door there Additionally, show an exit sign at the door on the electrical plans
C As shown, the stair does not exit into a corridor at the ground level Rather,
the stair opens into a "room" (the "Production" is a room), which is prohibited
E (i) The plans show an option to use a fire rated suspended ceiling for the
corridor Fire dampers will be required for each mechanical penetration through
the ceiling, then, AND the plans must detail how recessed light fixtures will be
installed in the ceiling
(n) A duct is shown going into Room 108 that also serves the corridor A
fire damper will be required there
30 Please show a 3" seismic gap on the plans, per the calculations
MECHANICAL
3 Adequate ventilation still isn't shown for Rooms 140, 146 and 147 NOTE Turbine
vents do not qualify to provide ventilation
6 If wet grinding will be done (as stated in your response), why don't the plumbing
plans show the associated plumbing'?
9 Room 142 will now have ventilation provided to it from the existing office area
Since Room 142 wasn't previously conditioned (refer to the roof plan), it is a newly
conditioned area and energy documentation is required

REVIEWED BY Kurt Culver
PLAN CHECK NO 95-455
SET II
DATE RECHECK COMPLETED
May 11, 1995
FOREWORD (PLEASE READ):
This plan review is limited to the technical requirements contained in the Uniform Building Code,
Uniform Plumbing Code, Uniform Mechanical Code, National Electrical Code and state laws
regulating energy conservation, noise attenuation and disabled access This plan review is based on
regulations enforced by the Building Department You may have other corrections based on laws
and ordinances enforced by the Planning Department, Engineering Department or other
departments
The following items need clarification, modification or change All items must be satisfied before the
plans will be in conformance with the cited codes and regulations Per Sec 303 (c), 1991 Uniform
Building Code, the approval of the plans does not permit the violation of any state, county or city law
A Please make all corrections on the original tracings and submit two new sets of prints to
ESGIL CORPORATION
B To facilitate rechecking, please identify, next to each item, the sheet of the plans upon
which each correction on this sheet has been made and return this sheet with the revised
plans
C The following items have not been resolved from the previous plan reviews The original
correction number has been given for your reference In case you did not keep a copy of the prior
correction list, we have enclosed those pages containing the outstanding corrections Please
contact me if you have any questions regarding these items
D Please indicate here if any changes have been made to the plans that are not a result of
corrections from this list If there are other changes, please briefly describe them and where they
are located on the plans Have changes been made not resulting from this list?
QYes QNo
5 Regarding the technical report The floor plan included within it shows two control areas For the
foam storage control area, revise the plans to show complete one hour separation for the area This
includes the framing over the room and any penetrations (doors, ducts, etc )
6 a) Obtain the City's approval to use the 1994 UBC
b) The 1994 UBC contains a requirement for smoke dampers, in addition to fire dampers
Please review the plans and determine if smoke dampers are required
CB 95-455 I!
May 11, 1995
c) For the non-rated corridor exception to be used, ICBO has interpreted in the past that the
office area must have a fire wall between it and the non-office portions This will require a
rated wall (with rated doors/windows) between the office portion and the manufacturing area
Alternately, fire rate the corridors and then the wall between the office/manufacturing may be
non-rated
d) Show on the plans that any relocated or new lights at the suspended ceiling will be
provided with adequate seismic restraint, per the 1994 UBC
7 Label on the plans any fire rated corridors (by cross-hatching, etc)
10 From the separate disabled access list
4 b) For the door at the bottom of the ramp at the computer room, show the 18" strike
edge requirement
c) Show on the plans the 12" push side requirement for the existing restroom doors at
the left hand side of the floor plan
5 The doors at Room 106 must have 5' in the direction of door swing The corridor there
must be made wider ALSO, corridor 134 must be wider where doors 4 and 1 (from the lunch
room) face each other
11 A Two exits are required from the lunch room As shown, those exits will now converge
(since the corridor no longer continues westward), which is prohibited
B Show on the plans that the lobby by grid H/2 is an existing fire rated corridor (inspector to
verify)
C The upper level corridor must be continuous all the way to the exterior At the west side,
the corridor/stair opens into Production Room 107, which is prohibited The area from the
stair to the exterior must be maintained as an exit corridor and cannot have any rooms or uses
along the way
D Provide fire separation from Room 108 (AND the rooms next to it) to the exit corridor
leading from the stair
E The as-builts show that rooms used to occur in the area where Corridor/Room 107 will be
located Those rooms likely didn't have fire-rated ceilings Since those areas will now be part
of the rated corridor system, provide various cross-sections in those areas, showing how the
fire-rated corridor will be constructed
F Previous editions of the Code required only wire glass at rated corridors The new Codes
require 3/4-hour windows The plans show some windows are being relocated As such, they
must comply with current Codes
G Provide fire separation between the corridor and Room 109
19 This correction from the previous list still applies
21 Provide the cross-sections at altered areas of the corridors
26 The area in question was a rated corridor previously
30 Provide calculations to justify the seismic gap shown The Code requires that the actual
deflection be multiplied by 3Rw/8 to determine the seismic separation
See next sheet for outstanding PME corrections

Technical Opinion Report
Taylor Made Golf Inc
2271 Cosmos Court
Carlsbad, California 920009
Dear Ms Balch,
Environmental and Risk Management, Inc (ERMI) has been contracted by Taylor Made
Golf Inc (Taylor Made) to conduct a technical opinion report and hazardous materials
inventory evaluation with respect to the proposed building plans submitted to the City of
Carlsbad The proposed building plans are for the interior remodeling and construction of
an inside second control area for the bulk storage of several types of foam products This
technical opinion report focuses on the characterization and quantity of material to be
stored in this room with respect to the Uniform Building Code and the Uniform Fire Code
requirements (1994 edition) The new control area/matenal storage room is a Type III
Group B Division 2 occupancy control area, located on the floor level, by the loading
dock Two of the control areas walls are the exterior concrete walls and the other two
are constructed with a one hour fire rating The room is fully sprmklered and a ventilation
rate of 350 cubic feet per minute The foam storage room is 312 square feet The foam
materials to be stored in this room will not migrate beyond the confines of the room
should a leak or spill occur, therefore a secondary containment dike has not been
incorporated in the design When exposed to air the materials will solidify immediately
The Existing Taylor Made facility is located at 2271 Cosmos Court in the City of
Carlsbad The facility is a Type III non rated fully sprmklered mixed office/manufacturing
facility The occupancy classification is Group B, Division 2 The basic allowable floor
space for a Type III Group B occupancy is 8,000 square feet The proposed facility
maintains two permanent side yards and an approved automatic fire sprinkler system,
Telephone (619) 737 3125 • Fax (619) 737 3126
which will allow for up to 36,000 square feet of floor space for product manufacturing
purposes (as per Uniform Building Code, 1994 edition)
The proposed quantities of flammable combustible, and hazardous materials will not
exceed the exempt quantities specified in the Uniform Building Code Tables 9A and 9B
ERMI has included a hazardous materials inventory summary and a Uniform Building
Code / Uniform Fire Code Table 9A and 9B summary with the report
The production activities Taylor Made currently performs at the facility includes the
attachment of the club head to the golf club shaft and the application of the hand grip
The club head is glued to the shaft using the Part A epoxy resin adhesive and Part B
hardener Small quantities of the combined resin/hardener will be in use at any one time
(10 gallons or less), and the neat material will be stored in 5 gallon cans
The proposed quantities of flammable, combustible, and hazardous materials used and
stored at this facility are below the exempt quantities allowable for this building Taylor
Made is currently developing the Hazardous Materials Business Plan which will
incorporate this new construction, which will be submitted to the San Diego County
Department of Health Services, Hazardous Materials Management Division Copies of
the Material Safety Data Sheets for the materials outlined here in this report are included
as an attachment If you have any questions regarding this report or require additional
information please contact me at (619) 737-3125

Craig Douglas

Inhalation 1-hour LC50• HO 7 mg/L in rats
Skin absorption LD50 s >2,25O mg/kg in rabbits
Oral LD5O 8,191 ing/kg in rats
The mixture is a mild to severe skin irritant and a
moderate eye irritant, but is not a skin sensitizer in
animals Toxic effects described in animals from exposure
by inhalation include upper respiratory tract irritation
A single 4-hour exposure to 60 ppm caused transient corneal
opacity and transient increases in the distance from the
cornea to the anterior surface of the lens of the eye
Toxicity described in animals from repeated exposure by
inhalation include decreased weight gain, absolute and
relative liver weight decrease, and degeneration of
olfactory epithelium (nasal tissue) Toxicity described in
animals from repeated exposure by ingestion include ueight
loss, but there were no pathological abnormalities noted
A single application of 1O uL to the eye caused corneal
opacity, The administration of 10-100 uL of a similar
mixture caused corneal opacity, transjent increases j.n
corneal thickness, and transient corneal anesthesia. A
single application of approximately 60 mg/kg to the skin
caused transient increases in the distance from the cornea
to the anterior surface of the lens of the eye
The mixture does not produce genetic damage in animals, or
in bacterial cell cultures, but it was positive in one
study with cultured mammalian cells Animal testing
indicates that this mixture does not have developmental, or
reproductive effects

OTHER I NO CONCLUSIVE EVIDENCE HAS BEEN DEVELOPED TO
INDICATE THAT MDI IS A CARCINOGENIC, TERATCNGENIC, OR THAT IT CAUSES
REPRODUCTIVE EFFECTS IN ANIMALS AND HUMANS. HOWEVER, MDI HAS BEEN REPORTED
BY NIOSH TO BE MUTAGENIC TO SALMONELLA TYPHIMURIUM BACTERIA. THERE IS NOT
FULL AGREEMENT IN THE SCIENTIFIC COMMUNITY AS TO THE SAFETY OF HUMAN
CANCER RISKS.
HUMAN EFFECTS OF DVEREXPOSURE -
INHALATION: MAY CAUSE RESPIRATORY SENSITIZATION IN SUSCEPTIBLE INDIVIDUALS.
AT ROOM TEMERATURE, VAPORS ARE MINIMAL DUE TO LOW VAPOR PRESSURE. IF HEATED
OR SPRAYED AS AN AEROSOL, EXCESSIVE CONCENTRATIONS ARE ATTAINABLE THAT CCUL
BE HAZARDOUS ON SINGLE EXPOSURE. EXCESSIVE EXPOSURE MAY CAUSE IRRITATION OF
THE EYE3, UPPER RESPIRATORY TRACT AND LUNGS. EFFECTS MAY BE DELAYED,
DECREASED VENTILATORY CAPACITY HAS BEEN ASSSOCIATED WITH EXPOSURE TO SIMIL«
ISOCYANATES; IT IS POSSIBLE THAT EXPOSURE TO MDI MAY CAUSE IMPAIRMENT OF
LUNG FUNCTION.
SKIN: MAY CAUSE ALLERGIC SKIN REACTION IN SUSCEPTIBLE INDIVIDUALS. PROLONGEr
OR REPEATED CONTACT MAY CAUSE SKIN IRRITATION. MAY STAIN THE SKIN.
