A representative of GeoSoils, Inc. observed on site soil and footing conditions. Soil conditions in the trench
are generally free of loose soil and debris, non-yielding and uniform, and plumb; and are in general
conformance with those indicated in the geotechnical report.
A representative of GeoSoils, Inc. observed and reviewed footing excavation depth/width. Footing
excavations generally extend to proper depth and bearing strata, and are in general conformance with
recommendations of the geotechnical report.
A representative of GeoSoils, Inc. reviewed footing setbacks from slope face (if applicable). The setback was
in general accordance with the recommendations of the geotechnical report.

NOTES TO SUPERINTENDENT/FOREMAN
1. Footing excavations should be cleaned of loose debris and thoroughly moistened just prior to placing concrete.
2. Based on expansion potential of underlying soils, presoaking of soil below slabs may be recommended. Consult the
geotechnical report for presoaking recommendations. We note that clayey soils may take an extended period time for
such, and the contractor should schedule accordingly.
3. In the event of a site change subsequent to our footing observation and prior to concrete placement (i.e. heavy rain, etc.),
we should be contacted to perform additional site observations and/or testing.
4. This memo does not confirm the minimum footing dimension as required by the project structural engineer's design, if
different from the geotechnical report.

An asbestos survey was performed by JMR Environmental Services, Inc. (JMR) at 2646
State St., Carlsbad CA 92008 on November 19, 2015.
The purpose of this survey was to determine the presence of friable (easily crumbled by
hand pressure) and non-friable asbest,)s-containing materials within the house. This
survey addressed the following types of building materials:
• 9 X 9 Floor Tile & Associated Black Felt Backing • 12 X 12 Floor Tile
• Sheet Vinyl • Drywall/Joint Compound
• 12" X 12" Ceiling Tile • Window Putty
• Roofing Material • Roof Penetration Mastic
• Transite Vent Pipe
This report includes laboratory results and a list of asbestos-containing materials with
estimates of square and linear footag1~. Recommendations as to the most practical
remedial action shall also be provided.
Procedure
Sample analysis of bulk materials was performed in accordance with EPA.600/R-93/116
Test Method: Method for the Determination of Asbestos in Bulk Building Materials.
This procedure involves examination of the samples with a petrographic microscope
utilizing polarized light, a procedure commonly known as polarized light microscopy
(PLM).
All results generated via bulk asbestoi; analysis are reported in terms of approximate
percent by volume. Laboratory results and field data sheets are attached.

Fair Moderate damage or deterioration. Material is f'Jreaking up into layers or beginning to come loose from the subMrate. The material is

Poor Sewn d11mage or deterioratkm. The material is non-cohesive. Pieces are dislodged and debris is evident. Non-friable materlsl has
become friable.
Unknown -Materials located but impossible to assess, i.e., underneath new finishes.
Discussion/Recommendations
All accessible areas were inspected for possible asbestos-containing materials. If
materials that are similar to those found to be asbestos containing are observed they
must be assumed positive until tested to prove otherwise.
Due to the non-destructive nature of the survey, inaccessible areas (such as inside
walls) were not sampled. If during renovation suspect materials are observed in these
areas, bulk sampling and analysis is recommended prior to resuming the renovation
activities.
9 X 9 Floor Tile
Asbestos fibers were detected in the 9 X 9 floor tile located in the living room under the
skip sheet and the west bedroom bottom layer. The 9 X 9 floor tile is in good condition.
Asbestos containing floor tile is considered to be non-friable with a low potential for fiber
release, unless aggressively disturbed as occurs in renovation or demolition. This
material must be removed in accordance with all local, state, and federal regulations
prior to renovation or demolition.
Window Putty
Asbestos fibers were detected in the window putty material located on the exterior
windows. This material is in fair condition. Window putty is considered to be friable with

a low potential for fiber release while in place. If this material is disturbed, the fiber

Transite Vent Pipe
Asbestos fibers are assumed present in the transite vent pipe located at the wall heater
unit. The transite vent pipe is in good condition. This material is non-friable with low
potential for fiber release unless broken, sanded, or otherwise disturbed. This material
must be removed in accordance with all local, state, and federal regulations prior to
renovation or demolition.

Penetration Mastic
Asbestos fibers were assumed present in the penetration mastic. The penetration
mastic appears to be good condition. This material was identified at roof penetrations
(vents, ducts, etc.) on the roof. This material is non-friable and presents a very low
potential for fiber release if intact. This material must be removed in accordance with all
local, state, and federal regulations prior to renovation or demolition.

If asbestos-containing materials are to be removed or disturbed for demolition or
renovation then a licensed, Californi.~ Division of Safety and Health (Cal/OSHA)
registered asbestos abatement contractor must be used and the work must conform to
Title 8, Section 1529 of the California Code of Regulations. If asbestos is present in
quantities greater that 1 % then all work must also conform to Federal Environmental
Protection Agency (EPA) regulations Title 40 CFR 61 Final Rule, and Federal
Occupational Safety and Health Adrninistration (OSHA) regulations Title 29 CFR
1926.1101. Various other federal, state, or local regulations may apply as well.

EA/Econ Renovations,
An investigation to identify the presence of lead-based paint (LBP) was performed by
JMR Environmental Services Inc. (JMR) at 2646 State Street., Carlsbad CA 92008, on
November 19, 2015.
JMR's survey included a thorough inspection of the subject house. Every room in the
house was tested, including the exteriors of structure. The purpose of this survey was to
determine the presence and assess the co111dition of LBP on fixed and painted surfaces.
Many building components at this site appeared to be homogeneous. The buildings
located on these properties are currently scheduled for demolition.
Equipment used to perform the survey was a Niten XLp 300 Analyzer.
This report includes XRF results, paint chip results, and a site map. Also attached are a
summary of Cal/OSHA's Lead in Construction Standard Title 8, CCR Section 1532.1 and a
copy of the California Department of Public Health (CDPH) Inspector's Certification.
Lead Standards
Several government agencies have definitions for what amount of lead constitutes paint
to be considered "lead-based." The Department of Housing and Urban Development
(HUD) has designated the amount to be 1.0 mg/cm2, via XRF, or 5,000 ppm, via Atomic
Absorption Spectrometry (AAS). The HUD regulations are related to potential hazards
in the home environment and not to construction activities.
CDPH has created lead-related regulations that have been adopted into Title 17 of the
California Code of Regulations. These regulations also define LBP as surface coatings
containing lead concentrations of 1.0 mg/cm2, via XRF, or 5,000 ppm, via AAS. The
CDPH regulations apply only to lead hazard abatement activities.

The Consumer Product Safety Commission (CPSC) has defined lead-based paint as
surface coatings containing lead concentrations greater than 90 ppm, via AAS. In 1978,
the CPSC banned the sale of lead-based paint to consumers and the use of lead-based
paint in residences and public buildings.
The California Department of Occupational Safety and Health (Cal/OSHA) Lead in
Construction Standard (Title 8, CCR Section 1532.1) requires contractors to protect their
workers when there is a potential for lead exposure due to the construction activities.
Currently, Cal/OSHA has no lead concentration listed where contractors are exempt from
Title 8, CCR Section 1532.1.
Test Procedures
Representative fixed, painted, surfaces were tested using the Niten Xlp 300 Analyzer.
Results greater than 0.5 mg/cm2 are considered to be positive for lead. Paint chip
samples are collected from representative components when XRF results are
inconclusive. Paint chip samples are also collected from damaged paint with negative
XRF results. Paint chip samples are then analyzed in a laboratory using Atomic
Absorption Spectrometry (AAS). A total of zero paint chip sample(s) were collected
during this investigation.
Sites are surveyed using the clockwise theory. When using this theory the inspector must
first identify the main entry point of the unit, enter at that location, sample that room first,
and then sample all remaining rooms in a clockwise rotation. The wall numbers are also
identified using the clockwise theory. Walls are numbered in the following manner.
When an inspector is standing in the entry way to a room, that entry wall is wall #A; the
wall immediately to the inspector's left is wall #B; the wall opposite the inspector is wall
#C; and the wall to the inspector's right is wall #D.
XRF assays were performed using the "unlimited" mode. At this setting the XRF
continues to assay the substrate until is has determined the painted surface to be either
positive or negative with a confidence level of 95%. XRF assay results are given on the
following pages. Positive assay results are shaded.
Discussion
In this summary the tested surfaces are grouped into one of three ranges of lead content.
The purpose of this is to categorize the surface lead content as relatively high, mid-ranged,
or relatively low. This will help in complying with the Cal/OSHA lead standard as explained
later. Furthermore, this categorization identifies which surface coatings HUD, CDPH, and
CPSC consider "lead-based."

Based on XRF and laboratory results, the following surfaces are considered to contain
lead concentrations in excess of 0.05 mg/cm2 or 1,000 ppm.
• Interior -wood sills and window components. interior door jambs
• Kitchen -walls. ceiling. baseboards. ceramic countertop tile. wood cabinetry
• West bedroom -all walls and ceiling
• Exterior -walls. wood sills and window components. door jambs. columns, fascia
and eaves
A visual inspection determined several paints to be in "fair" or "poor" condition, including
but not necessarily limited to the following:
• Exterior eaves
•
Loose and peeling paints must be removed and remaining paint stabilized prior to building
demolition.
Please note that although some of the tested paints did not exceed the HUD/CDPH
definition of LBP, all site paints should be considered having a lead content requiring dust
controls and personal protection during building demolition or renovation, in compliance with
Title 8, CCR Section 1532.1. This is further explained in the following section.
Cal/OSHA Compliance Recommendations
Title 8, CCR Section 1532.1 requirements apply to all construction activities where lead
is disturbed and a potential for employee exposure exists. Since common lead analysis
methods have a minimum detection limit, a painted surface cannot be shown by
practical means to be completely lead-free. In effect, Cal/OSHA's requirements apply
to all paint that is disturbed by an employee. Newer paints often contain small amounts
of lead (up to 90 ppm is allowed) due to contamination during the manufacturing
process.
When any amount of lead is disturbed an employee initial exposure assessment must
be performed. If an employer has exposure data (air sample results) from similar
workplace operations and conditions within the past 12 months then this data can be
utilized.
If employee exposure data from similar workplace operations are not available, then
initial air monitoring is required. Several provisions are required during the initial air
monitoring including respiratory protection, protective clothing, change areas with
storage facilities, hand washing facilities, biological monitoring, and various types of
training (see the included summary of Title 8, CCR Section 1532.1 ).
We recommend initial air monitoring begin in areas where painted surfaces have higher

lead concentrations so that the results, if favorable, can apply to other painted surfaces
with similar or lower lead concentrations (if the work practices are similar). Favorable
results would be those that are below the a¢tion level of 30 micrograms per cubic meter
of air (µg/m3).
Inspection Limitations
This investigation was planned, develop¢d, and implemented based on previous
experience in performing LBP investigatiC1>ns. This inspection was patterned after
Chapter 7 of the HUD Guidelines for the Evaluation and Control of Lead-Based Paint
Hazards in Housing (1997 Revision). JMR utilized state of the art practices and
techniques in accordance with regulatory standards while performing this investigation.
Evaluation of the relative risk of exposure to lead identified during this investigation is
based on conditions observed at the time of the investigation. JMR cannot be
responsible for changing conditions that may alter the relative exposure risk or for future
changes in accepted methodology.

APCD requires a 10 day notification when performing demo as well. If I am not mistaken, the City of Carlsbad is
requiring you to provide the APCD demo notification for the final removal of the structure you mentioned. We
submitted a notification for the porch and eve demo only, attached. We did not submit a notification for the asbestos
removal because the asbestos was non-friable and it is not regulated by them. You will still need to provide the survey
and let them know the non-friable asbestos was already removed when submitting the demo notification. Please let me
know if you have any questions.

That's the key work "hazardous material "APCD only regulates Hazardous asbestos. We remove non -hazardous
asbestos (Non Friable) and the all the lead was all removed. So you have nothing on site once you get a demo and APCD
demo permit. We did get an APCD demo permit for porch and eve removal and give copy of asbestos report to them.
We can furnish non friable waste manifest, lead waste manifest, 10 day APCD demo notification permit copy and testing
results.
