and sign the form at the end. 1. Is your project a significant redevelopment? DEFINITION: Significant redevelopment is defined as the creation, addition or replacen^ient of at least 5,000 square feet of impervious surface on an already existing developed site. Significant redevelopment includes, but is not limited to: the expansion of a building footprint; addition to or replacement of a structure; structural development including an increase in gross floor area and/or exterior construction remodeling; replacement of an impervious surface that is not part of a routine maintenance activity; and land disturbing activities related with structural or impervious surfaces. Replacement of impervious surfaces includes any activity that is not part of a routine maintenance activity where impervious material(s) are removed, exposing underlying soil during construction. If your project IS considered significant redevelopment, then please skip Section 1 and proceed with Section 2. If your project IS NOT considered significant redevelopment, then please proceed to Section 1. SOIL AND FOUNDATION ENGINEERING • GROl/NDWATER • ENGINEERING GEOLOGY 30 October 2006 THE ALLEN GROUP Job No. 04-8756 6005 Hidden Valley Road, Suite 150 Carlsbad, CA 92009 Attn: Mr. Harve Fllul< Subject: Earthwork Observation and Testing Services Kelly Ranch Corporate Center - Parcel 4, Building 3 Carlsbad, Caljfornia Gentlemen: In accordance with your request, Geoteehnical Exploration, Inc. has provided earthwork observation and testing services In connection with the subject project. Robert Prater Associates (RPA) previously performed a geotechnical investigation for the entire Kelly Corporate Center site, the results of which were presented in their report titled "Geotechnical Investigation for Kelly Ranch Corporate Center, Palomar Airport Road and College Boulevard, Carlsbad, California" dated April 30, 1997. In addition, RPA provided earthworlc observation, testing, and as-built geology services in connection with the previous mass grading and construction of existing improvements, including Building Pads 1, 2A, 2B and 3, the results of which were presented in their report dated April 12, 2000. In addition, RPA also performed an update geotechnical reconnaissance for the subject building project the results of which were presented in their report dated July 19, 2004. We previously issued an interim grading report dated July 15, 2005, as well as letters dated January 30, March 27, and April 7, 2006, regarding base under on-site curbs and gutters and presenting R-Value test results and pavement section recommendations for the on-site pavements and the Palomar Airport Road improvements. 7420 TMJE Smn» SAN DEGO, CA. 92121 • (858) 549-7222* FAX: (858) 549-1604 • EMAIL: geotecti@gel-sd.com mm Kelly Ranch Corporate Center - Parcel 4, Building 3 Job No. 04-8756 Carlsbad, California Page 2 During the period of October 12, 2004, to August 21, 2006, our representatives were present to observe the earthwork operations and to provide field density testing services. The approxtmate locations of the field density tests are shown on the attached Figure Nos. I through III. The results of the field density and laboratory compaction tests are presented in the attached Tables A and B, respectively. We also provided footing observation services for the subject building and the central plant. Based on our observations and the attached test results, It is our opinion that the soil engineering aspects of the mass grading are in compliance with the approved geotechnical report(s) and the grading and Improvement plans. Details regarding our earthwork observation and testing services are as follows. MASS GRADING The mass grading operations Included additional removal and recompaction of alluvial soils along portions of the northern and eastern building perimeters, as well as the central portion of the southern perimeter and the area of the mechanical building (see attached Figure No. III). The recompaction work consisted of removing alluvial soils to within a foot of the ground water levels and refilling the excavations with compacted structural fill. Portions of the excavation bottoms required stabilization with reinforcing fabric and/or a layer of compacted aggregate base. After the recompaction work these areas were surcharged in conformance with the plans (see attached Figure No. IV). Prior to the placement of the surcharge fill, one settlement monument was installed on each of the new recompaction areas and monitored for a period of about 7 weeks. The approximate locations of the monuments are indicated on Figure No. IV. Based on our review of the data, settlements of up to about 0.04 feet occurred from the surcharge load and were Kelly Ranch Corporate Center - Parcel 4, Building 3 Job No. 04-8756 Carlsbad, California Page 3 essentially complete in about 10 days. The results of the settlement data are presented on the attached Figure No. V. Subsequent to the completion of the surcharge period and removal of the surcharge fills, the building area was finish graded and capped with an 8-inch thick layer of low expansion potential imported fill soil; an additional 4-inch thickness of low expansion potential fill was placed as part ofthe building slab construction. Based on the preceding, it is our opinion that the building pad was suitably prepared for construction of the proposed building in accordance with the recommendations ofthe update geotechnical reconnaissance report dated July 19, 2004. TRENCH BACKFILL Based on our observations and attached test results, it is our opinion that the sewer, water, storm drain, TV, telephone, site lighting, and gas and electric trench backfills were compacted to a minimum of 90 percent relative compaction in accordance with the project specifications. PAVEMENT Based on our observations and the attached test results, it is our opinion that the following items of work were satisfactorily completed in accordance with the project specifications. 1. Preparation and compaction ofthe pavement subgrade soils to a minimum of 95 percent relative compaction. 2. Placement and compaction of aggregate base for the pavements to a minimum of 95 percent relative compaction. Kelly Ranch Corporate Center - Parcel 4, Building 3 Job No. 04-8756 Carlsbad, California Page 4 3. Placement and compaction of asphalt concrete to a minimum of 95 percent relative compaction. FOOTINGS Our representatives were also onsite to inspect the materials exposed in the footing excavations for the subject building and central plant. The materials consisted essentially of well compacted silty and clayey sand fill soils as anticipated in the April 30, 1997, RPA investigation report. Accordingly, it Is our opinion that the footing excavations were carried to the proper depth and that the materials exposed are suitable for the support of footings designed in accordance with the recommendations presented in the April 30, 1997, report for the project.
