On behalf of Verizon Wireless, we are submitting a Consistency Determination application to
allow for antenna modification at their existing site. The project site is located at 2020 Cassia
Road.
Verizon is proposing to remove and replace six (6) antennas and nine (9) RRU's within the
existing cupola, along with adding two (2) new Raycaps to the existing Wireless Communication
Facility. No changes are proposed to the existing equipment area. The property is zoned as
Office.
Included with this submittal are the following documents:
• 2-full size sets of ZD's
• 1-ll"xl 7" sets of ZD's
• 1-Signed application form (Consistency Determination)
• 1 Photo Simulation
• 1 -Authorized Agent letter from VZW
