Arrived on site to provide periodic special inspection of Type 2 and 4 shear waUs located at B&l, B&4
and D&S.5. Referenced sheet S3 Shearwall Schedule and Holdown Schedule. Inspection included sheathing
type and thickness. Verified nail size, spacing, edge distance and penetration. Top and bottom shear transfers
were complete. All work inspected appeared to be complete per contract documents.
NOTICE: 1 hereby certify that, al the time of this inspection. I have observed all of the above reported work, unless Olherwise noled. To the best
of my knowledge and belief. I have found this work to comply with the opprowed plans, ,pecifications. and applicable $eclion(s) of the governing

Arrived on site to provide continuous special inspection during the epoxy all thread placement at F AP's
and HDU4's located on grid line F. 1/2" diameter all thread were placed to a minimum embedment of 5" at
FAP's per detail 9 provided by engineer, and 5/8" diameter all thread were placed to a minimum embedment
of 12 ½" per Holdown Schedule. Contractor used Simpson SET XP, lot # l 000065628, expires O l /07 /22 and
is placed in compliance with ESR 2508. Verified hoe depth, diameter and cleanness. Observed the epoxy
injection and dowel installation in accordance with the manufacturer instructions.
Inspection also included the Type 5 shear wall nailing on grid line F. Referenced sheet S3 Shearwall
Schedule. Inspected sheathing type and thickness. Verified nail size, spacing, e~ge distance and penetration.
Top and bottom shear transfers were complete. '

Uniform Mechanical Code, National Electrical Code and state laws regulating energy

Please make all corrections, as requested in the correction list. Submit FOUR new
complete sets of plans for commercial/industrial projects (THREE sets of plans for
residential projects). For expeditious processing, corrected sets can be submitted in one
of two ways:
1. Deliver all corrected sets of plans and calculations/reports directly to the City of
Carlsbad Building Department, 1635 Faraday Ave., Carlsbad, CA 92008, (760) 602-
2700. The City will route the plans to EsGil and the Carlsbad Planning, Engineering and
Fire Departments.
2. Bring TWO corrected set of plans and calculations/reports to EsGil, 9320 Chesapeake
Drive, Suite 208, San Diego, CA 92123, (858) 560-1468. Deliver all remaining sets of
plans and calculations/reports directly to the City of Carlsbad Building Department for
routing to their Planning, Engineering and Fire Departments.
NOTE: Plans that are submitted directly to EsGil only will not be reviewed by the City
Planning, Engineering and Fire Departments until review by EsGil is complete.
1. All sheets of plans must be signed by the person responsible for their
preparation. (California Business and Professions Code). Signatures will be
verified before the plans are approved.
2. Plans deviating from conventional wood frame construction shall have the
structural portions signed and sealed by the California state licensed engineer or
architect responsible for their preparation, along with structural calculations.

3. Recheck the location of the smoke alarms. These should comply with the
following requirements:
a) Such smoke alarm locations shall comply with the following:
i) They shall be not less than 3' from the door opening of a bathroom.

iii) They shall be at least 3' from supply registers of heating/cooling
systems.
iv) They shall be at least 3' from the tip of the blade of a ceiling~
mounted fan.

4. A porch that is accessible from the interior of the dwelling will require a minimum
of one receptacle outlet. CEC 210.52(E) (3). This receptacle must be GFCI
protected. Provide a receptacle outlet at the covered patio at the front of the
building.

inspection is required for both existing and new duct sealing.

exchanger} requires HERS inspection.
