Certification Statement:
I Certify that I am the Legal Business Owner of the subject business and that all of the above information is true
and correct to the best of my knowledge.I agree to accept and abide by any conditions placed on the subject
project as a result of approval of this application.I agree to indemnify,hold harmless,and defend the City of
Carlsbad and its officers and employees from all claims, damage or liability to persons or property arising from or
caused directly or indirectly by the installation or placement of the subject property on the public sidewalk and/or
the operation of the subject business on the public sidewalk pursuant to this permit unless the damage or liability
was caused by the sole active negligence of the City of Carlsbad or its officers or employees.I have submitted a
Certificate of Insurance to the City of Carlsbad in the amount of one million dollars issued by a company which has
a rating in the latest "Best's Rating Guide" of "A-"or better and a financial size of $50-$100 (currently class VII) or
better which lists the City of Carlsbad as "additional insured" and provides primary coverage to the City.I also
agree to notify the City of Carlsbad thirty days prior to any cancellation or expiration of the policy.The notice shall

Description/Explanation:
Pipeline Expansion Segment 5 is part of the Carlsbad Municipal Water District's (CMWD)
$37 million Phase III Recycled Water Program and will provide recycled water to
customers along El Camino Real (N. of Chestnut), Kelly, Tamarack, Cannon, Marron,
Haymar.
CMWD started its recycled water program in 1990 with Phase I and by 1995 CMWD
served over 1,000 acre-feet per year (afy) of recycled water.In 2000, Phase II was
implemented with the construction of the 4-million gallons per day (mgd) Carlsbad Water
Recycling Facility (CWRF). Phase II was completed in 2008 and increased service to
approximately 4,000 afy. The Phase III Program began in 2012 with the preparation of a
feasibility study and includes the expansion of the CWRF,18 miles of new pipeline, and
the construction of a 1.5 million gallon storage tank. Only the storage tank and 8 miles of
pipeline remain to be constructed.
Pipeline Expansion Segment 5 is located along El Camino Real (N. of Chestnut), Kelly,
Tamarack, Cannon,.This project includes the construction of approximately 11,077
linear feet of 8 -inch diameter and 32,661 linear feet of 6-in diameter polyvinyl chloride
(PVC) pipeline and recycled water services. Recycled water in these pipeline segments
will be used for landscape irrigation.
