HomeMy WebLinkAboutMPG Fire Educators; 2014-08-11;MPe FIRE EDUCATORS INVOICE / CONTRACT FOR SERVICES Date: August 11,2014 Attention: Battalion Chief Jeff Chumbley, Carlsbad Fire Department Project title: Fall North County Driver/Operator lA Course (Sept. 15-19,2014) Terms: 40 Hours of in class instruction, driving / pumping, assigned course work, and homeworkThe total cost of instructor fees is $2,700.00.The Carlsbad Fire Department and the Carlsbad Firefighter Foundation are partnering to contract this course for the benefit and training of participants from several regional Fire Departments. The Carisbad Fire Department will pay $1500.00 ofthe total $2700.00 Instructor fee, and this will pay for Carlsbad Firefighters participating in the course.. Course delivery is contingent on both organizations signing their respective financial agreements to meet the total $2700 00 instructor fee. Instructor fees are payable by check (made payable to Michael Geddes) delivered to the instructor in person, or mailed to the address below within seven days afler completion of the course. In the event that either organization fails to contract their portion of the course, the result would be a cancellation of the course and a full refund of any fees paid to the instructor by participating agencies. Geddes 2613 Jacaranda Avenue Carlsbad, Ca. 92009 MPG Fire Educators Description: The Instructor will present and expand upon California State Fire Marshall Curriculum for Driver/Operator I A. All student books, materials, processing fees, and postage will be provided by the hosting agency (Carlsbad Fire Department). All teaching facilities, audio visual systems, wireless internet access and other services related to course delivery (aside from curriculum and presentation) are also the responsibility of the hosting agency.The driving / pumping portions of the course will be conducted by and evaluated by qualified agents assigned by the hosting agency.These agents will assure that the required portions of die State Fire Marshall curriculum for the course are met under the direction of the classroom instructor. Additional skills or information presented will serve only to expand upon the required curriculum. In addition, it is understood by all involved parties that participants will drive/pump apparatus supplied by (and under the insurance coverage of) their respective employers during this course. It is further agreed upon by all parties involved that this instructor is not responsible for any costs or liabilities related to the fuel, maintenance, repair, or operation of Fire Apparatus used in this course. Ustiy, participants and their respective employers accept responsibility for the safety and well being of participants during this course. The classroom instructor shall not be held liable or responsible for any unforeseen injuries or incidents that occur outside the classroom environment. All participants are encouraged to ensure, with their employers and the hosting agency, that they are insured for unforeseen injuries or incidents (as the operation and driving of Fire Apparatus has inherent risks and is dangerous). Signed and complete documentation of pre-requisite exercise completion will be a requirement of course completion. The course will be provided in a five day format between September 15, 2014 and September 19, 20l4.This signed invoice serves as a contract between the signing parties. In the event of unforeseen cancellation due to emergencies or illness, both parties will work to find another date to complete the course as described.
