RESOLUTION NO. 98-215
A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
CARLSBAD, CALIFORNIA, AUTHORIZING THE
EXPENDITURE OF FUNDS FOR SETTLEMENT OF THE
WORKERS’ COMPENSATION CLAIM OF THOMAS SUGG
WHEREAS, TRISTAR, Third Party Administrator for the City of Carlsbad, has

8 Insurance Fund to pay the settlement,

recommended a settlement of the Workers’ Compensation claim of Thomas Sugg; and
WHEREAS, there are sufficient funds available in the Workers’ compensation Self-
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Carlsbad,
10 California, as follows:

2. That the expenditure of $1 1,914.00 from the Workers’ Compensation Self-Insurance 12
1. That the above recitations are true and correct.

PASSED, APPROVED AND ADOPTED at a Regular Meeting of the City Council of the 3.6

3. That the Council Accept the Stipulations with Request for Award on file in the Risk 14
Fund is authorized for the settlement of said claim. 13
City of Carlsbad on the 7th day of July 1998, by the following vote, to wit:
