Subject Adopt a resolution authorizing the purchase of 11 new and recertified
Philips cardiac monitors/defibrillators from BoundTree Medical and
authorizing the disposal of 12 Zoll cardiac monitors for a credit toward the
purchase price of the new monitors.

Adopt a Resolution authorizing the purchase of 11 new and recertified Philips cardiac
monitors/defibrillators from BoundTree Medical and authorizing the disposal of 12 Zoll cardiac
monitors for a credit toward the purchase price of the new monitors.
Executive Summary
Approximately four years ago, Zoll cardiac monitors were first used by the fire department.Since
the introduction,there have been multiple documented issues,involving functionality,
conductivity,and reliability.Attempts have been made to resolve the issues with the
manufacturer,and to date,they have been unable to fix most of the problems.The Fire
Department conducted an extensive, methodical review, and field trials of the two (2) remaining
major cardiac monitor/defibrillator manufacturing companies:Philips and
Physiocontrol/Medtronic.The review found the Philips cardiac monitor/defibrillator
("monitors") to be a superior product meeting the current standard of care, while providing a
technology platform capable of meeting future needs.The Fire Department is now requesting
City Council authorization to purchase 11 new and recertified Philips cardiac
monitors/defibrillators from BoundTree Medical and authorizing the disposal of 12 Zoll cardiac
monitors for a credit toward the purchase price of the new monitors.
Discussion
The Carlsbad Fire Department provides Advanced Life Support services (ALS)through the
placement of Paramedic Firefighters on our ambulances and fire apparatus deployed from our six
fire stations.A key piece of lifesaving diagnostic and treatment technology used by our
paramedics is a cardiac monitor/defibrillator.Cardiac monitors are used for monitoring vital
signs, obtaining electrocardiograms (ECG's), and obtaining the oxygenation status of the patient.
Reliability,functionality and ease of use are critical to providing quality, timely and accurate
patient care.Additionally, when 12-lead ECG's are obtained, they are sent to the hospital to
expedite patient treatment.This communication expedites hospital interventions and
significantly changes patient outcome for the better.With this technology the fire department
and the County of San Diego Emergency Medical Services continue to build treatment protocols
to leverage the technology and improve patient treatment and outcomes. Emergency Medical
calls represent approximately 80% of the 12,500 calls for service in Carlsbad and these monitors
are used on nearly all EMS calls.
Over the last three to four years, the fire department has documented multiple issues involving
functionality,conductivity,and reliability with the Zoll cardiac monitors.Some of the
documented issues include:inaccurate blood pressure readings,false heart attack readings,
pulse oximetry sensor failure, inaccurate carbon monoxide readings, and on several occasions the
monitor has shut itself off during patient care.
Three major manufacturers of cardiac monitors (Zoll, Philips, and Physiocontrol/Medtronic) meet
the needs of the fire department to deliver critical, emergency service.Following the field trials,
the Fire Department identified the Philips cardiac monitor as the best product that met all the
required specifications, together with ease of use and handling, reliability, easier acquisition of
12 lead EKG,immediate feedback during patient CPR,and overall size and weight of the monitor.
Originally the fire department purchased 12 Zoll cardiac monitors; one was dedicated for use by
the Tactical Medics unit and the others were placed on fire and ambulance apparatus. It was later
determined that an AED device was better suited for the needs of the Tactical Medics; therefore,
one cardiac monitor is no longer needed.By purchasing 11 monitors, there will be three new
monitors placed on all of the ambulances,six recertified monitors on all engines and the truck,
and two will act as reserves.New and recertified monitors are identical in software capacity and
use.All monitors come with the same one year manufacturer warranty and two year extended
warranty, which is included in the quoted price.
Boundtree Medical will take the 12 Zoll cardiac monitors in as trade and supply the fire
department with 11 new and recertified Philips cardiac monitors to maintain the standardization
of equipment. This transaction will result in a net $14,980.45 credit with Boundtree Medical, one
of the departments' supplier of medical supplies.This credit will be used toward the future
purchase of additional Automatic Electrical Defibrillator's (AED),improving the service capacity
to the community.This purchase will be made through a cooperative government contract that
has already been competitively bid, CMAS Contract Number:4-12-65-0023C.
Fiscal Analysis
The 12 current Zoll cardiac monitors will be traded in and this will completely fund the purchase
of the Philips cardiac monitors.

A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF CARLSBAD,
CALIFORNIA, AUTHORIZING THE PURCHASE OF 11 NEW AND RECERTIFIED
PHILIPS CARDIAC MONITORS/DEFIBRILLATORS FROM BOUNDTREE
MEDICAL AND AUTHORIZING THE DISPOSAL OF 12 ZOLL CARDIAC
MONITORS FOR A CREDIT TOWARD THE PURCHASE PRICE OF THE NEW
MONITORS
WHEREAS,the City Council of the City of Carlsbad,California has determined that the fire
department provides Advanced Life Support (ALS)services and a key piece of lifesaving diagnostic and
treatment technology used by paramedics is the cardiac monitor.Emergency Medical calls represent
