The permit was originally issued on 08/16/93. Due to an absence of approved inspections within the required time, the permit expired. Our records show that on July 22, 1994, a renewal fee was paid and the permit was renewed to an "issued" status. The permit will remain issued as long as the required and approved inspections are obtained within 180 days of the last approved inspection.
