Review and discuss the current transportation services provided through the Senior Center.

CARLSBAD SENIOR CENTER
TRANSPORTATION PROGRAM
Dear Transportation Participant:
EXHIBIT 1
Welcome to the Carlsbad Senior Center Transportation Program. We are very happy to
be able to serve you. As a participant, please allow me to share with you the general
guidelines that make our program operate.
QUALIFICATIONS
Seniors who are currently 60 years of age or older and who no longer have a driver's
license are eligible for transportation services. Transportation requests for seniors who
do have a driver's license but no longer have a vehicle available are also accepted.
Temporary transportation due to medical reasons may be approved with a doctor's
notice. Please call 602-4650 with any questions. Seniors must be able to safely enter
and exit the bus.
REGISTRATION
You must obtain a Transportation Participant Registration Card from the Senior Center
reception desk or one of the Senior Center Drivers. A card can be mailed to you upon
request.. The card must be completed (front and back). This information will be kept in
the strictest confidence, and is required by Aging & Independence Services, San Diego
County, that helps to fund this program. Should you have difficulty in completing the
card, ask the front desk or driver for assistance. Please notify the front desk if you will
require wheelchair lift. Wheelchair lift appointments may be limited based on the bus
availability.
TRANSPORTATION TO LUNCH
Because Carlsbad is such a large area to service we had to divide our service area into
certain days. The receptionist at the front desk or a driver will inform you of the days we
service your area.
Booking the bus automatically reserves lunch for you. Be sure to call the Senior Center
reception desk (760) 602-4650 before 8:30 a.m. to get on the bus pick-up list for the day
you desire to come to lunch. To cancel your ride to lunch, please do so prior to
8:30 a.m. Do not request bus pick-up and not be home. Precious time and gas is
wasted as well as keeping other seniors waiting for service.
Pick-up time is different each day because of; shopping, medical appointments,
prescription pick-up, etc. We cannot give you a specific time for pick-up. We request
that you be ready by 10:00 a.m. The bus will pull up to your residence and the driver will
blow the .horn and wait three (3) minutes. If the driver does not see you in that time, he
must leave as other seniors are waiting for pick-up service.
We also provide transportation after lunch on Tuesday's for people who want to stay and
play Bingo and on the 2nd Thursday of the month you can stay after lunch and enjoy our
monthly movie. Transportation will be provided.
Procedures
The driver cannot leave the vehicle unless previous arrangements have been made. If
you have a special circumstance, please discuss it with the office staff or your driver.

Please be patient. The bus driver is doing his or her very best. But don't worry; if you
have a reservation your lunch will be waiting for you.
MEDICAL APPOINTMENTS
Every week day afternoon , except Wednesday (shopping day) and first Thursday of the
month (WalMart) transportation service is available for scheduled doctor appointments,
(760) 602-4q50) on a first come first serve basis. The appointment should be
scheduled between 1 :30 and 2:30 p.m. and must end by 4:30 p.m. If your appointment
goes beyond 4:30 p.m. you must make other arrangements to get home. The city
vehicles must be in the city parking lot by 5:00 p.m. each day.
Please be ready when the Senior Center vehicle arrives. We suggest that you are ready
for pick-up at least one-half hour before your appointment. There is a $5. charge for this
service.
A minimum of a 24 hour notice is necessary for medical service and cannot be for;
medical emergencies, ongoing appointments (i.e., therapy, dialysis, allergy shots, etc).
Occasionally the driver will accomplish several missions while doing a medical
appointment; therefore he may not be waiting for you when your appointment is
completed. Please be patient, he will return to take you home. Please do not assume
the driver will take you to a lab, pharmacy etc., after your appointment.
Our service area for medical appointments is very limited. From the Ocean east to
Cedar and Vista Way (Tri City Medical area), South to Rancho Santa Fe Road (Scripps
Encinitas,) 500 Garden View and 477 N. El Camino, North to Mission Medical Park
(2201 Mission Avenue,) in Oceanside. Scripps lab. is now in Carlsbad on Salk Ave. (off
ECR).
SHOPPING
Wednesday afternoon (after lunch) limited transportation is available for grocery
shopping, except south Carlsbad. You may go to either Stater Brothers or Trader Joes.
A reservation is required (602-4650). It is also imperative to be ready when the driver
arrives to pick you up. Shopping on the 1st Thursday of the month is for Wal Mart only.
There is a 3 bag maximum per senior to ensure we have enough passenger space on
the bus.
SUGGESTED DONATION
We do not charge for transportation service however, we do request a suggested
donation of $2.00 per one way trip.
All donations are welcomed. All funds donated will go directly into the transportation
program to help us provide continued service.
We are pleased that you are using this service (tell a friend). The transportation
program appears to have many do's and don'ts, however once you become familiar with
the program it will work for you. For transportation questions and information do not
