Dear Linda:
Thank you for allowing EMSAR California to continue servicing your gurneys. As
requested, this letter is intended to provide you with a breakdown of our service and
repair costs. The amount of services to be conducted per year should be based on your
call volume. Refer to your gurney owner's manual to help with the amount of services
per year you should have.
Per your request, the following proposal will be based on two services annually. If your
needs demand more service this proposal can be amended at any time.
Carlsbad FD has 6 Ferno manual gurneys to service. This agreement shall be for two
years starting July 1, 2010 and ending June 30, 2012. The service will be $770.00 per
service or $1,540.00 per year. Service will be conducted two times a year. Please keep in
mind that parts are not covered in the Preventative Maintenance (PM) price. EMSAR
will go to one location to perform this service. The service will take one day to
complete. If a second day is required to complete the service due to the gurneys not
being available, then the remainder of the gurneys will be billed for time and materials.
These costs will include travel at $65/hr and labor at $85/hr.
PM Service
A PM consists of the following:
Inspection and testing
Disinfecting
Lubrication
Equipment Tune-up and Functional adjustments
Installation of parts (not the cost of the part) within one hour PM allotted time
Cycle test
Service report
Equipment Reports
Equipment reports will be provided or sent to customer no later than 72 hours after
service.
Term:
EMSAR California will provide this service on 6 ambulance gurneys for a period of two
years from 7/1/10 to 6/30/12.

Repair Guidelines
Repairs above and beyond the normal PM service are in addition to the cost of the PM.
A certified technician will perform repairs on all qualified equipment at a rate of $85/hr.
Equipment removed from your facility for repair will be estimated for your approval
prior to any repair. All estimates will need to be approved by the appropriate personnel.
Cancelled Service
If a PM is canceled within 24 hours of the scheduled service you will be billed the total
PM amount.
Terms
If your department requires a purchase order, a copy of the purchase order must be
presented to EMSAR California. If the price of the service were to increase, EMSAR
California will contact you in writing 30-days prior to the increase. To terminate the
contract either party must do it at least 30 days prior to the next service date.
