Our survey was conducted on January 08, 2020, by Mr. Andrew Jordan, Certified Asbestos Consultant with ET&T. 1. Scope of Work The purpose of this limited survey was to collect bulk samples of suspect asbestos containing building materials which may be disturbed during building improvements. The samples were sent to an NlST/NVLAP accredited lab for analysis. The following procedures were conducted as part of the assessment: Limited visual inspection in readily accessible areas of concern. Collection of suspect asbestos containing materials. Written report and laboratory data interpretation. 2. Asbestos Bulk Sampling The purpose of bulk sampling is to determine if a material contains asbestos. Asbestos "bulk" samples were taken by "wetting" a suspect surface with a surfactant and collecting the material. These materials are sent to an accredited laboratory for analysis by polarized light microscopy (PLM) by EPA method 600/R-93-116. Asbestos containing material is a material that contains greater than 1% asbestos. 1106 Second Street, Suite 102 • Encinitas, CA 92024 • Tel: (760) 804-9400• Fax: (760) 630-9303 ET&T Job # 197621 • 2910 Jefferson St., Carlsbad, CA 92008 • Survey Date: January 08, 2020 www.ETandT.com Page 2 of 5 Asbestos Bulk Sampling Results Table 1: Asbestos Bulk Sampling Results Sample ID [ - Location - Material - - - Results 01 Suite #101— Northwest Office Wall: Drywall/Joint Compound ND - - - 02 [ Suite #101—Bathroom Wall: Drywall/Joint Compound - ND 03 -Suite #101 —Southwest Office Wall: Drywall/JointCompound - - ND 04 Suite #101—Lobby Wall: Drywall/Joint Compound ND - 05 Suite #101—Southeast Room Wall: Drywall/Joint Compound ND 06 Suite #101 - Ceiling Tile (Beige/Course) IND - 07 Suite #101— Ceiling Tile (White/Smooth) ND 08 - Suite #101 - Ceiling Tile in Bathroom - ND 09 Suite #101— Bathroom: Floor Tile (Top Layer) - ND -F 10 Suite #101 - Bathroom: Floor Tile (Bottom Layer) ND 11 Suite #101 - Lobby: Carpet Mastic - - ND 12 F_ Suite #101 - Lobby: Desk Mastic - - ND - 13 Suite #103 - Lobby Wall: Drywall/Joint Compound - - - ND - 14 Suite #103 - Center Room Wall: Drywall/Joint Compound - ND 15 Suite #103—Corner Room Wall: Drywall/Joint Compound - ND - 16 Suite #103 - Ceiling Tile (Beige/Course) - - - ND - 17 F - Suite #103 - Ceiling Tile (Beige/Smooth) - - - ND 18 Suite #103 - Bathroom: Vinyl Flooring &_Mastic ND 19 T_ - - - Suite #103 - Carpet Mastic - - ND 20 Suite #100—Lobby Wall: Drywall/Joint Compound ND 21 F Suite #100—Exam Room #6 Wall: Drywall/Joint Compound ND 22 T Suite #100—Exam Room #3 Wall: Drywall/Joint Compound ND 1106 Second Street, Suite 102 • Encinitas, CA 92024 • Tel: (760) 804-9400• Fax: (760) 630-9303 ET&T Job # 197621 • 2910 Jefferson St., Carlsbad, CA 92008 • Survey Date: January 08, 2020 www.ETandT.com Page 3 of 5 ET&T0.94. _-, 1~_ ii'it'I'i 1EWIRONMENTALI 23 Suite #100—Bathroom Wall: Drywall/Joint Compound - - - ND 24 Suite #100 - Exam Room #2 Wall: Drywall/Joint Compound ND - 25 Suite #100 - Break Room: Linoleum ND 26 Suite #100 - Lobby: Ceiling Tile - - - ND - 27 Floor #1—CommonHallway: Ceiling Tile - -- - ND 28 F Floor #1—Common Hallway: Carpet Mastic ND 29 Suite #201 - Entry Wall: Drywall/Joint Compound ND - 30 j Suite #201 -Southwest Room Wall: Drywall/Joint Compound - - ND - 31 Suite #201— South Room Wall: Drywall/Joint Compound F- ND 32 Suite #201: Ceiling Tile ND 33 Suite #203 - Front Entry Wall: Drywall/Joint Compound ND 34 Suite #203 - South Room Wall: Drywall/Joint Compound ND 35 Suite #203 - North Room Wall: Drywall/Joint Compound ND 36 Suite #203: Ceiling Tile - ND 37 Suite #203: Carpet Mastic ND 38 Exterior: Building Paper behind Wood Siding (West) - ND 39 Exterior: Building Paper behind Wood Siding (North) ND F 40 Exterior: Building Paper behind Wood Siding (South) - - ND 41 Exterior: Building Paper behind Wood Siding (East) -- - ND 42 Exterior: Siding Caulking ND 43 Exterior: Siding Caulking F_ ND 44 I Exterior: Siding Caulking - ND ND = No Asbestos Fibers Detected 1106 Second Street, Suite 102• Encinitas, CA 92024 • Tel: (760) 804-9400• Fax: (760) 630-9303 ET&T Job # 197621 • 2910 Jefferson St., Carlsbad, CA 92008 • Survey Date: January 08, 2020 www.ETandT.com Page 4 of 5 INDOOR ENVVIRONMERNTAL 4. Summary There were forty-four (44) bulk samples collected from suspect asbestos containing building materials during the survey. There were no asbestos fibers detected in the drywall/joint compound samples collected from inside the master bathroom. Asbestos containing material is a material that contains greater than 1% asbestos. Please see Table 1 and the attached laboratory report for more information. S.
