I have enjoyed the time on the Commission and feel that I have served you and the citizens to the best of my ability. During my term I have missed only one meeting, and have carried out my duties, including initiating the fund-raising book that has brought many thousands of dollars to the Arts Office, enabling us to maintain and expand the TGIF Jazz Concerts. My first term is due to end in March of this year, and I would respectfully ask that you re-appoint me to the Arts Commission for a final term. With best personal wishes, yours very truly, Arthur Wood. % - APPLICATION FOR APPOINTMENT BOARD(S) ANDIOR COMMISSION(S) (Please print or type) &.& I-b-77 NAME(S) OF BOARD(S) AND/OR COMMISSION(S) IN WHICH INTERESTED: k @ ARTS &flfl~Sgod le%.+ $4 J" NAME: RJW-WR Ud@D ADDRESS: 4 37 0 hEflOQKW'QOP CITY: rnllflrn ZIP CODE: YaooS HOME PHONE: 72 q ,30 3-4 PRESENT OCCUPATION: lye n~~c/ BUSINESS ADDRESS: d/& WORK PHONE: d /n BUSINESS NAME: / U.S. Citizenship (required for all appointments) - No Resident of Carlsbad (required for all appointments) <Yes No Registered voter in Carlsbad (required for all appointments) - ./Yes No I am familiar with the responsibilities assigned to the Board($) or Commission(s) on which I wish to serve; I am willing to be interviewed regarding my qualifications for appointment by a committee designated by the City Council or at the request of an individual Council Member.
