All of the pedestrian bridges connecting Building 1 to Buildings 2 and 3, as well as to the Upper Courtyard, were not complete. Reconstruction of the central stair and its platform attachments at the building 1 roof and the bridge to unit H were incomplete. The upper waterproofing membrane detail at the roof parapet of building 1 was not installed in accordance with our design. The design was unilaterally modified by the membrane subcontractor and the membrane supplier. By agreement with the general contractor a letter from the supplier was to be forwarded to American Real Estate Group accepting responsibility for this change and confirming application of the membrane warranty. WORK IN PROGRESS Contractor was continuing with the ref raming work at the First and Second Floors of Building 1. All of the requested selected demolition work in Buildings 2 and 3 had been completed. II. OBSERVATIONS planning architecture engineering 8055 Manchester Ave Suite 600 Playa del Rey - Co 90293 Telephone (213) 823-4389 A. D. Curran and B. Moor attempted to meet with Mr. Tony Mata in the morning on their way to the project site. He was not available. We were able to meet after the site visit at Mr. Mata's office prior to returning to Los Angeles. Plan check and building permits for Buildings 2 and 3 were discussed. Mr Mata indicated that he would require the submittal of complete plans for all of the reconstruction work in Buildings 2 and 3 prior to issuing the building permit. He did not stipulate that this submittal must precede all demolition work. In addition, he did not stipulate that plan check must be completed prior to issuance of the building permit. These items he will discuss with Esgil Corporation and contact Mr. Curran with a definitive response. During the meeting we also discussed the requested increase in the occupant load for the roof deck of Building 1 to 98 persons. Previously, during Mr. Mata's discussion with Esgil Corporation this request was denied. However, Esgil has been under the impression that Building 1 is not sprinkled. Building 1 is sprinkled throughout. Mr. Mata agreed to re- discuss this issue with Esgil during his meeting with them 9 March. K. Patrick, D. Burge, K. Gallo, B. Moor and D. Curran walked through and examined all of the demolition work in Building 3 and the adjacent attached central walkway structure. All of the observable problems were discussed and possible solutions reviewed. Subsequent to the Building 3 walk-thru, K. Patrick, K. Gallo, B. Moor and D. Curran discussed the schedule for the third phase of the project. This will involve preparation of plans and specifications and the reconstruction of Buildings 2 and 3. In this regard K. Gallo mentioned that to maintain work force continuity on the project plans should be available in approximately six weeks for Buildings 2 and 3. Discussion then centered on the priority areas for reconstruction and where the contractor would most likely start his work. This might indicate the areas for which plans should be prepared first. Following the discussion it was agreed that K. Gallo would provide DGCA with a probable sequence of construction activities in Buildings 2 and 3. In addition, DGCA will meet with T. Mata (see paragraph A above) and discuss the city's requirements for plan submittal. The results of this information will be incorporated into a schedule for Ken Patrick. The product was a twenty five pound, asphalt impregnated, fiberglass reinforced, felt manufactured by GAF. D. Curran approved the use of this product for the base sheet to be applied and attached to plywood roof deck only. The second base sheet which is to be applied to the top of the Foamglas surface is to be one of the products identified in the specifications. The sub-contractor accepted these conditions and proceeded to apply a second row of staples at the lap between the felt sheets. D. Curran pointed out problems with some broken Foamglas panels. All panels which are broken must be discarded. Only undamaged panels may be used. In addition, the contractor must be certain that all panels are completely enclosed in an asphalt matrix so that there are no voids at the panel joints and the panels are fully attached at their surfaces to adjacent panels. D. Curran inspected the additional boundary nailing required at the roof deck plywood diaphragm along Grid Line "F". Two rows of eight penny nails at 6" on center was required but only one. row was installed. The contractor began immediately to install this nailing. In addition, there was a non T & G plywood panel joint approximately one foot north of Grid Line "F" where the surfaces of the adjacent plywood panels were off-set between 1/4 to 1/2 in several locations. Contractor agreed to block these offset joints from the underside of the roof deck. Contractor indicated that the City will require DGCA to initial all plans and details, where appropriate, to indicate that the work has been installed in accordance with the prescribed design. During a subsequent phone conversation, 14 March 1988, D. Curran indicated that he would prepare an additional detail book and plans for the purposes of providing this documentation to the city. III. OBSERVATIONS A. Contractor had completed installation of the Foamglas fill and Tremco waterproofing membrane fl on the roof deck of Building 1. A water test was performed earlier in the week wherein the roof drains were blocked and water was left to stand on the roof for a period of 24 hours. According to the contractor no leaks were observed in the roofing system. Some leaks were located in the drain pipes which connect the roof drains to the Lower Court drainage trench. These leaks were repaired. The Tremco waterproofing membrane appeared to conform to the drawings and specs. All of the parapet flashing and counter flashing was in place and appeared acceptable. It was noted that in some cases the bottom edge of the parapet sheet metal flashing terminated flush with the roof deck waterproofing membrane rather than 1/2" clear as specified. This did not appear to create any particular problem. Contractor had removed the lightweight concrete fill on the Second Floor bridge to Unit H and opened up the west edge of the bridge framing. There was considerable dryrot in the bridge framing and adjacent stair walls. In addition, the connection of the bridge to Building 1 was identical to the connection between the roof deck and the central platform above. D. Curran informed the contractor that this bridge must be removed because of the extensive dryrot and the - unacceptable connection to Building 1. Curran and B. Moor measured the vertical distance between the top of the bridge plywood and the top of the first concrete stair tread leading to the roof deck. At the south edge the distance was 12-1/4" and at the north edge the distance was 15-3/4 g. The stair risers varied from 7" to 7-3/8. With this information DGCA can determine the geometry of the reconstructed ramp which must tie into this stair; D. Curran, B. Moor and K. Gallo met with Ace Welding and discussed the preparation of shop drawings for the bridges connected to Building 1. The question of inspection for the field welds was discussed. The Contractor was concerned that a welder would have to be on site continuously during the construction of the bridges rather than inspect the welding at the completion of field work. In this regard, D. Curran contacted Tony Rychell of Testing Engineers for a clarification of this requirement. Tony Rychell stated that, if the field wells are single pass fillet welds, all Site Visit Report / Project No. 86008 2 inspections can be done at the completion of the field welding but prior to the certified welder leaving the job. This would a11ow the inspector from Testing Engineers to inspect the work and have the welder make any corrections before leaving the job. This information was transmitted by phone to Keith Gallo. D. D. Curran and B. Moor began preparation of a punch list for the completed framing work in Building 1 in response to the City's requirements as outlined by K. Gallo. DGCA must initial the design documents to indicate that the work was installed as prescribed. The first area to be checked was the removal of blocking above the stemwall in the underfloor area along Grid Lines E and F to provide cross ventilation to the Building 1 underfloor area. Most of the blocking had been removed but in the checking process it was determined that the existing joists on the south side of the stemwall were too close to the stemwall and restricted the required ventilation area. To solve this problem the first joist south of the stemwall must be headed off wherever blocking removal is specified. Contractor had placed half of the lightweight concrete roof deck of Building 1 on Tuesday, 29 March. The placement was in a checkerboard pattern starting from the southwest corner of the roof deck per the plans. All of the placed slabs were nearly crack free except those where there are penetrations for the HVAC vents and the fireplace flu enclosures. However K. Gallo water-tested these areas and demonstrated positive waterf low to the adjacent drains. In addition, using a hand level, he demonstrated adequate back slope on the lightweight concrete slab surface between the parapet and the drains. Contractor had placed plywood over the top of the planter area. The 20' x 20' slab section adjacent to the central stair platform is 2 1/2 thick. This is 1" thicker than the specified slab thickness and results in a localized flat area about 6' north of the central stair platform. D. Curran felt that this was not a serious problem and the slab could be left in place. K. Gallo indicated that this condition could be re-evaluated after the entire roof deck is poured. Localized corrective work could be performed then. The ventilation grills on the north side of Building 1 which provide cross ventilation to the Building 1 under floor area have an approximate 2" gap between the top of the wood mud sill and the bottom of the vent frame. This gap should be closed by blocking or sealed when. the wall is re-finished with stucco. E. Contractor must make certain that the north- south shear walls in the underfloor area of Building 1 have either plywood sheathing on both sides of the wall between the top of the stemwall and the plate or solid blocking is placed in those areas where the wall is very short. DGCA continued punch list preparation for the Building 1 ref raming. Remaining lightweight concrete for the Building 1 roof deck to be poured Monday 4 April. III.. Keith assumes that the sub- contractor will want to do Buildings 2 and 3 without moving off the site. D. Curran and B. Moor explained that this would not be possible since reconstruction work on Buildings 2 and 3 may require access from the top side of the roof. K. Gallo asked about the letter regarding the 6 x 14 beams over the kitchen areas. D. Curran explained that this was covered in a site visit report. Keith asked for the specification for the lights to be used on the roof deck of Building 1. The specification for this fixture was transmitted to Ken Patrick in November 1987. A copy of this spec will be transmitted to Keith. B. D. Curran and B. Moor continued their investigation in Building No. 3. Contractor had not completed a portion of the demolition work requested last week and therefore had two workmen removing stucco and selected areas on the third floor between the central walkway and Building 3. Diagramed all the east-west shear walls at the second floor of Building 3. The most northerly wall appears to be over-stressed and should be checked. The diaphragm on the north side of the wall in the kitchen does not extend to the plate line. The diaphragm on the wall adjacent to the corridor has been changed from stucco to gyp board. In several locations at the second and third floors where joists bear on continuous bearing walls there is no blocking between the joists. Inspected all of the connections between the roof diaphragm and the shear walls. In Site Visit Report / Project No.86008 2 (VP-91 1VV'. the case of all interiors shear walls the plywood diaphragm is nailed to a 2 x ripped member which at best is toe-nailed to the 2 x 8 roof joist below. In many cases where the wall is perpendicular to the 2 x roof framing there is no nailing for shear transfer into the wall. We examined the condition at the exterior shear walls and found the connections to be very complex and indeterminate without further investigation. 5. Examined the connection at the second floor between the central walkway and Building 3 framing. All of the 4 x 14 walkway beams are hung from a 4 x 14 transfer member which in term is "thru bolted" into a 6 x beam which is supported at its ends by built-up stud columns with light post cap connections. At the base of the built-up stud columns there is no hardware connection to the supporting framing. As we were leaving the site, heavy rainfall occurred and we took the occasion to examine drainage from the outfall of the east retaining wall and east side yard drainage system. Not much water was leaving the outfall. Ill. NEXT STEPS B. Moor to continue the Building 3 site investigation tomorrow, Wednesday 2 March 1988. Examined the flange post connections at the shear walls along grid line A at the first floor. In some cases the flange posts did not exist below the first floor and in other cases (specifically the most westerly shear wall above the first floor) the tie anchors strapping across the first floor line do not exist. The contractor had determined that there was no wood post in the wall at the ends of the first floor 4 x beam which spans across the mechanical equipment recess along grid line A. I informed the contractor that a new 4 x post would not be necessary. The existing corner stud framing would be adequate as long the existing 4 x beam is tie strapped to the double plate along grid line A. B. Moor and D. Curran continued their investigation in Building No. 3. The north-easterly units at the first and second floors had been modified by expanding into what was originally an exterior corridor serving an exterior exit stairway. To accomplish this modification, the contractor installed a new stud wall to support the roof and second floor framing. There is no blocking along this wall between the joists. An additional result of this conversion is the substitution of a gyp board shear wall for a specified stucco shear wall. Areas for additional selective demolition were indicated along the exterior hallway framing at the second floor and at the roof framing above the second floor units. III. NEXT STEPS A. WORK IN PROGRESS (BUILDING 1) East Side Yard: Property line concrete block retaining wall was in place. Cells were ready for grouting. The inside wall at the northeast corner of Building 1 where the new concrete stair goes, appeared to be improperly placed. B. Moor will review this condition with K. Gallo. Contractor had installed all of the new interior footings which replace the original spread footings on the underfloor sloped area. All of the 4 x 6 columns supporting the new 6 x 14 beams were in place below the First Floor and property attached to the new footings Contractor was excavating for new footings under the new 6 x 6 columns aligned with the existing 6 x 6 columns above at the Living/Dinning Areas. Contractor was excavating new footings along Grid Lines A and B near the Mechanical Equipment Room at the west end of Building 1. Contractor had removed all of the bridges and stairs from the roof of Building 1 and installed temporary platforms for subsequent work. Contractor had completed the installation of the new 6 x 14 beams over the kitchen at the First and Second Floors (see below). Contractor's crews were working on the roof deck of Building 1 installing the 2x 12 blocking in the existing parapet and ref raming along the south edge for the new bridge connections. Contractor had completed the rough framing to extend all of the flu enclosures along the north edge of Building 1. All of the new drains along the north side of the roof deck has been installed. It appeared that all of the new First Floor framing to replace dry rotted existing framing had been completed. This work however, has not been inspected by DGCA. Contractor had not started demolition of the balconies at the First Floor of Building 1. II. OBSERVATIONS Discussed the placement of a new 4 x 14 header beam to support the existing stairs from the third level of Buildings 2 and 3. Contractor wanted to create a composite header beam in order to facilitate installation. This composite beam would consist of the existing 2 x 14 rim joist plus a 1/2" plywood filler piece and a new 2 x 14 member nailed together. New 4 x 14 blocking would be added at the points where the stair stringers are attached to the roof deck framing. D. Curran said this would be acceptable provided the existing 2 x 14 is not damaged and the composite members are properly attached to one another with "thru" bolts or lag screws. The remaining framing at these stairs is similar to the new framing specified where the new roof drains occur. Discussed with contractor the roof edge detail where the stairs from Buildings 2 and 3 touch down from on the roof deck. The Foamglas and lightweight concrete would terminate at a wood nailer to be placed above the header referenced above. The exact height of this header would have to determined from the roof slope specified Site Visit Report / Project No. 86008 IF 'TYTh 4 ' on the roof plan. In addition, this nailer would have to be cut and trimed so that it would fit over and around the stair connection and the stair stringer. D. Curran sketched this condition during the discussion with the contractor. Discussed the ref raming to receive the new roof deck drains and the reconstructed bridges. Referred to the revised roof deck framing plan which was discussed and transmitted to the contractor prior to Christmas. Approved the rough framing for the extension of the flu enclosures. Also discussed the probable framing for the new fiberglass planter tubs. Explained that this framing and detailing would be independent of the new roof installation. In this regard it was noted that no modification is intended to the top of the parapet between the planter and the roof deck and the guard rail which is attached thereto. Inspected the spread footings and new wood posts below the First Floor framing. These support the new posts and 6 x 14 beams placed above the kitchens at the First and Second Floors. This underfloor framing was acceptable. In addition, contractor had placed the new 6 x 10 stub columns below the First Floor framing to support the two new 4 x 6 posts which had been placed in the party walls between each of the units to support the new beams over the kitchen areas. Contractor had wedged this stub column so that it provided direct bearing beneath the plywood and plate above. He had also provided a "hold- down" type attachment connecting the column base to the concrete stemwall. Contractor had not yet installed the .. come-along" connection between the 2 x roof/floor joists and the existing 4 x beam on the north edge of Building 1. These beams have, in most cases, rotated outward at their underside and pulled away from the joists which they support, leaving the joists with minimal bearing on the joist hangers. The "come-along" connections are intended to pull the beams back in line with the joists. D. Curran prepared a detailed sketch of this connection, dated this date, and left a copy with the contractor. This item was previously resolved verbally in November. Site Visit Report / Project No. 86008 3 (SVR-17. DOC Inspected the placement of the new 6 x 6 columns in the underfloor area below Unit A. These columns (4 total) were placed with lightweight post bases and post caps. Because of the heavy load carried by these columns heavy duty hardware should be utilized. A detailed sketch was prepared of the post base to be used in all instances. A copy was left with the contractor. In addition, D. Curran reviewed the connection at the top of the 6 x 6 columns based upon the typical ref raming detail for the new balcony ref raming connection at the wall where the new 6 x 6 posts occur. D. Curran prepared an additional sketch illustrating the connection at the top of these new 6 x 6 posts. D. Curran inspected each of the new 6 x 14 beams at the kitchen soffits at the First and Second Floors. Unit F: The new 6 x 14 beam has no visible grade mark and is badly split. The beam should be replaced. Unit G: The new 6 x 14 beam is grade marked DF1 instead of SS and is slightly split. The beam column connection is ok. Unit H: The new 6 x 14 is grade marked SS as specified. The beam is slightly split. The 4 x 6 columns are grade marked DF2 and appear to be ok. Unit I: The new 6 x 14 beam is grade marked DF1 instead of SS. It is heavily split. The east post has a vertical crack and should be replaced. The west post wood looks very bad. The framing of the bearing wall adjacent to the east post is ok. Unit J: The new 6 x 14 is beam is grade marked SS and is slightly split. Both wood posts are ok. The east bearing wall needs a king stud adjacent to the east wood post.. Unit B: The new 6 x 14 beam is grade marked DF1 with a large horizontal split at the center. This beam should be replaced. Both posts are acceptable. The east post is grade marked DF1. The west post is grade marked SS. Blocking is missing above the west Site Visit Report / Project No. 86008 4 (SVR-17. DOC) U bearing wall as is a king stud adjacent to the west post. This blocking should be installed and checked for all of these short bearing walls. In addition, the crib post that is to be placed above the 6 x beam should be checked for nailing and placement. Unit A: The new 6 x 14 beam is grade marked SS. There is a slight horizontal crack at the center. There are no grade markings on the posts. The east post is ok and the west post looks like construction grade. The east bearing wall needs solid blocking above. The crib post needs to be rotated to align with the post below the beam. Unit C: The new 6 x 14 beam is grade marked SS with a horizontal split at the center. The posts are ok. The crib post above needs to be rotated to align with the post below the beam. The east bearing wall needs solid blocking above. Unit D: The new 6 x 14 beam is grade marked SS. There is a 1- 1/2' deep continuous horizontal split at the center of the beam on each side. The worst crack is on the north side. The beam must be replaced. The west post is grade marked DF1. There is no visible grade mark on the east post. Both posts, however, look ok. The crib post is in place above the beam. Unit E: The new -6 x 14 beam is grade marked DF1 and looks ok. Both posts look ok. East bearing wall needs solid blocking above plus a king stud adjacent to the east post. The crib post is in place above the beam. I. D. Curran and Mike discussed the ref raming for the central platform which connects to the central stair and the roof deck of Building 1. The existing platform is hung from a 2 x 14 which is nailed into the 2 x 14 rim joist at the south edge of the Building 1 roof deck. This is not an acceptable condition. In addition, the connection of the stair to the platform consists of two half inch lag screws from each of the existing channel stair stringers Site Visit Report I Project No. 86008 5 (SVR-17. DOC) into the existing 2 x 14 rim joist at the west edge of the platform. This rim joist must be replaced and the connection revised. Also, the existing beam at the south end of the platform has dry rotted and should be replaced by a new 4 x beam with a new wood post at the west end. III. N E X T S T E P S A. In view of the fact that construction work is proceeding, you indicated that if a permit is not taken out by Monday, 23 No 1987, you will "red tag the job and stop all work. At that paint, the agreement which we reached, Monday 19 October 1987, during our meeting at the Building Department will be null and void. Construction cannot then proceed without complete plans checked prior to the re-start of construction activities. When the building permit is issued as previously agreed, the first step would be for us to meet at the site and work out the logistics of the mutual observations o the work. Most notable to require limited or determined variance was the stairs' accessibility to the upper levels at rear decks and street side units. The stairs that appear to have the most critical variances are at the rear of the complex, which services three levels, as well as the garage ingress and egress. We believe repair to these stairs may possibly involve the total removal and reconfiguration of the stringer and tread assembly. If need be, a separate landing will be installed to provide the proper length or run dimension equal to the rise allowance provided by U.B.C. The remainder of stairs adjacent to the front entry and elevator were designated as minor repairs and would only require adjustment of lower and upper landing dimension.
