ITEM EXPLANATION: The subject contract amendment is to cover design and review services required ( construction of the Poinsettia Lanellnterstate 5 Interchange project. The amendment supplements the previous agreement with Boyle Engineering for c of the project. Services covered include shop drawing review, support for contract cl order preparation, routine site meetings and visits, plan clarifications and reviews. The amendment is for time and material, riot-to-exceed $70,000. Similar servic Palomar Airport Road did not exceed $55,000. FISCAL IMPACT: Contract amendment is for time and materials not-to-exceed $70,000. $9,078,5( available for the project. THIS AMENDMENT NO. 3, made and entered into as of the 9th day oi , 1994, by and between the CITY OF CARLSBAD, a municipal corporatic hereinafter referred to as "CITY", and BOYLE ENGINEERING CORPORATION, hereinafter refer1 to as "CONSULTANT." SF 2 e 0 POINSETTIA LANEII-5 INTERCHANGE Exhibit “A” - Scope Of Professional Engineering Service Amendme&-No. 3 To Agreement For Design Services June IO, 1994 This is a scope of professional engineering services for Amendment Nc the “Agreement for Design Services for Improvements to the Poinsetti; Lanehterstate Route 5 Interchange,” dated July 8, 199 1. Professional engineering services are needed by the City of Carlsbad (4 for construction assistance. This scope of services, prepared by Boyle Engineering Corporation (Consultant), is based upon our discussions P the City. Certain assumptions have been made in preparing this scope services. To the extent possible, they are stated herein. If the actual to1 effort required is different from the assumptions presented herein, bot1 and Consultant agree to revise the scope and fee accordingly. In general, this work consists of a) final design tasks not included in tl original Agreement for Design Services, and b) providing engineering and materials basis according to the attached rate schedule, not to excc $70,000. Specifically, Consultant agrees to provide the following sew 1. Coordinate with Caltrans after delivery of the final plans, specific and estimate (PS&E) to assist Caltrans District 11 and Caltrans Headquarters with preparation of the PS&E for advertising, as ret by the City. This includes responding to Caltrans questions and preparation of a log of borings for the retaining wall. 2. As requested by the City, assist the City in ajoint CityKaltrans r( of the bid item prices obtained by Caltrans after opening bids, if 1 indicate a cost overmn of no more than 10 percent of the Enginee Estimate. 3. As requested by the City, assist the City in consultation with Cal determine a course of action should the bids obtained by Caltran indicate a cost overrun exceeding 10 percent of the Engineer’s E 4. Prepare for and attend one preconstruction meeting organized by Caltrans. Document observations with a written memo. 5. Prepare for and attend one partnering workshop organized by C2 as requested by the City. 6. Prepare for and attend up to 12 regular construction status meet organized by Caltrans, as requested by the City. 7. As requested by the City, prepare for and make periodic site vis during construction to observe construction progress. Up to 10 I services related to construction activities.
