TEST Environmental Surveys and Testing was retained by MSK Development Group to provide asbestos abatement monitoring (site surveillance) and clearance air sampling during the removal of asbestos containing materials at 1858 Buena Vista Way in Carlsbad, California.
were conducted on February 28 to March 5, 2001 by Mr. Magnus Leopold, Certified Asbestos Consultant from TEST.
The asbestos abatement monitoring and clearance air sampling
The asbestos abatement was conducted by Abatement Service Operations, Inc., I68 Miller Avenue, El Cajon, California.
I. ASBESTOS ABATEMENT LOCATION
The asbestos abatement was conducted at the following locat ion :
1858 Buenli Vista Way, Carlsbad, California
1. Floor tile - (12"x12") tan w/ floral pattern - in Kitchen (mastic negative for asbestos). Floor tile and mastic underneath above tile was also negative for asbestos.
2. Roof - Asphalt sealing compound (black) on wood boards of roof below layers of asphalt roof tile, asphalt She& (vapor barrier), and Mood shingles (house + garage + backyard building).
3. Roof to crawlspace - transite ventilation pipes (3 pipes) (one from crawlspace to attic, one in attic, one in crawlspace).
4. Roof - miscellaneous asphalt penetration mastic (black) around pipes and chimney (house + backyard building)

11. ASBESTOS ABATEMENT PROCEDURE
A. Floor tiles - (12"X?2") - tan (floral pattern) - Kitchen
1. Set-Up and Removal Procedures
Critical barriers of plastic were put up on the windows, doors, and other penetrations to contain the area. A one stage decontamination unit was built at the front end of the Containment and a negative air unit was attached to the back end. The negative air machine (containing HEPA filtration system) exhausted the air from the containment area to the outside of the building. OSHA asbestos warning signs were placed on the containment plastic and also on the entrance to the decontamination unit. Barrier tape was put up to define the abatement area and to keep unauthorized personnel away from the abatement activities. The floor tiles were wetted using a Hudsen sprayer and then removed using long handled tile scrapers. The floor tiles were removed from containment using "bagout" procedures and placed in the waste dumpster. Following the removal of the floor tiles, the Industrial Hygienist from TEST inspected the containment area for asbestos and gave a visual clearance. AS0 then sprayed the containment with a lockdown encapsulant.
B. Roofing - Asphalt Sealing Compound Layer (on wood roof) (house + garage + backyard building)
1. Set-Up and Removal Procedures
A waste dumpster was double lined with plastic sheeting. A one stage shower was erected as a decontamination unit. OSHA asbestos warning signs and OSHA asbestos barrier tape were placed around the house. The top layers of the roof (wood shingles and asphalt sheeting (vapor barrier)) were removed from the roof. The plastic dropsheets were then placed around the house and in the attic.
The asphalt roofing tiles and asphalt sealing compound which was on top of the "hot mop" was removed cleanly from the wood roof. The wetted larger pieces were placed directly in a double lined dumpster and the wetted smaller material placed in plastic bags and then put into the dumpster.

C. Transite Ventilation Pipes
The transite ventilation pipes (3) from the roof to the crawlspace were removed removed by loosening the metal straps
holding them in place, and then sliding the pipes out through the roof. The transite pipes were wrapped in 6 mil plastic and then placed in the waste dumpster.
4. Roof - Miscellaneous Asphalt Penetration Nastic
Miscellaneous asphalt penetration mastic around the pipes going through the roof and around the chimney were removed by removing the metal aprons at each location and placed in the waste dumpster.
111. ABATEMENT PERSONNEL AND PROTECTIVE EQUIPMENT
Abatement Service Operations, Inc. personnel involved in the
ACM removal were Enrique Flores, Jose Salas, Francisco Soto, and Jose Moreno. The certifications for worker asbestos removal training, medical clearance, and respirator fit testing were reviewed and are included in this report. Abatement Service Operations, Inc. personnel wore full protective equipment during removal which included Tyvek suits (with attached hoods and boots), and half-face respirators with HEPA cartridges.
IV. CLEARANCE AIR SAMPLING
The clearance air sample was collected in the Kitchen containment area and analyzed by phase contrast microscopy (PCM).
Phase Contrast Microscopy (PCM) Sampling Procedures
The air samples were taken using a Dawson high volume air sampling pump connected to a plastic cassette with a two inch anti-static cowl and 25 millimeter 0.80 micron mixed cellulose ester filter (MCEF) at a flow rate of approximately 15 liters per minute for approximately 1 1/2 hours.
Phase Contrast Microscopy (EM) Analysis Method
The air samples were analyzed according to National Institute of Occupational Safety and Health (NIOSH) Method 7400A by a National Voluntary Laboratory Accreditation Program (NVLAP)

the State Licensed Contractor that is doing the work. The contractor
must provide use with a letter, signed and dated by the property owner,
