Initially this questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of storm water standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your project will either be subject to 'Standard Stormwater Requirements' or be subject additional criteria called 'Priority Development Project Requirements'. Many aspects of project site design are dependent upon the storm water standards applied to a project. Applicant responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City staff has responsibility for making the final assessment after submission of the development application. If staff determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than initially assessed by the applicant, this will result in the return of the development application as incomplete. In this case, please make the changes to the questionnaire and resubmit to the City. If applicants are unsure about the meaning of a question or need help in determining how to respond to one or more of the questions, they are advised to seek assistance from Land Development Engineering staff. A separate completed and signed questionnaire must be submitted for each new development application submission. DEFINITION: Significant redevelopment is defined as the creation, addition or replacement of at least 5,000 square feet of impervious surface on an already existing developed site. Significant redevelopment includes, but is not limited to: the expansion of a building footprint; addition to or replacement of a structure; structural development including an increase in gross floor area and/or exterior construction remodeling; replacement of an impervious surface that is not part of a routine maintenance activity; and land disturbing activities related with structural or impervious surfaces. Replacement of impervious surfaces includes any activity that is not part of a routine maintenance activity where impervious material(s) are removed, exposing underlying soil during construction. If corrective action requires any physical change or modification to the subject equipment, the applicant shall apply for and obtain an authority to construct for all such modifications. 2 All materials containing Volatile Organic Compounds (VOC) or Toxic Air Contaminants (TACs) shall only be applied in the booth specified above, where the exhaust fans and exhaust filters are installed and operating properly. This condition shall not apply to the following operations as defined by Rule 67.20: touclvup coatings, coatings applied using non-refillable hand-held aerosol spray containers, and application of body fillers, and bedliner coatings. 3 This operation shall comply with all applicable requirements of Rule 67.20. 4 Total Volatile Organic Compound (VOC) emissions from all materials used in the above operation shall be less than an average of ten pounds per day. The average daily VOC emissions shall be calculated by dividing the total VOC emissions in a calendar month by the number of days on which coating operations are conducted in that calendar month. (Rule 20.2) 5 The Permittee shall only apply materials containing trivalent chromium, nickel, copper, beryllium, manganese, or arsenic when using non-atomizing application technology including brushes, rollers, flow coating, dip coating, touch up markers or marking pens. For HVLP equipment, the applicant will have available on site pressure gauge(s) in proper operating condition to measure the air cap pressure or have available manufacturer's technical information showing the correlation between the handle air inlet pressure and the air cap pressure. 9 If the correlation option specified above is chosen to demonstrate compliance, a handle air inlet pressure gauge will be required on site in proper operating condition to measure the handle air inlet pressure. The applicant shall maintain a permanent air pressure at the air cap of 0.1 to 10 psig. 10 For all application equipment subject to Rule 67.20(d)(5)(vii), the applicant shall operate the equipment in accordance with written District approval for coating application equipment. 11 All District approval documentation for coating application equipment, issued in accordance with Rule 67.20(d)(5)(vii), shall be maintained on-site and made readily available to District personnel upon request. 12 Monthly usage of specialty coatings, except antiglare/safety coatings, applied to motor vehicle and mobile equipment shall not exceed three gallons or 5% by volume (whichever is greater) of all coatings applied in the motor vehicle and mobile equipment coating operation. (Rule 67.20) 13 Precoat usage shall not exceed 25% of the total monthly volume of low VOC primers and primer surfacers applied in the motor vehicle and mobile equipment coating operation. (Rule 67.20) 14 Non-plastic part surface preparation materials shall meet one of the following requirements: a) the VOC content does not exceed 200 grams per liter (1.67 pounds per gallon), as applied; or b) the initial boiling point is at least 190 C (374 F); or c) the total VOC vapor pressure is 20 mm Hg or less at 20 C (68 F). 15 Materials used for surface preparation of replacement plastic parts meet one of the following requirements: a) the VOC content shall not exceed 780 grams per liter (6.5 pounds per gallon), as applied; or b) the total VOC vapor pressure shall be 45 mm Hg or less at 20 C (68 F). (Rule 67.20) 16 VOC-containing materials used to clean application equipment shall meet one of the following requirements: a) the VOC content does not exceed 200 grams per liter (1.67 pounds per gallon); or b) the initial boiling point is at least 190 C (374 F); or c) the total VOC vapor pressure is 20 mm Hg or less at 20 C (68 F); or d) the cleaning material is flushed or rinsed through the application equipment in a .D. #APCD2009-SITE-06613 Page 2 of 4 Lloyd's Auto Specialties Inc July 8, 2010 Application #: APCD2009-APP-989202 contained manner that will minimize evaporation to the atmosphere; or e) the application equipment or equipment parts are cleaned in a container which is open only when being accessed for adding, cleaning, or removing application equipment or when cleaning material is being added, provided the cleaned equipment or equipment parts are drained to the container until dripping ceases; or f) a system is used that totally encloses the component parts being cleaned during the washing, rinsing, and draining processes; or g) a device, approved prior to operation by the Air Pollution Control Officer, which has been demonstrated to be as effective as any of the equipment described above in minimizing VOC emissions to the atmosphere. 17 Recordkeeping requirements shall comply with Rule 67.20 and shall contain, at minimum, the following information: a) maintain a current list of coatings, coating additives, thinners, surface preparation materials and equipment cleaning materials in use. Materials shall be identified as follows: 1) coatings shall specify category as per Rule 67.20(d)(1), including manufacturer name and manufacturer identification. 2) if applicable, all low VOC primers and low VOC primer surfacers as per definition in Rule 67.20(c)(25), including manufacturer name and manufacturer identification. 3) coating additives, thinners, surface preparation and equipment cleaning materials, including manufacturer name and manufacturer identification. b) Daily or monthly usage records shall contain the following: 1) coating manufacturer name, manufacturer identification, amount used, all added components and their mix ratios, motor vehicle and mobile equipment type (group I or group II) and specify if color match is required. 2) surface preparation material manufacturer name, manufacturer identification and amount used. 3) equipment cleaning material manufacturer name, manufacturer identification and amount used. 4) if monthly records are maintained, the number of operating days per calendar month. c) maintain a copy of the records provided by the manufacturer or supplier as specified in Rule 67.20(d)(10). 18 All records shall be retained on site for at least three (3) years and made readily available to the District upon request. 19 Current material safety data sheets (MSDS), manufacturer's specifications or analytical data regarding volatile organic compounds (VOC) and toxic air contaminant (TAG) content shall be maintained on site and made readily available to the District upon request. MSDS, manufacturer's specifications, analytical data or a combination thereof shall, at a minimum, contain the following information: VOC content weight or weight percentage, TAG content weight or weight percentage and material density (weight per volume) or material specific gravity (material density relative to the density of water). 20 All solvents, materials containing solvents and other materials containing organic compounds shall be stored in closed containers in accordance with Rule 67.17 requirements. 21 Access, facilities, utilities and any necessary safety equipment for source testing and inspection shall be provided upon request of the Air Pollution Control District. 22 The permittee shall, upon determination of applicability and written notification by the .D, #APCD2009-SITE-06613 Page 3 of 4 Lloyd's Auto Specialties Inc July 8, 2010 Application #: APCD2009-APP-989202 District, comply with all applicable requirements of the Air Toxics "Hot Spots" Information and Assessment Act (California Health and Safety Code Section 44300 et seq.). This Authority to Construct authorizes temporary operation of the above-specified equipment. This temporary Permit to Operate shall take effect upon written notification to the District that construction (or modification) has been completed in accordance with this Authority to Construct. This temporary Permit to Operate will remain in effect, unless withdrawn or modified by the District, and a revised temporary permit (Startup Authorization) is issued or a Permit to Operate is granted or denied.
