I, Mr. And Mrs. Swick, state a release of liability and give permission
to M&W Landscaping (Lie. # 384461) to act as our agent in pulling the
necessary application permit(s) with the City of Carlsbad for work to be
done at our residence located at 1671 Corte Orchidia, Carlsbad, Ca 92009
We have contracted with M&W Landscaping to install two electrical outlets
in the landscaped areas extending from the house and for a non-metallic gas
line running from the gas meter to the back yard for which city inspections
will be necessary
