ADDRESS J PERMIT; DEPUTY PERMISSION IS GRANTED TO DO THE ABOVE WORK IN ACCORDANCE WITH ORDINANCE NO. I09 (NEW SERIES); ORDINANCE NO. i<-59 (NEW SERIES); AND ORDINANCE NO. U62 (NEW SERIES), AND ACCORDING TO THE FOLLOWING CONDITIONS: APPROVED BY ROAD SUPERINTENDENT COUNTY ROADC(MH SSI ONER DATED I9. . . B^^- DATED:, THIS PERMIT SHALL BE KEPT AT THE SITE OF THE WORK AND MUST BE SHOWN TO ANY AUTHOR- IZED REPRESENTATIVE OF SAN DlEGO COUNTY. ADEQUATE PROVISIONS SHALL BE MAINTAINED TO PROTECT THE TRAVELING PUBLIC. BARRICADES, RED LIGHTS AND WARNING SIGNS SHALL BE MAINTAINED, TOGETHER WITH FLAGMEN WHERE NECES- SARY. ALL APPROACHES TO PRIVATE DRIVEWAYS AND INTERSECTING STREETS SMALL BE KEPT OPEN TO TRAFFIC AT ALL TIMES. ALL EXCAVATED MATERIAL SHALL BE CAST AWAY FROM THE IMPROVED PORTION OF THE HIGHWAY, AND ALL EXCESS MATERIAL INCLUDING EXCESS EXCAVATION SHALL BE REMOVED FROM THE RIGHT OF WAY AND THE ROADWAY LEFT IN A NEAT AND ORDERLY CONDITION, ALL ROADSIDE DRAINAGE DITCHES SHALL BE RESTORED TO A TRUE GRADE AND THE INTAKE AND OUTLET ENDS OF ALL CULVERTS SHALL BE LEFT FREE FROM ALL EXCESS MATERIAL. -CLAY AND EARTH WHICH ADHERES TO THE PAVED SURFACE OF THE ROADWAY SHALL, BE REMOVED bV HAND SCRAPING, WASHING, AND SWEEPING, OR BY ANY OTHER METHOD THAT W(LL NOT DESTROY OR LOOSEN THE SURFACE, BUT WILL LEAVE A CLEAN NON-SKID SURFACE. '* ALL TRENCH EXCAVATIONS SHALL BE KEPT AT LEAST 5 FEET FROM THE EDGE OF THE IMPROVED ROADWAY OR PAVEMENT ANO ALL PIPES OR CONDUITS SHALL HAVE A MINIMUM COVERING OF TWO (2) FEET TO THE TOP OF THE PIPE. TRENCHING FOR INSTALLATION ACROSS ANY INTERSECTING ROADWAY OPEN TO TRAFFIC SHALL BE PROGRESSIVE, NOT MORE THAN I/2 WIDTH OF TRAVELED WAY TO BE DISTURBED AT ONE TIME, THE REMAINING WIDTH SHALL BE KEPT OPEN TO TRAFFIC BY BRIDGING OR BACKFILLING. IN BACKFILLING, FLOODING THE TRENCH WILL NOT BE ALLOWED WITHIN THE UPPER TWENTY-FOUR (2l+) INCHES. ALL LOOSE BACKFILL SHALL BE DAMPENED AND TAMPED WITH A PNEUIVIATIC TAMPER TO REDUCE ALL SETTLEMENT TO A MINIMUM. -I- SHORING WILL BE REQUIRED WHERE NECESSARY. IT IS EXPRESSLY UNDERSTOOD IN THE GRANTING OF THIS PERMIT THAT IN CASE OF RECON- STRUCTION OR MAINTENANCE Or THE ROAD BY THE COUNTY, AND IT BECOMES NECESSARY TO LOWER OR REMOVE THE INSTALLATION THAT, UPON REQUEST Or THE GOUNTY ROAD COMMISSIONER, THE SAME SHALL BE LOWERED, MOVED, OR REMOVED AT THE EXPENSE OP THE PERMITTEE. ALL ROADWAY SURFACING WHICH HAS BEEN DAMAGED OR REMOVED SHALL BE REPLACED AT THE EXPENSE or THE PERMITTEE TO THE WIDTH AND THICKNESS Or THE EXISTING ROAD SURFACING AND TO THE SATISrACTION Or THE ROAD COMMISSIONER, OR HIS AUTHORIZED REPRESENTATIVE. WHEN THE EXISTING SURFACING CONSISTS or AN ASPHALTIC ROAD OIL MIXTURE, THE REPAIRS SHALL BE MADE USING AN ASPHALTIC ROAD OIL OF AN S. C. U GRADE, OR AS SPECIRIED BY TIIE ROAD COMMISSIONER. WHERE THE INSTALLATION, AS HEREIN APPLIED FOR, EXTENDS PARALLEL TO THE ROADWAV AND WITHIN THE PAVED OR OILED SURFACE AREA, OR WHERE A LARGE AREA OF THE EXISTING SUR- FACING HAS BEEN SCARRED AND DESTROYED, AND AFTER THE NECESSARY PATCHING HAS BEEN MADE, AS HEREINBEFORE SPECIFIED, THE PERMITTEE WILL BE REQUIRED TO PLACE A SEAL COAT OVER THE ENTIRE WIDTH OF THE EXISTING OILED SURFACING TO COVER THE PATCHES AND THE SCARS MADE BY THE EQUIPMENT AND TO RETURN THE SURFACE OF THE ROADWAY TO A UNIFORM TEXTURE. THE SEAL COAT TO BE APPLIED SHALL CONSIST OF EMULSIFIED ASPHALT SPREAD AT THE RATE OF I/IOTH OF A GALLON PER SQUARE YARD AND FOLLOWED IMMEDIATELY WITH A SANDING AT THE RATE OF APPROXIMATELY 6 POUNDS PER SQUARE YARD. IF THE INSTALLATION AS HEREIN REQUESTED DESTROYS OR DAMAGES A HARD SURFACED ROADWAY, THE REPAIR MATERIALS SHALL BE THE SAME KIND AS WAS ORIGINALLY REMOVED. ANY PAVEMENT ALONG THE EDGE OF THE TRENCH WHICH SETTLES OR HAS BEEN SHATTERED, SETTLED OR BROKEN, DUE TO THE INSTALLATION, SHALL BE REMOVED AND REPLACED TO THE GRADE AND THICKNESS OF THE ORIGINAL PAVED SECTION. IF THE GRANTOR SHALL SO ELECT, REPAIRS TO THE PAVING WHICH HAS BEEN DISTURBED SHALL BE MADE BY THE EMPLOYEES OF THE GRANTOR, AND THE EXPENSE THEREFOR SHALL BE BORNE BY THE PERMITTEE, WHO SHALL PURCHASE AND DELIVER ON THE ROAD ALL MATERIAL NECESSARY FOR SAID WORK AS SPECIFIED BY THE ROAD COMMISSIONER. ALL PAYMENTS FOR LABOR, EQUIPMENT, ETC., EMPLOYED BY THE GRANTOR FOR OR ON ACCOUNT OF THE WORK REQUIRED SHALL BE MADE BY SAID PERMITTEE FORTHWITH ON RECEIPT OF WRITTEN ORDERS, PAYROLLS, OR VOUCHERS APPROVED BY THE GRANTOR. THE GRANTOR WILL GIVE REASONABLE NOTICE OF ITS ELECTION TO MAKE SUCH REPAIRS. IF THE GRANTOR DOES NOT SO ELECT, THE GRANTEE SHALL MAKE SUCH REPAIRS PROMPTLY, EXCEPT WHERE THE GRANTOR ELECTS TO MAKE REPAIRS TO PAVING AS ABOVE PROVIDED IN THIS PARA- GRAPH AND EXCEPT WHERE PROVISION TO THE CONTRARY IS MADE BY SPECIAL WRITTEN AGREE- MENT . THE PERMITTEE SHALL INDEMNIFY AND SAVE THE COUNTY OF SAN DIEGO HARMLESS FROM ANY DAMAGE, COST OR EXPENSE OR CLAIM FOR DAMAGE, COST OR EXPENSE ARISING EITHER DIRECTLY, INDIRECTLY, OR CONSEQUENTIALLY FROM THE INSTALLATION ABOVE REQUESTED. THIS PERMIT IS ONLY GRANTED AS TO ANY PORTION OF SAID STREETS OVER W/HICH THE COUNTY OF SAN DIEGO HAS JURISDICTION, AND ALL WORK SHALL BE DONE TO THE COMPLETE SATISFAC- TiON OF THE COUNTY ROAD COMMISSIONER, OR HIS AUTHORIZED REPRESENTATIVE. ^1 A i/c
