Manitou's surveyors located the forms for Building #2 at 1965 Cassia Road shown as
Building #2-101b on the Grading Plan for Project No. SDP 98-19, DWG 389-9A,
approved by the City of Carlsbad Engineering and Planning Departments on September
23, 2003. The forms were located on May 10, 2004. The location of the forms is in
substantial conformance with the horizontal location shown on the above Grading Plan,
DWG 389-9A. The finish floor elevation shown on the approved plan is 317.37 feet
above Mean Sea Level and the elevation of the forms was measured at 317.41 feet above
Mean Sea Level.

These manuals reference torqulng values. All factory connections ana torqued m the
factory prior to shipping, but any Incoming, outgoing, etc connections, are obviously
connected by the contractor in the field.
Attached are some pages out of these Installation manuals, rr the entire manual is needed,
it can be provided.

ENERGIZE ANY SWITCHBOARD WITH DAMAGED BUS
OR COMPONENTS
To accommodate future serviceability, the manufacturer
recommends that die heed of the cerrisge boll should
be moumsd from the rear of the switchboard for FRONT
ACCESSIBLE switchboards with the hex nut with Bellevillewasher positioned to the front.
For AEAR ACCESSIBLE iwUchboards, the manufacturer
recommends that the head of the carriage belt should be
mounted from the front of the switchboard with the hex
nut end Belleville washer positioned to the rear,
Repeat the process until ell holes In the horizontal bus
are connecied with bolts and nuts for each shipping split
Inspect connections to ensure that mere la no foreignrnawto at the connection point end that all connections

recommended in the foundation report are applicable.
The cast-in-place drilled friction piles listed below penetrated material for which the
allowable supporting capacities recommended in the foundation report are applicable. The
piles were excavated to diameters at least as large as specified and the excavations extended
at least to the depths indicated on the Foundation Plans.
The excavations for the cast-in-place belled piers listed below were bottomed on material for
which the bearing values recommended in the foundation report are applicable. The
excavations were at least as large as specified on the Foundation Plans.
The driven piles listed below were observed to be driven to the specified lengths and/or
driving resistances to obtain the supporting capacities recommended in the foundation
report.
Based upon observations, it is our opinion that the foundation recommendations presented in the
