requirements apply to their projects. Unless none of the requirements apply, the completed checklist must be
included in the building permit application. It may be necessary to supplement the completed checklist with
supporting materials, calculations or certifications, to demonstrate full compliance with CAP ordinance
requirements. For example, projects that propose or require a performance approach to comply with energy-
related measures will need to attach to this checklist separate calculations and documentation as specified by
the ordinances .

My apologies. Please disregard my last email. Your permit CBC2019-0418 is not completely approved yet. Fire
Prevention requires a resubmittal to address their concerns. You can still complete items 1-5, but I cannot issue the
permit at this time.

Your permit CBC2019-0418 for 2530 Campbell Pl, is approved and ready to issue after the following is completed:
1) Address Esgil Remarks: Person responsible for plans needs to sign all sheets on both sets of plans prior to permit
issuance.
2) Provide Contractor's information to include State and City of Carlsbad Business License numbers
3) Contractor provides Worker's Compensation Information.
4) Contractor Signs Application _)<5) Return a complete Hazardous Materials Questionnaire with a County of San Diego stamp from, OR an Asbestos
Survey Report
6) Pay remaining building permit fees: $509.91.
