RECOMMENDED ACTION:
Approve Resolution No. 7r3 0 recommending to the San Diego Coui Board of Supervisors that Tri-City Hospital be designated as a
level I1 trauma center.
ITEM EXPLANATION :
Earlier this year the County Board of Supervisors designated si
hospitals in the county as trauma centers. At the time of c!esi
tion, Tri-City Hospital did not meet the county criteria for le
I1 trauma centers. Recently, Tri-City IIospital was surveyed by
team of experts for the purpose of determining its capabilities
a trauma center. The hospital received a score of 90.1 points;
a passing score is 70.
Despite their high score, Tri-City IIospital is not being recomm
by the Department of Health Services to the County Board of
Supervisors to be included in the San Diego County Trauma Syste
Under the current county policies only one trauma center can be
designated in north county.
Yhe benefit of having Tri-City Hospital designated as a trauma
center would be that Carlsbad residents could be treated at a local hospital instead of a San Diego hospital in the event of
trauma incident.
FISCAL IMPACT:
Tile designation of Tri-City Hospital as a trauma center would i!
little impact on the City, but would result in savings ($8CO-:!2
to local residents who require trauma care.

has niaintained an excellent relationship with the Fire
Departments of these cities and these agencies have indicated
their support of Tri-City's designation as a Trauma Center. ... Tri-City Hospital is able to receive trauma victims transported
by helicopter because it has a fully licensed helipad.
...Tri-City Hospital, in an effort to maintain a constant state of
preparedness for trauma victims, has a designated trauma surgic suite, designated trauma beds, and designated accommodations fo
the trauma team to facilitate response times. ... Tri-City Hospital offers a fully equipped and staffed Emergency
Department. All physicians in the department are Board certifi by the American Board of Emergency Medicine. In addition, all of these physicians hat7e obtained the status of "Fellow", which indicates board certification by the American Board of Emergenc
Medicine and membership in the American College of Emergency
Medicine for three years.

Hospital Staff have demonstrated a firm commitment by providing
a highly qualified, in-hospital trauma team, This in-house Trauma Team, which includes a surgeon and anesthesioloqist,

ready to fly you to the Trauma Center at Scripps MemorialHospital. The trip to the helipad willtak
minutes. At this point, approximately 29 minutes have passed. ARRIVAL

At the helipad, the medical helicopter will be waiting. the crew has been spending an ave
minutes reassessing your condition andpreparing you for your flight. The flight to Scripp's Trau
will take another 1 1 minutes. That's a total of more than 60 minutes before the doctors at tl
Center begin their work. This is where the clock stops without Tri-City Hospital as a Trauma

Why should Tri-City Hospital be designated as a trauma center?
LocatiodVolume: Tri-City Hospital is the only hospital in the North Countycoastal area qualified to receive trauma cases. BE
the Interstate 5 corridor and Highway 73 with their high speed and trafficvolume, more trauma casesoccur here than in a
area in the County.
Cost: The average added cost to the patient to be transported to another Trauma Center is 5800.00. This does not inc

Time: Tri-City Hospital's designation as a Trauma Center will cut the transport time in half for a large number of coastal Nort
patients. The time saved could be used to save your. life.

A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF CARLSBAD, CALIFORNIA RECOMMENDING TO THE SAN DIEGO COUNTY BOARD OF
- LEVEL I1 TRAUMA CENTER SUPERVISORS THE DESIGNATION OF TRI-CITY HOSPITAL AS A
WHEREAS, the San Diego County Board of Supervisors are
authorized to designate hospitals in the county as trauma
centers; and
WHEREAS, an evaluation of Tri-City Hospital showed that
Tri-City Hospital qualifies for designation as a Level I1
Trauma Center with a score of 90.1; and
WHEREAS, Tri-City Hospital's district has a large volumt
of trauma cases; and
WHEREAS, approximately 30 north county victims are bein]
transported to other hospitals every month when excellent tr;
care is available at Tri-City Hospital, and
WHEREAS, there is a significant added cost to the traum
victim for transport to other hospitals; and
WHEREAS, the Tri-City Hospital Board of Directors have
I demonstrated a firm committment to provide highly qualified,
in-hospital trauma care 24 hours a day.
NOW, THEREFORE, BE IT RESOLVED by the City Council of t
City of Carlsbad, California that the City hereby urges the
San Diego County Board of Supervisors to designate Tri-City
Hospital as a Level I1 Trauma Center.

MAJOR TRAUMA
To benefit patients who have what is called multi-system trauma
disease. That is,injuries to organs within the chest or organs
within the abdomen, long bone fractures, head injuries, patients
who are in shock, which is in simple terms a lowering of the
blood pressure from such multi-system injuries; we're talking
about patients who sustain significant gunshot injuries, chest,
abdomen, with significant bleeding, significant organ damage,
and significant lowering of blood pressure and shock, these
patients need to be treated within, what formally was called by most people as the golden hour, but the literature now suggests
that perhaps the golden hour is now the golden two hours.

SUBJECT: REQUEST FOR SUPPORT OF TRI-CITY HOSPITAL APPLICATION TO ADD PSYCHIATRIC BEDS
The attached letter from Tri-City Hospital requests your
support of their application to establish 29 psychiatric
beds at the facility.
Larry Moss of Tri-City Hospital has provided this additional
information :
-- San Diego Regional Master Health Plan authorizes
up to 35 additional psychiatric beds in the whole
county region.
-- Tri-City is one of six hospitals applying to the Regional Health Services Agency (HSA) for allocation of the authorized beds. The other five include San Luis Rey (Encinitas), Alvarado, Centre City, Paradise Valley, and one other San Diego metropolitan
area hospital.
-- The Carlsbad-Oceanside-Vista area is presently the
only subregion of the County without any psychiatric
bed facilities; the nearest being at Palomar in
Escondido and at San Luis Rey in Encinitas.
-- Tri-City Hospital now receives an average of 16
patients per month who are in need of these facilities.
-- San Luis Rey Hospital in Encinitas is a privately owned
facility specializing in psychiatric care. Tri-City
Hospital is a public facility (cperated by a special
district). These differences have led to some concerns over the wil1ing:ness of San Luis Rey to accept certain classes of patie:nts such as Medi-Cal patients and referrals from police agencies. Tri-City claims it
makes no such distinctions in its admission policies.

-- Tri-City believes that HSA will favor an application
from either Tri-City or San Luis Rey because of the
absence of facilities in the Carlsbad-Oceanside-Vista
area. On the premise that psychiatric beds are needed
in this subregion, approval of the application from
Tri-City is the only means of providing for that need.
If the City Council desires to support the application of Tri- City Hospital, adoption of the attached resolution would
accomplish that purpose.

Dear Mayor Packard:
As you might be aware, Tri-City Hospital is currently in the
process of preparing a Certificate of Need application to the
State of California for 29 psychiatric beds. The Certificate
of Need process is a .Long and arduous one and we face a significant degree of competition from other hospitals in the County for permission to establish a Mental Health Unit. It
is the feeling of the Psychiatric and the Administrative Staff
at Tri-City that there is a very definite need for psychiatric
beds in the Tri-City area. Ours is the only sub-area of the
entire San Diego/Impe.rial County without any psychiatric beds.
In order for our application to be approved, it is imperative
that we demonstrate significant csmmunity support for our pro-
posal. The application will be strengthened if it contains
evidence of a consensus of opinion within the Tri-City area
that the needs of the people will be better served if a Menta!
Health Unit is established at; Tri-City Hospital. Toward ::his
end, I ask for your assistance.
We are in the final stacjes of preparing our application and p: to submit it to the Heaith Systems Agency in late January. Ii would be greatly appreciated if you could write a brief lettei stating your feelings regarding the need for psychiatric beds
in the North County as well as your support for Tri-City Hos- pital's application.
I am very appreciative of any help you can offer and would be
happy to answer any questions you mighk have.

WHEREAS, the City Council has been informed that at pres€
there are no such beds available in the Carlsbad-Oceanside-Vis
area, and that there is a clear, identifiable need for such
facilities;
NOW, THEREFORE, BE IT RESOLVED by the City Council of thc

1. That the above statements are true and correct.
2. That the City Council of the City of Carlsbad hereby
