On July 22, 2004 representative of Engineering Design Group was onsite during the installation of
new anchor bolts along the rear foundation wall. Anchor bolts were installed as spacing as dictated
in the retrofit plans for 2408 La Costa Avenue, Carlsbad, California.
The installation process involved drilling of holes to depths of approximately 5-1/2 inches in the
foundation stem wall, cleaning of holes with compressed air and wire brush, infilling of holes with
Simpson high streo,;Hfol-~ type epoxy, and placement of anchor bolts until the epoxy material
extruded fr e top o the hole. The anchor bolt installation was performed in general
accord with manuf urers specifications and as required by the retrofit plans.

Special Inspection of Anchor Boll Installation Retrofit Repairs

3, 2004.
A representative of Engineering Design Group was onsite during the installation of new anchor
bolts along the rear foundation wall. Anchor bolls were installed as spacing as dictated in the
retrofit plans for 2408 La Costa Avenue, Carlsbad, California.
The installation process involved drilling of holes to depths of approximately 5-1 /2 inches in the
foundation stem wall, cleaning of holes with compressed air and wire brush, infilling of holes with
Simpson high strength set type epoxy, and placement of anchor bolts until the epoxy material
extruded from the top of the hole. The anchor bolt installation was performed in general
accordance with manufacturers specifications and as required by the retrofit plans.

official, in writing, that the proposed foundation repairs have been installed in accordance with the
design".
