LIMITED ASBESTOS SURVEY: 2262 Avenida Magnifica Building 9, Carlsbad, CA 92008
Dear Mr. Wilcox,
At your request, Environmental Testing Associates (ETA) conducted bulk sampling of suspect asbestos containing
materials (ACM) in the above referenced residential building structure. The samples were delivered under proper
chain of custody to an accredited laboratory for analysis. The objective of the investigation was to determine the
possible presence of asbestos within the identified damaged building materials at the subject property.
1.0 SAMPLING FOR ASBESTOS AND PLM ANALYSIS RESULTS
ETA representative Mr. Kevin Riley, CAC 17-5871, NV-IJPM-1674 conducted the sampling activities. Mr. Riley
is a State of California—DOSH-Certified Asbestos Consultant.
Sampling of suspect-ACM was conducted on July 28, 2018, following the provisions of 40 CFR Part 763-86. ETA
collected 21 bulk samples and delivered them to LA Testing Laboratory an accredited laboratory located in South
Pasadena, California. Due to multiple layers a total of 34 suspect materials were analyzed by the lab.
Analysis of suspect ACM samples for asbestos was performed per Appendix A, Subpart F, 40 CFR Part 763,
Section 1, Polarized Light Microscopy, by LA Testing which is accredited by the National Voluntary Laboratory
Accreditation Program for asbestos analysis of bulk samples using polarized light microscopy.

This checklist is intended to assist building permit applicants identify which Climate Action Plan (CAP) ordinance
requirements apply to their projects. Unless none of the requirements apply, the completed checklist must be
included in the building permit application. It may be necessary to supplement the completed checklist with
