determined to be in violation of the City of Carlsbad Municipal Code.
The inspection revealed that those items described and listed below do not conform to City Codes and
require corrective action.

18.12.225: Expiration of electrical permit number 90-1399 for 100 Amp three
phase service.
1. Make corrections as noted in attached correction notice.
2. Reinstate permit by applying for and receiving a new permit.
A second inspection will be conducted on November 22. 1991 in order to assure compliance. Should
there be a failure to comply, the City has the authority to initiate criminal action and/or commence with
an administrative abatement action. ALL COST INCURRED BY SUCH ACTION, PLUS
ADMINISTRATIVE FEES AND A POSSIBLE REINSPECTION FEE FOR EVERY INSPECTION
AFTER THE COMPLIANCE DATE, WILL BE ASSESSED AGAINST THE PROPERTY AND
BECOME A LIEN THEREON.

Dear Mr. Garrison,
We received a copy of your Order to Comply yesterday and have
forwarded it to our contact person, Mr. Bob Kleffel of Carlsbad
Industrial Associates, today.
To clarify our position in this matter we wish to point out
that we were contracted to add an additional meter and main
disconnect within the electric room only, and this was completed
and approved by the Building Department. When we were advised of
the original correction list we met with the City Inspector and
the suite tenant (Puritan Bennett) representative at the site and
the tenant representative agreed it was their responsibility and
they would take care of it.

brought to the attention of Puritan Bennett maintenance staff
by tne eiectrica 1 inspector in my presence and 'they indicated
t h e y w o u 1 d t ak e e a r e of them. Th o-1- e a r e 311 within the su i t e
and vjcre absolutely outside of our scope of work.
Item 5 - Probably the lamp has burned out.
Item 6 - We have no idea what is involved with this requirement.
