This form is required to participate in the city’s Building Permit Inspection Self-Certification Program (SCP), which
allows for a California-licensed contractor to self-certify that certain residential building construction projects were
completed in compliance with relevant building code standards in-lieu of a final inspection from the city. Refer to
Informational Bulletin IB-270 for additional information and requirements. This form is to be completed and
submitted with a Residential Building Permit application (form B-1).

I affirm and certify under penalty of perjury that:
1. I have reviewed and understand the requirements of the City of Carlsbad’s SCP program as detailed in
Informational-Bulletin IB-270 and will comply with those requirements.
2. I have received approval from the property owner to complete the work authorized under the referenced
building permit and made them aware that completed work will be self-certified under this program.
3. I am a California-licensed B General Building Contractor with current worker’s compensation insurance, C-
type Contractor depending on specialty with current worker’s compensation insurance, Civil Engineer or
Structural Engineer, and my license in active.
4. I shall assume responsibility and liability for the overall installation, inspection, and certification of all work
associated with the referenced building permit and will ensure that all work is completed in compliance
with all applicable requirements of the California Building Standards Code as adopted and amended by the
City of Carlsbad and in accordance with any manufacturer’s installation requirements.
5. I have reviewed and understand the requirements set forth in the Self-Certification Inspection Checklist
