STAFF: FIRE AUTHORIZING THE PURCHASE OF NINE REPLACEMENT CARDIAC MONITORS FROM ZOLL MEDICAL CORPORATION AND APPROPRIATION OF FUNDS <—^^(^^ 32- RECOMMENDED ACTION: Adopt resolution No. 2012-237 authorizing the purchase of nine (09) replacement cardiac monitors from Zoll Medical Corporation and appropriation of funds. ITEM EXPLANATION: Staff is requesting that nine (09) new Zoll X Series® cardiac monitors be purchased from Zoll Medical Corporation. The new units will replace cardiac monitors currently in use that were purchased in 2002, and have exceeded the industry standard for replacement. The Fire Department has received a total of $220,000 ($110,000 each fiscal year) from the General Fund during the FY 2011-12 and FY 2012-13 operating budget for this purchase. Following the current replacement schedule. Fire would also ask for an additional appropriation in FY 2013-14 to standardize the entire fleet. However, by purchasing all nine (09) cardiac monitors now instead of waiting for the FY 2013-14 operating budget, the City will realize $53,000 in savings. Of all the EMS equipment utilized, the cardiac monitor is among the most important. Used on virtually every call, the cardiac monitor is used for monitoring vital signs, obtaining electrocardiograms (ECG's), and obtaining the oxygenation status ofthe patient. Additionally, when 12-lead ECG's are obtained, they are sent to the hospital to expedite patient treatment. Other monitoring features include obtaining temperature and obtaining the level of carbon monoxide expressed as the level of carboxyhemoglobin in the body. Looking fonA/ard to future EMS protocols, it is anticipated that both of these functions will be utilized more frequently. The Carlsbad Fire Department conducted an extensive, methodical review of the three (3) major cardiac monitor companies: Philips, Physiocontrol/Medtronic, and Zoll. Field trials were completed utilizing all three (3) monitors. After the initial trial, Zoll developed a new monitor that was also beta tested; in all, the Carlsbad Fire Department did clinical field trials on four (4) cardiac monitors. After this review, the Zoll X Series® was found to be the superior model and the one that best suits the fire department now and going fonA/ard to the future. The Zoll cardiac monitor has several patented features that staff feels renders it superior to other monitors. These features include: • Patented "See-thru" CPR technology that allows the paramedic to see the underlying cardiac rhythm while performing CPR. Contemporary research shows the most effective treatment for the cardiac arrest patient is defibrillation if indicated, and high-quality CPR. ACTION: APPROVED X CONTINUED TO DATE SPECIFIC • DENIED • CONTINUED TO DATE UNKNOWN • CONTINUED • RETURNED TO STAFF • WITHDRAWN • OTHER - SEE MINUTES • AMENDED • Page 2 • The ability to utilize smart phones for real-time patient data entry as a portal to transmit the EKG to the hospital while still at scene or enroute to the hospital. • Patented Rescue Net™ technology where data can be seen on a larger screen mounted in the back of the ambulance and the hospital can also see real-time patient data including vital signs and cardiac monitoring. • Many of our local hospitals, as well as the Oceanside Fire Department, the most frequent boundary drop agency Fire interacts with, use Zoll monitors. Utilizing the same monitor allows interoperability of the monitor pads making the patient care transfer transition easier. Likewise, there would be interoperability with the receiving hospitals. Zoll Medical Corporation is the sole manufacturer and source of X Series® Defibrillators for the EMS Market. Zoll will not sell an X Series® Defibrillator to the Carlsbad Fire Department through any vendor or dealer. The Purchasing Officer has approved this purchase as a sole source purchase. Section 3.28.110 of the Carlsbad Municipal Code (CMC) states that "goods, services and/or professional services that can be reasonably obtained from only a single source" are exempt from the provisions of CMC 3.28. FISCAL IMPACT: The new units will replace cardiac monitors currently in use that were purchased in 2002, and have exceeded the industry standard for replacement. The Fire Department has received a total of $220,000 ($110,000 each fiscal year) from the General Fund during the FY 2011-12 and FY 2012-13 operating budget for this purchase. As such, staff requests that City Council approve the purchase of nine (09) replacement cardiac monitors and an additional appropriation of $85,000 to the Fire Department. By purchasing all nine (09) monitors at one time, Zoll Medical Corporation is providing the City with $35,000 in competitive discounts and $18,000 for the trade-in value of our current LifePak monitors; a total savings of $53,000. This brings the total cost for nine (09) monitors to approximately $305,000. If the Fire Department waits until the FY 2013-14 operating budget to finalize this purchase, the competitive discounts and existing trade-in value would be lost. As such, the total costs to outfit the entire fleet during the FY 2013-14 operating budget would rise to approximately $358,000. ENVIRONMENTAL REVIEW: Pursuant to Public Resources Code Section 21065, this action does not constitute a "project" within the meaning of CEQA in that it has no potential to cause either a direct physical change in the environment, or a reasonably foreseeable indirect physical change in the environment, and therefore does not require environmental review. (California Code of Regulations Title 14, Section 15061(b)(3).) EXHIBITS: 1. WHEREAS, Section 3.28.110 of the Carlsbad Municipal Code provides that "goods, services and/or professional services that can be reasonably obtained from only a single source" are exempt from the provisions of Carlsbad municipal Code 3.28; and WHEREAS, Zoll Medical Corporation is the sole manufacturer and source of X Series® Defibrillators for the EMS Market. Zoll will not sell an X Series® Defibrillator to the Carlsbad Fire Department through any vendor or dealer. The City's Purchasing Officer has approved this purchase as a sole source purchase; and WHEREAS, the City will realize $53,000 in savings by purchasing all nine (09) replacement cardiac monitors needed to outfit the entire Fire fleet now; and WHEREAS, the City Council previously appropriated a total of $220,000 through the operating budget, and WHEREAS, an additional appropriation of $85,000 is requested to effectuate the purchase now and realize the savings offered by Zoll Medical Corporation. NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Carlsbad, California, as follows that: 1. The above recitations are true and correct. 2. That the Fire Department is authorized to initiate a purchase order with Zoll Medical Corporation for the purchase of nine (09) replacement cardiac monitors for a total cost not-to-exceed $305,000. 3.
