INSTRUCTIONS:
To address post-development pollutants that may be generated from development projects, the city requires that new
development and significant redevelopment priority projects incorporate Permanent Storm Water Best Management
Practices (BMPs) into the project design per Carlsbad BMP Design Manual (BMP Manual). To view the BMP Manual,
refer to the Engineering Standards (Volume 5).
This questionnaire must be completed by the applicant in advance of submitting for a development application
(subdivision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of
storm water standards that must be applied to a proposed development or redevelopment project. Depending on the
outcome, your project will either be PRIORITY
requirements.
Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City
staff has responsibility for making the final assessment after submission of the development application. If staff
determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than
initially assessed by you, this will result in the return of the development application as incomplete. In this case, please
make the changes to the questionnaire and resubmit to the city.
If you are unsure about the meaning of a question or need help in determining how to respond to one or more of the
questions, please seek assistance from Land Development Engineering staff.
A completed and signed questionnaire must be submitted with each development project application. Only one
completed and signed questionnaire is required when multiple development applications for the same project are
submitted concurrently.

Subject: Letter Amendment-Storm Water Quality Management Plan (SWQMP),
Permit No. GR2017-0004, Site Address 3266 Lionshead Avenue, Carlsbad, CA

AECOM is pleased to provide the City of Carlsbad with this written correspondence regarding our
review of the proposed improvements relative to the existing SWQMP for Permit No. GR2017-0004.
Please note, given the insignificant nature of this change, this update or amendment is being
provided mainly for purposes of record keeping so as to document that a very minor change was
made to the parking lot but given the small relative decrease, no change to the amount of runoff for
this area was recognized. The information below summarizes the background and conclusion
associated with this change.
Per the attached Figure 1, AECOM is to remove 29,281 s.f. of existing asphaltic pavement and
replace it with a slightly lesser amount of 28,064 s.f. of proposed asphaltic pavement, resulting in a
small net decrease to the impervious area of 1217 s.f. for this localized area of the site. The location
of the new asphalt is in the exact same location (see Figure 1) as it was previously defined in
SWQMP Permit No. GR2017-0004. As a result of this very small relative change (Existing Area minus
Proposed Area) of (-) 1217 s.f. which equates to (-) 0.028 acres reduction in area, it would potentially
decrease the proposed amount of runoff for this area. However, given the small magnitude of this
reduction in area, the associated runoff that would result is insignificant. With this in mind there is no
change to the runoff amount that gets directed toward the bioretention facility located on the
northwest side ot the parking lot and therefore no change to the SWQMP will be required for Permit
No. GR2017-0004.
