RECOMMENDED ACTION:
Review and acknowledge revised disclosure statement form.
ITEM EXPLANATION:
The attached revised disclosure statement has been distributed to various
City departments by the City Manager's office. The Manager's office
wants to ensure that City staff, as well as any board, commission, or
committee is aware that a disclosure statement is available for review
prior to taking any discretionary action in an effort to avoid possible
conflict of interests.
In regards to issues requiring discretionary review by the Parks and
Recreation Commission, where appropriate, this statement form will b
required to be completed by parties requesting action by the

The necessity of this form with respect to issues before the Parks and
Recreation Commission will be discretionary on behalf of the parks and
Recreation Director and will be considered on a case by case basis.

CONFLICT OF INTEREST - DISCLOSURE STATEMENT
There has been some discussion of possible conflicts of interest with regard to
discretionary action taken by the City Council on several projects and applications under
review. In an effort to prevent this type of situation in the future, the City's Disclosure
Statement has been revised to include staff input and provide a more comprehensive
format than that which was previously used.
The Disclosure Statement Form (attached) should accompany any project or application
which may be subject to discretionary review by the City Council, or any appointed
Board, Commission or Committee. With this information available, those authorized to
take action will have the opportunity to abstain from voting, if they believe a conflict of
interest exists.
Please ensure that all of your staff and any Board, Commission or Committee you work
with, is aware that a disclosure statement is available for their review prior to taking any
discretionary action.
