Narrative Report of Operation:
Requirements: Per sheet SN2, the Structural Notes for shear walls shows 5/8" diameter threaded
rod anchors epoxied with 5" minimum embedment into existing footings using Simpson SET-XP
epoxy.
I checked the two drilled holes for cleanliness and depth (exceeded 7") and then observed epoxy
placement of two (2) Simpson all-thread rods using Simpson SET -XP epoxy. These items were
placed as indicated in the drawing below and as shown in the pictures on page 2 of this report.
