Drivers shall stop where the following describec street intersects Cannon Road: Avenida Encinas . It SECTION 2: That Title 10, Chapter 10.40 of the Carlsbad Municipal Code is amended by the addition of Section 10.40.046 to read as follows: t110.40. 046 Parking restricted on portion of Avenida Ehcinas. Parking of vehicles is prohibited during all hours of the day and night on the westerly and easterly sides of Avenida Encinas between Palomar Airport Road and Cannon Road. EFFECTIVE DATE: This ordinance shall be effective thirty days after its adoption; however, enforcement of Section 10.28.046 shall not occur until the acceptance of that portion of Avenida Encinas not yet dedicated, and the City Clerk shall certif'y to the adoption of this ordinance and cause it to be pub- lished at least once in the Carlsbad Journal fifteen days after its adoption. The report includes the activities pertaining to the following: Liability Insurance, Property Insurance, Workers' Compensation Program and Safety and Loss Prevention Program. I. LIABILITY INSURANCE The City liability insurance program during the first quarter of the fiscal year included: 1) the purchase of insurance, 2) the review of liability claims activities, and 3) the monitoring of financial activities. A. The Purchase of Liability Insurance. July 1st is the anniversary date for the City's General Liability insurance policy. At the present time the City has 20 million dollars in coverage with a $100,000 self- insured retention (SIR). The cost of the premium was $63,643. B. Liab il it y C1 aims Ac t iv i t y . PROPERTY INSURANCE October 30, 1981 $100,000 -0 - $100,000 65,339 600 1,585 35,646 600 $ 35,046 The property insurance program during the first quarter included the purchase of automobile physical damage insurance and the review of present policies that will expire December 1, 1981. A. Purchase of Automobile Physical Damage Insurance Under the new autombile physical damage policy the total coverage is $1,307,486. Vehicles costing over $10,000 are fully covered with a $500 deductible. Vehicles costing under $10,000 are only insured for fire, theft and vandalism with a $250 deductible. The premium for the automobile physical damage insurance was $6,762. Memorandum Page 3 B. Other ProDertv Insurance Policies October 30, 1981 The City has three policies that provide property in- surance that will expire December 1, 1981. One is the boiler and machinery policy, another is the property (fire) and equipment policy and the third is the con- tractor's equipment policy. The City's Risk Manage- ment Consultant and the City's broker are reviewing our present policies and the recommendation for the pur- chase of this insurance will be presented to Council in November. 111. WORKERS' COMPENSATION PROGRAM The activities of the City's workers' compensation program during the first quarter of fiscal year 1981-82 included the purchase of insurance, the indexing and recording of workers' compensation accident reports and monitoring of the financial report activities. A. Purchase of Excess Workers' Compensation Insurance July 1 is the anniversary date for the City's excess workers' compensation insurance policy. The coverage has been increased from $2,000,000 to $5,000,000 with $100,000 self-insurance retention. The cost of the premium was $18,800 which was a savings of $4,940 over fiscal year 1980-81. B. Workers' Compensation Accidents During the first quarter 20 accidents, injuries or exposures were reported. The following accidents were received and indexed in our office. SAFETY AND LOSS PREVENTION PROGRAM The activities of the safety and loss prevention program during this quarter include a training programs, the recording of automobile accidents with followup recommendations, driver's license screening, inspection of City facilities and the use of safety engineering services. During the first quarter two all-day safety and loss prevention training program were held by the City's Safety Engineer. There were 26 supervisors representing 9 City departments in attendance at the workshop. These workshops were designed to cover the following areas where accidents had been most frequent and costly. Other training sessions including signing and barricading on roadways and driver training courses are being planned for the next quarter. The Police Department is screening all employees that drive City vehicles to ascertain if they have a valid drivers license and if there have been any driving convictions that have been recorded by the DMV. . , . '.) Memorandum Page 5 October 30, 1981 Two maintenance facilities The City Yard and the Park's Yard have been inspected by the Safety Engineer during the first quarter of this fiscal year. A report of his findings has been given to the Director of Maintenance at the Yard and to the Parks and Recreation Director. During the first quarter there were two City vehicles that were involved in accidents. The cost of repairs to these vehicles was $1,354. One fire rig was also destroyed by fire and the estimated loss of this vehicle was over $15,000. As a part of the followup to this area of the loss control program it was recommended that a fisheye mirror be installed on the street sweeper that was involved in an accident to help improve the vision of the operator. The mirror has been installed. In the case of the other accident with a City truck,the employee involved was sent to the eye doctor to see if his prescription needed to be changed. The report came back that it did not need to be changed. No disciplinary action was taken; however, the employee was counseled by his supervisor to be more careful when driving a City vehicle. The Fire Department is conducting their own review of the accident involving the brush rig and their recommendations will be given to the Fire Chief for implementation. As a result of the investigation, the captain involved was counseled as to the seriousness of the accident and what he could have done to have prevented it from happening. Two suggestions were made: (1) A more thorough size-up of the area prior to going into it; and (2) posting a lead man in front of the vehicle before cresting the hill and entering the steep terrain. Also, department personnel have reviewed their standard operating procedures for this type of fire and an engineer (if available) will be assigned to operate the apparatus in the future. The new Risk Management Program has been well received by management personnel and other City employees. As the Risk Management continues to develop it will play an even larger role in the City's ongoing effort to protect their employees, equipment and buildings from possible loss. 1 RU TF-FL ETCH E R Risk Management Coordinator
