RESOLUTION OF BOARD OF DIRECTORS OF COSTA REAL MUNICIPAL WATER DISTRICT
AS DEFINED IN ADMINISTRATIVE CODE AUTHORIZING HEALTH INSURANCE COVERAGE FOR RETIREES
WHEREAS, the District desires to provide health
insurance for retired District employees and directors, and
WHEREAS, the District policies regarding implementation
of health insurance programs are embodied in the District's Ad-
ministrative Code:
NOW, THEREFORE, the Board of Directors of the Costa
Real Municipal Water District does hereby resolve and order that
the District's Administrative Code shall be amended according to
the attachments, Sections 2.9 and 5.11.
PASSED, APPROVED, AND ADOPTED this 17th day of
December 1986.

Sec. 2.9. Health Insurance. The District will provide each Director with a choice of ACWA-Blue Cross Prudent Buyer, ACWA-California Care or Kaiser health insurance. The District will provide the ACWA policies at no cost to the Director, and will contribute an amount not to exceed the cost of the
ACWA policy toward the Kaiser program at the option of the Director.
- Sec. 2.9 (Addendum) Health -- Insurance for Retirees. The District will continue to pay, after retirement of a Director, premiums for existing health insurance provided for District directors under the following guidelines:
1. Any Director who has voluntarily retired after the age of 50 with no less than five years of service with the District, (Retired shall refer to leaving office for any reason.) (NOTE: Directors electing ACWA-Blue Cross Prudent Buyer must have 12 years of service), and whose age, combined with years of service with the Distridt equals 70 or
more;
2. Health insurance coverage for the retiree will
be coordinated with Medicare and other benefits provided by federal and state law, and will thereby be reduced when retiree qualifies for those benefits;
3. The District will pay, after retirement of a Director under the above conditions, health insurance premiums for the spouse and dependents of the Director at the time of retirement;
4. Health insurance coverage for the spouse and dependents of a retiree will be coordinated with
Medicare and other benefits provided by federal and state law, and will thereby be reduced when the spouse and dependents qualify for those benefits.
5. Upon death of the retiree, health insurance coverage for the spouse and dependents will be

continued, provided spouse keeps District inf orbed of his/her name, address and marital status. In the event such spouse remarries, his or her
eligibility for such coverage ceases immediately and shall be terminated. As used herein, "spouse" shall mean the spouse of the Director at the time of the Director's retirement.
6. If the retiree divorces, neither his new spouse
nor his former spouse will be eligible for coverage and it shall cease immediately and be terminated. The former spouse would only be eligible for coverage under the COBRA Law.
Sec. 2.10. Dental Insurance. The District will include each Director and his/her dependents in the dental
program of the District. Each Director shall pay the premium for the program. 8 I'
Sec. 2.11. Eye Care Plan. The District will provide each
Director and his/her dependents with an eye care plan. The plan will provide a reasonable amount of
coverage at no cost to the Director.

Sec. 5.10. Work week, Payroll Period and Pay Day. The work
week shall consist of forty (40) hours in any 7 day
period as approved by the General Manage;. Th; payroll period shall be bi-weekly and shall contain two (2) work weeks. Payday shall be every other Thursday in payment of wages up to and including the previous Sunday.
Sec. 5.11. Health Insurance and Life Insurance. The District
will provide each full-time employee and his/her dependents with a choice of ACWA-Blue Cross Prudent Buyer, ACWA-California Care, or Kaiser health in- surance. The District will provide either of the two ACWA policies at no cost to the employee, and will contributeanamount not toexceed the cost of the ACWA policy toward the Kaiser program at the option of the employee.
The District will provide each full-time employee a
life insurance policy or group policy, as determined by the Board of Directo,rs, which will provide a reasonable amount of protection or
coverage at no cost to the employee.
Prior to being employed, a physical quesionnaire
form shall be filled out. -This will be followed by a physical examination, the cost to be paid by the District.
An employee shall not be entitled to any benefits effective the day following the day that the status
of employment is terminated. Employment is terminated as of the last day the employee performs work or a service for the District. The day of
termination is not extended through the period of accumulated vacation or sick leave the employee is entitled to as of the date of termination.
Effective January 1, 1987, terminated employees who leave for reasons other than gross misconduct on the part of the employee, will be eligible, along with their dependents, for coverage under the new COBRA Law.

with years of service with the District equals 70 or more;
2. Any employee who has retired by having reached
the mandatory retirement age established under the State of California Public Employees' Retirement
System and who has no less than five years of service with the District;
3. Health insurance coverage for the retiree will be coordinated with Medicare and other benefits provided by federal and state law, and will thereby
be reduced when retiree qualifies for those benefits ; I,
4. Health insurance coverage for the spouse and dependents of a retiree will be coordinated with Medicare and other benefits provided by federal and state law, and will thereby be reduced when the spouse qualifies for those benefits.
5. Upon death of the retiree, health insurance coverage for the spouse and dependents will be continued, provided spouse keeps District informed
of his/her name, address and marital status. In
the event such spouse remarries, his or her eligi- bility for such coverage ceases immediately and
shall be terminated. As used herein, "spouse" shall mean the spouse of the employee at the time of the employee's retirement.
6. If the retiree divorces, neither his new spouse nor his former spouse will be eligible for coverage

Sec. 5.12. Uniform and Dress Codes. The District will furnish
each full-time field employee with a clean uniform each day. Employees are to wear the furnished
uniforms during the normal District working hours. Those uniforms will be maintained by the District.
Employees are expected to keep themselves neat and clean insofar as is practical. All expense of laundering or cleaning of uniforms shall be borne by the District.
Sec. 5.13. Safety Equipment. The District shall furnish all
necessary or required tools and equipment to insure a safe area in which to work.
Supervisory personnel are charged with the responsibility to see that no unsafe practices or
conditions exist, and that all State laws in this regard are enforced. Violations of this provision will be a subject for discipline or discharge of
the employee and/or any supervisor who has
knowledge of the condition, practice or act.
Safety footwear: The District shall provide all employees protective clothing, tools and other equipment necessary for the safe performance of job functions and duties without cost to the employee.
District shall provide two pairs of steel-toed
safety shoes annually to each field personnel. The District reserves the right to determine the
acceptability of any such footwear.

Board of Directors of said District at a regular meeting of
the Board held on the 17th day of December, 1986, and that it
was adopted by the following vote:
