I certify that I have read the application and state that the above information is correct. 1 agree to comply with all City ordinances and State laws
relating to building construction. I hereby authorize representatives of the City of Carlsbad to enter upon the above mentioned property for inspection
purposes. I ALSO AGREE TO SAVE INDEMNIFY AND KEEP HARMLESS THE CITY OF CARLSBAD AGAINST ALL LIABIIJTIES, JUDGMENTS, COSTS
AND EXPENSES WHICH MAY IN ANY WAY ACCRUE AGAINST SATO CITY IN CONSEQUENCE OF THE GRANTING OF THIS PERMIT.
OSHA: An OSHA permit is required for excavations over 5'0" deep and demolition or construction of structures over 3 stories in height.
Expiration. Every permit issued by the Building Official under the provisions of this Code shall expire by limitation and become null and void if the
building or work authorized by such permit is not commenced within 365 days from the date of such permit or if the building or work authorized by
such permit is suspended or abandoned at any time after the work is commenced for a period of 180 days (Section 303(d) Uniform Building Code).

Dear Eugene,
This letter is to inform you of the status of the permit you obtained for the deck and
bathroom at 1052 Chestnut Avenue. The permit was originally issued on 08/16/93. Due to
an absence of approved inspections within the required time, the permit expired.
Our records show that on July 22, 1994, a renewal fee was paid and the permit was renewed
to an "issued" status. The permit will remain issued as long as the required and approved
inspections are obtained within 180 days of the last approved inspection. However, even
with approved inspections the renewal that was granted on 7/22/94 only extends this permit
for a total length of 3 years from the initial issuance date of 8/23/93, meaning that the
permit will completely expire on 8/23/96 if it has not passed it's final inspection.
Hopefully you will be able to complete the requirements and obtain a final inspection before
8/23/96. Please keep in mind that there is no occupancy or use of these facilities allowed
until this permit is finaled by the Building Inspector.
