The testing span of the panels were 140D. Dial indicators were located midspan, at each edge of the panel. After initial deflection readings were recorded, test loads were applied to the test specimen in 5 psf increments to failure. At each Increment, deflection measurements were recorded upon Initial application of the load and after 5 minutes. After the measurements were recorded, the load was released and Initial set readings recorded. Set readings were again recorded after 85 minute period. ('OSI") owns a custom-designed Microsoft Access database that has been continually improved, updated and amended to reflect the current California Fire Code. Over 125,000 chemicals are currently recognized and classified by their Chemical Abstract Service Number (CAS No.) in OSI's database, called "CAS2CIass." The foundation of OSI's Technical Report are the Physical Hazard Summary and Health Hazard Summary reports produced by CAS2CIass. 1.1. Report Preparation Process Before presenting a Technical Report to a Code Official, OSI collects the tenant's "proposed" chemical inventory and imports the inventory into CAS2CIass. OSI then makes a proper hazard determination for every unclassified chemical. OSI uses trained and degreed HazMat professionals to determine the hazard categories, which are based on the chemical's Material Safety Data Sheet (MSDS), the manufacturer's product literature, and textbooks, along with the concentration, physical state, chemical properties and health hazards of that chemical. OSI also refers to NIOSH, OSHA, EPA and DOT hazard classifications. OSI uses all these physical and chemical references to properly classify the chemical according to the categories listed in Appendix E of the 2016 California Fire Code (CFC). When 100% of the tenant's chemical inventory is properly classified, OSI generates these two tables: Physical Hazard Summary and Health Hazard Summary. OSI then compares the tenant's summaries to the Maximum Allowable Quantities (MAQs) per Control Area of Hazardous Materials Posing a Physical Hazard listed in 2016 CBC Table 307.1(1) and 2016 CFC Tables 5003.1.1(1) & 5003.1.1(3) and the Maximum Allowable Quantities (MAQs) per Control Area of Hazardous Materials Posing a Health Hazard in 2016 CBC Table 307.1(2) and 2016 CFC Tables 5003.1.1(2) & 5003.1.1(4). If the entire inventory does not exceed the MAQs, OSI advises the architect to design and construct one control area. If the MAQs are exceeded, OSI advises the architect to design more control areas until the inventory does not exceed MAQs. Many hours of technical expertise are spent designing the quantity and one-hour boundaries of the control areas. OSI and the architect use this CAS2CIass report at the beginning of the design process and the information is used to form the basis of the building design. This Technical Opinion Report is based on the storage, use and handling of Hazardous Materials. 1.2. Report Submission to the Carlsbad Fire Department OSI and Cryterion Medical, Inc. understand the Carlsbad Fire Department will review this hazardous material opinion and Technical Report and that the Building Official will request Carlsbad Fire Department's approval on this Hazardous Materials Opinion and Technical Report. For example, Acetone is listed in Table 2 (Chemical Inventory Form) and Table 2.3 (Detailed Inventory Report) has three hazard classes, Flammable Class I-B (F-IB) (chemical liquid has a flash point below 73 degrees Fahrenheit and boiling point of above 100 degrees Fahrenheit), Irritants (IRR) (acute chemical exposure irritates the skin), and Other Health Hazards (OHH) (chronic chemical exposure causes affect target organs of the human body: Central Nervous System (CNS), and Lung damage. Operational Controls The following operational controls are installed in Cryterion Medical, Inc.'s facility located at 1949 Palomar Oaks Way, Suite 100, Carlsbad CA 92011 4.1. Automatic Fire Extinguishing System for buildings 12016 CFC 903&2OI6NFPA 131 Cryterion Medical, Inc.'s facility is equipped with an approved, automatic wet fire extinguishing system in accordance with the 2016 CFC Section 903 and the 2016 NFPA 13. 4.2. Fire Extinguishers [2016 CFC 906] The Cryterion Medical, Inc.'s facility is equipped with portable dry chemical fire extinguishers. There will be an appropriate amount of approved fire extinguishers mounted not more than 75 feet apart within Cryterion Medical, Inc.'s facility located at 1949 Palomar Oaks Way, Suite 100, Carlsbad CA 92011. 4.3. Security [2014 NFPA 730 & 2015 NFPA 731] Cryterion Medical, Inc.'s Facility hazardous material storage areas are equipped security measures to prevent unauthorized entry or access to hazardous materials. The security measures in place at 1949 Palomar Oaks Way, Suite 100, Carlsbad CA 92011 is within compliance of the Code. 4.4. Seismic Bracing and Shelf Construction for hazardous materials [2016 CFC 5003.9.9] Shelves greater than six feet high are braced and anchored and is of substantial construction. Shelves are provided with lips or guards. Shelf storage of Hazardous Materials are maintained in an orderly manner and in compliance with the Code. 4.5. Storage of Hazardous Materials [2016 CBC Table 307.1 (1) & Table 307.1 (2) and 2016 CFC 5001, 5003, Tables 5003.1.1(1)through 5003.1.1(4), 5003, 5004] The storage, use and handling of hazardous materials in quantities not exceeding the maximum allowable quantity per control area indicated in 2016 CFC Tables 5003.1.1(1)through 5003.1.1(4), shall be in accordance with the 2016 CFC Sections 5001 and 5003. None of the hazardous materials stored, used or handled at Cryterion Medical, Inc.'s facility exceed the maximum allowable quantity per control area for "B" Occupancy in accordance with the 2016 CFC. 4.6. Approved Flammable Liquid and Hazardous Materials Storage Cabinets [2016 CFC 5003.8.7 & 5704.3.2] Where storage cabinets are used to increase maximum allowable quantity per control area or comply with the 2016 CFC Chapter 50 such cabinets shall be in accordance with the 2016 CFC Sections 5003.8.7.1 and Cryterion Medical, Inc. Flammable and combustible liquids are stored in approved flammable & combustible liquid and approved (DOT) storage containers. Doors are well fitted, self-closing, and equipped with a latch. Cabinets shall be in accordance with the 2016 CFC Sections 5003.8.7.1. Safety cans are listed in accordance with (UL 30) when used to increase maximum allowable quantity of flammable or combustible liquids. All Hazardous Materials are stored in (FM) approved and/or (UL) Listed self-closing safety cabinets or containers. 4.7. Separation of Incompatible Materials [2016 CFC 5003.9.8] Incompatible materials are separated from each other in storage in (FM) approved self-closing storage cabinets, or in separate (UL) listed or (DOT) approved waste collection containers. To ensure compliance, one or more of the following methods will separate incompatible materials: Storage in separate approved self-closing storage cabinets as follows: Flammable/Combustible liquids (Class I, II, and Ill-A) is stored in approved flammable liquids cabinets, Acids are stored in acid cabinets, Bases are stored in base cabinets. Trained personnel will further distinguish between incompatible materials (i.e. acids and bases; oxidizers and flammables; acids and flammables) and store them in an appropriate manner with adequate distance to avoid incompatible reactions. Incompatible gases are separated by a distance of more than twenty feet Incompatibles are separated using noncombustible partitions/barrier greater than 18" high. Employees and Staff whose job requires them to handle hazardous materials will receive chemical safety instruction on the separation of incompatible materials 4.8. Empty Containers and Tanks [2016 CFC 5003.2.5, 2012 NFPA 30] The storage of empty containers and tanks previously used for the storage of flammable or combustible liquids, unless free from explosive vapors, is stored as required for filled tanks and containers. Tank and containers, when emptied will have the covers or plugs immediately replaced in openings. In accordance with the 2016 CFC and 2012 NFPA 30. 4.9. Sources of Ignition [2016 CFC 5003.7, 5704.2.4 & 5803.1.4] In locations where flammable vapors could be present, precautions are taken to prevent ignition by eliminating or controlling sources of ignition. Adequate grounding and bonding is provided to prevent the accumulation of static electricity wherever Class I or II liquids are stored, used and handled. Smoking and open flames are prohibited in locations where flammable vapors could be present. Sources of ignition will comply with the 2016 CFC Sections 5003.7.1 through 5003.7.3. 4.10 Building Exhaust and Ventilation [2016 CIVIC Chapter-5, 2016 CFC 5001 .3.3.10] Where ventilation is necessary to limit the risk of creating an emergency condition resulting from normal or abnormal operations, means of ventilation shall be provided. Wet Laboratories 11 and 12 each is provided with 50 cfm per snorkel with a nitrous oxide hose running into each snorkel and flowing at 0.25 cfm. The existing building exhaust and ventilation system is in accordance with the 2016 CFC and 2016 California Mechanical Code. 4.11. Hazard Identification Signs [2016 CFC 5003.5 & 5703.5, 2012 NFPA 704] Unless otherwise exempted by the fire code official, visible hazard identification signs specified in 2012 NFPA 704 for the specific material contained shall be placed on stationary containers and above ground tanks and Cryterion Medical, Inc. May 4, 2017 Hazardous Materials Technical Report Page 21 at entrances to locations where hazardous materials are stored, dispensed, used or handled in quantities requiring a permit and at specific entrances and locations designated by the fire code official. The marking and Identification Signs is in accordance with the 2016 CFC Sections 5003.5, 5703.5 and the 2012 NFPA 704. 4.12. Laboratory use of Combustible and Flammable Liquids Cryterion Medical, Inc., will dispense combustible and flammable liquids within locations identified in Table 1. Combustible and flammable liquids when used in any amount in their pure form, they will be used within the facility areas identified in Table 1. Class lB Flammable liquid (70% isopropyl alcohol) will be used as a laboratory disinfectant within the areas identified in Table I and Attachment 2. Combustible and/or flammable liquids transferred into Low-density polyethylene (LDPE) safety wash bottle(s) (Model No. 2425-0504) complying with OSHA regulations. Refer to (Figure 1) for picture of Low-density polyethylene (LDPE) safety wash bottles/squirt bottles (Model No. 2425-0504). (Figure 1) Low-density polyethylene (LDPE) safety wash bottles 4.13. Compressed Gases [2016 CFC 5301, 5501] Storage, use and handling of compressed gas containers, cylinders, tank and systems shall comply with the 2016 CFC Chapter 53. Partially full compressed gas containers, cylinders or tanks containing residual gases shall be considered as full containers for the purposes of the controls required. Compressed gases in service and in storage are stored upright and adequately secured in a Gas Cabinet with one or more support straps and/or chains to prevent cylinders from falling or being knocked over. Cylinders, including empty ones, will have their caps in place and valves tightly closed. Cylinder will always be secured. Cylinders will only be moved by properly trained Cryterion Medical employees and Licensed Commercial Contractors with an approved cylinder cart and/or dolly. The storage, use and handling of compressed gases at Cryterion Medical, Inc., shall be in accordance with the 2016 CFC. 4.14. Oxidizing Materials [2016 CFC 6303] The storage and use of oxidizing materials in amounts not exceeding the maximum Allowable quantity per control area indicated in the 2016 CFC Section 5003.1 shall be in accordance with the 2016 CFC Sections 5001, 5003, 6301 and 6303 and Chapter 53... Cryterion Medical, Inc., will store and use oxidizing materials In quantities below maximum allowable quantities indicated in the 2016 CFC Table 5003.1.1(1). Oxidizing materials will be stored, used and handled in accordance with the 2016 CFC Sections 5001, 5003, 6301 and 6303. 4.15. Oxidizing Gases [2016 CFC Chapters 50, 53, 63] The storage, handling and use of oxidizing gases shall be in accordance with the 2016 CFC Chapter 50, Chapter 53, and Chapter 63. Cryterion Medical, Inc., existing Nitrous Oxide Gas system is installed within Cryterion Medical, Inc. (Plan Check No: CBC2017-0056. Rev.) May 4, 2017 Hazardous Materials Technical Report Page 22 Wet Lab (11) and Wet Lab (12). The existing Nitrous Oxide System(s) were designed to provide a continuous flow of Nitrous Oxide in and out of the system at an approximate rate of 0.25 cubic feet per minute (cfm). Oxidizing Gas (Nitrous Oxide) will not be used within Portable Clean Room. Refer to Attachment 2 (Facility First Floor Plan) for 'Portable Clean Room" location. 4.16. Organic Peroxides [2016 CFC 6201, 2002 NFPA 432] The storage and use of organic peroxides in amounts not exceeding the maximum Allowable quantity per control area indicated in the 2016 CFC Section 5003.1 shall be in accordance with the 2016 CFC Sections 5001, 5003, 6201 and 6203. Cryterion Medical, Inc., will store and use organic Peroxides in quantities below maximum allowable quantities indicated in the 2016 CFC Table 5003.1 .1(1). Organic Peroxides will be stored, used and handled in accordance with the 2016 CFC and the 2002 NFPA 432. 4.17. Spill Control & Secondary Containment for liquid & Solid Hazardous Materials [2016 CFC 5004.2] Rooms, buildings or areas used for the storage of liquid or solid hazardous materials shall be provided with spill control and secondary containment in accordance with the 2016 CFC Sections 5004.2.1 through 5004.2.3. Spill control and secondary containment will be in accordance the 2016 CFC. 4.18. Explosion Control [2016 CFC 911, 5004.6 & 5005.2. 1.2] Explosion control shall be provided in accordance with the 2016 CFC Section 5004.6 when an explosive environment can occur because of the characteristics or nature of the hazardous materials dispensed, used, or as a result of the dispensing use process. The explosion controls within Cryterion Medical Inc.'s facility will be in compliance with the 2016 CFC Sections 911, & 5004.6. 4.19. Limit Controls [2016 CFC 5005.1.4] Limit controls shall be in accordance with the 2016 CFC Sections 5005.1.4.1 through 5005.1.4.4. Temperature Controls will be maintained in the event of a power failure. Temperature controls will be in compliance with the 2016 CFC. Pressure controls are not applicable. Cryterion Medical, Inc.'s facility will not store stationary tanks onsite. 4.20. Treatment Systems [TITLE 22 CCR CH-12 ARTICLE-1 {66262.10] A generator who treats, stores, or disposes of hazardous waste on-site shall comply with the California Code of Regulations Title 22. Social Security, Division 4.5. Environmental Health Standards for the Management of Hazardous Waste Chapter 12-Standards Applicable to Generators of Hazardous Waste Article 1- Applicability, Section 66262.10. Cryterion Medical will properly manage any hazardous waste generated at 1949 Palomar Oaks Way, Suite 100, Carlsbad CA 92011 in accordance with all Federal, State and Local regulations. 4.21. Protection from Vehicles [2016 CFC 312] Vehicle impact protection required by the 2016 CFC Section 312 shall be provided by posts that comply with the 2016 CFC Section 312.2 or by other approved physical barriers that comply with the 2016 CFC Section 312.3. Currently Cryterion Medical, Inc.'s facility stores all hazardous materials inside the building and any vehicle impact protection required will be in compliance with the 2016 CFC Section 312. 4.22. Employee Training and Response Procedures/Hazard Communication [2016 CFC 406, 2016 CFC 407, and Title 8 CCR §51941 Persons responsible for the operational of areas in which hazardous materials are stored, bulked, dispensed, handled, or used shall be trained initially and annually on the chemical hazards of the materials and the appropriate spill mitigation procedures. The supplied training is in compliance with 2016 CFC Section 406, Cryterion Medical, Inc. (Plan Check No: CBC20I7-0056. Rev.) May 4, 2017 Hazardous Materials Technical Report Page 23 2016 CFC Section 407, and Cal/OSHA Title 8 CCR §5194. Cryterion Medical will provide Cal/OSHA compliant Hazard Communication safety training course and will continue to provide refresher courses every year. The following information is taught to each hazmat employee in the facility: In the event of a FIRE: First, immediately notify local fire department by dialing 911 from a landline when possible, versus a mobile phone. Next, if possible, have an employee trained on the proper use of a fire extinguisher extinguish the fire with an ABC or D fire extinguisher. If the fire cannot be extinguished safely with one extinguisher, evacuate all personnel from the building. SMALL SPILL: Use the emergency spill kits compatible with the spilled materials. Refer to Material Safety Data Sheets (MSDS) and/or Safety Data Sheets (SDS) for protective clothing and clean up guidelines. Waste products are handled and disposed of as hazardous waste. LARGE SPILL: Secure the area and warn others away. Contact Eugene J. Jung Jr., (631) 921-6820 to develop a clean-up plan. Call the Fire Department by dialing 911 if the spill represents a health hazard or materials are released outside of the building. Arrangements will be made with a licensed disposal firm for assistance with cleanup and disposal of any hazardous wastes. 5. Hazardous Materials Information Summary The chemical classifications within this technical report are based on 2016 CBC Tables 307.1(1) and 307.1(2) and the 2016 CFC Tables 5003.1.1(1)through 5003.1.1(4). None of the hazardous materials exceed the Maximum Allowable Quantities (MAQs) for Physical or Health Hazards for storage or use. This technical report identifies hazardous materials by chemical name and Chemical Abstract Service (CAS) Number.
