Statement of the Matter
The life insurance written by Standard Insurance Company has been
received. This includes the master contract, acceptance form and the
administration manual. Staff is requesting Council authorization for
the City Manager to sign the acceptance of Group Insurance Policy and
the Policyowner Certification.
The list of assureds has been reviewed and found to be correct as to
amounts of insurance provided.

RESOLUTION NO.3554
A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF CARLSBAD, CALIFORNIA, APPROVING
GROUP LIFE INSURANCE POLICY NO. 311713-A
WITH STANDARD INSURANCE COMPANY AND THE
LIST OF ELIGIBLE EMPLOYEES AND AUTHOR-
IZING THE CITY MANAGER TO SIGN THE POLICY
ACCEPTANCE AND THE ELIGIBILITY LIST.
The City Council of the City of Carlsbad, California,
does hereby resolve as follows:
1. That Group Life Insurance Policy No. 311713-A with
Standard Insurance Company and the list of eligible employees,
a copy of which is attached hereto marked Exhibit A and incorpor-
ated herein by reference, is hereby approved.
2. That the City Manager is hereby authorized and di-
rected to sign the policy acceptance and the eligibility list for
and on behalf of the City of Carlsbad.
PASSED, APPROVED AND ADOPTED at a regular meeting of the
City Council of the City of Carlsbad, California, held on the
3rd day of December , 1974, by the following vote, to wit:
AYES: Councilmen Frazee, Chase, Lewis, McComas and Skotnicki
